PURPOSE: Alfuzosin XL (Sanofi-Aventis, UroXatral(R)) has to be taken just after a meal for the best efficacy due to its specific pharmacokinetics. We studied patient compliance and associated factors with regard to the correct instructions for taking alfuzosin XL. MATERIALS AND METHODS: Alfuzosin XL was prescribed to 62 patients from February to May 2008. At the first visit, we provided a prescription for alfuzosin XL with the instructions to "take one tablet just after dinner once a day". At the second visit, we evaluated patient compliance and the factors that influenced the patient compliance for taking the alfuzosin XL. The physician explained the instructions to the patients. At the third visit, we compared the compliance and associated factors with the results from the first visit. RESULTS: A total of 50 patients completed the study. After the first visit, twenty one patients (42.0%) were not taking the alfuzosin XL according to the prescription, that is, 20 patients took alfuzosin XL before going to bed, and one patient before meals. (p<0.05) After direct instructions by a physician, 49 patients (98.0%) took the alfuzosin XL correctly. (p<0.001) CONCLUSIONS: Taking alfuzosin XL as prescribed was accomplished in only 58% of patients. When alfuzosin XL is prescribed, the physicians should be aware of the importance of providing instructions directly to patients.

Background: There has been an increasing trend in reported sexual abuse patients in the Philippines in the past 20 years. Patient evaluation is critical from health care providers and it is imperative to have an acute care protocol that heath care providers can use in managing these patients. Objective: To assess the adherence to the acute care protocol as applied to adult female sexual abuse patients who consulted at the Philippine General Hospital OB Admitting Section from March to August 2019 Methods: The study design used was an observational, cross-sectional study via descriptive analysis using a designed questionnaire. Relevant data regarding compliance to the acute care protocol was assessed from patient experience via the questionnaire after undergoing the standard services of the hospital. Data was then documented, tabulated and processed via Microsoft Excel data sheets. Results: In terms of patient perspective, 96.2 % of all cases (n=27) were provided service by OBGYNs and psychiatrists and 100% were assessed by social workers. Although stated in the protocol, legal and/or police assistance was not provided by the hospital during patient consult. Conclusion The institution was able to adhere to the acute care protocol services of patients in terms of obstetric and gynecologic, psychiatric and social worker services but not legal and/or police assistance.

PURPOSE: It has been well known that doctor-patient communication is a key to the better doctor-patient relationship. However, in Korea, there has not been much researches on the causal factors influencing the doctor-patient communication as well as on patient satisfaction and compliance. This study attempts to investigate the relationships between patient social styles and doctors' communication styles as well as patient outcomes. METHODS: Patient subjects were recruited from two mid-size local hospitals. The questionnaires consist of social styles, patient-perceived doctors' communication, and patient satisfaction/compliance measures. One hundred ninety useable data were analyzed. RESULTS: For those highly satisfied patients, patients with analytic style showed the highest percentage of viewing their doctors as highly empathic. On the other hand, for those patients with high compliance, expressive style patients showed the highest rates of regarding their doctors as highly affective. And amiable style patients are most likely to view their doctors as highly cognitive. For analytic style patients, cognitive empathy had positive effects on patient satisfaction. For amiable style patients, doctors' cognitive empathy had positive influence on patient compliance. For expressive style patients, doctors' affective empathic communication had positive effects on both patient satisfaction/compliance while cognitive had positive effects only on patient compliance. CONCLUSION: The results show that the different social styles of patients might influence on the fact how the patients perceive their doctors' communication as well as how much they are satisfied and compliant. Thus, when we as an medical educators need to realize the importance of this mechanism and bring this learning into classrooms.

Patient noncompliance with the physician's prescribed therapeutic regimen is a serious problem in glaucoma because of the possibility of blindness. In this report the compliance with medical treatment of 67 patients with chronic glaucoma was studied for 2 years and 45% of the patient was identified as noncomplier. The authors' observations relative to noncompliance were lack of the knowledge of disease and its prognosis, little concern about disease, side effects of medication, improper timing and increased number of medication dosage, long-term use of medication, improperties of the physician-patient interaction, and other serious medical disorder, etc. The noncompliance does not correlate with the patient's age, sex, education, marital status, previous surgery for glaucoma,concerh of family, and increased waiting time to see the doctor.

OBJECTIVE: To explore the effect of reinforced health education on deep radiofrequency thermotherapy for patients with tumor. METHODS: From June 2012 to June 2014, 106 patients who underwent deep radiofrequency thermotherapy in our hospital were randomly selected, and were divided into a control group (n=69) and an observation group (n=65). The observation group received reinforced health education while the control group received the traditional health education before treatment. Th e reinforced health education included preparation, cooperation and health behavior during and aft er treatment. Th en the compliance rate, degree of satisfaction, and the awareness rate of related knowledge were compared and analyzed aft er treatment between the 2 groups. RESULTS: Th e compliance rate in the observation group and the control group was 85.51% and 63.08% respectively; there was significant difference in the awareness rates of related knowledge and the overall degree of satisfaction between the patients and their family members (all P<0.05). CONCLUSION Implement of reinforced health education is benefit to patients to understand the content of health education before treatment and keep health behavior after treatment, and can also improve the compliance, the degree of satisfaction in the deep radiofrequency thermotherapy for patients with tumor.

PURPOSE: This study aimed to identify the effect of individualized education on stroke patients' post-discharge anxiety and self-care compliance. METHODS: It was a pretest-posttest design with non-equivalent groups. A total of 32 hospitalized stroke patients in experimental group were given individualized education three times at one day before, the day of and one week after discharge. Data were analyzed using descriptive statistics, t-test and chi2-test. RESULTS: The scores of anxiety at posttest were 45.66+/-9.58 in control group and 35.44+/-8.21 in the experimental group. Self-compliance scores were 44.00+/-7.99 in control and 61.06+/-7.69 in the experimental group. These indicate that anxiety score is significantly lower (t=-4.58, p<.001), and self-compliance score is significantly higher (t=8.70, p<.001) in experimental group. CONCLUSION: If stroke patients receive a relevant individualized education, it could help reduce their anxiety. Also it would be useful to maintain and improve their self-care compliance after discharge.

PURPOSE: The purpose of this study was to identity factors affecting patient adherence and to develop an explanatory model for patient adherence in patients with spinal cord injury. 8 variables that were based on the previous research and a review of literature were used to construct hypothetical model. Social support, economic status, perceived barrier, patient provider relationship and rehabilitation related knowledge were the exogenous variables, depression, self-efficacy and patient adherence were the endogenous variables. METHODS: Data form 117 patients with SCI were analysed to test the hypothetical model, using SAS and LISREL 8.53 program. RESULTS: The overall fitness of the model was good (GFI=.991, AGFI=.915, NNFI=1.299, NFI=.953, p=.632) Depression, powerlessness, economic status were the strong factors influencing patient adherence. Powerlessness was significant factors for self-efficacy. CONCLUSION: To improve of patient adherence should focus on nursing intervention for depression, powerlessness and economic status.

BACKGROUND: Dialysis adequacy indexed by Kt/V in hemodialysis patients is recommended as a single-pool Kt/V of at least 1.2 per session thrice weekly. But many patients can not achieve this adequacy target. Although dialysis time is the most important as a factor influencing Kt/V, it is difficult to prolong dialysis time in practice because of its economic impact and poor patient compliance. The aim of this study is to investigate the effect of increasing blood flow rate on dialysis adequacy in hemodialysis patients with low Kt/V. METHODS: This study enrolled 36 hemodialysis patients with single-pool Kt/V less than 1.2 per session thrice weekly, which was measured in dialyzer blood flow rate of 200-230 mL/min. We increased 15% of blood flow rate in patients less than 65 kg of body weight and 20% in patients more than 65 kg. And then we compared Kt/V and urea reduction ratio (URR) between before and after increasing blood flow rate. RESULTS: The mean age was 48+/-11 years (23-73 years) and the number of male was 25. Of the total patients, 24 patients had dry weight less than 65 kg. Mean dialysis duration was 52+/-50 months (3-216 months). Mean Kt/V before increasing blood flow rate was 1.02+/-0.09. It increased to 1.14+/-0.12 after increasing blood flow rate (p<0.001). Of the total 36 patients, 13 patients (36.1%) achieved adequacy target (Kt/V> or =1.2). Mean URR before increasing blood flow rate was 56.9+/-4.0%. It also increased to 60.8+/-4.1% (p<0.001). CONCLUSION: Our data suggest that increment of blood flow rate to 15-20% of previous flow rate is effective in achieving dialysis adequacy in patients with low Kt/V.
