3.Three Cases of Retronychia.

Retroncyhia shows ingrowth of the nail toward the proximal nail folds. Initial events start from minor trauma of the digital pulp that disturbs longitudinal nail growth. This disturbed growth of the nail changes the growth direction to the proximal nail folds, leading to persistent paronychia, formation of granulation tissue, and other nail changes. Simple avulsion is curative and can improve most symptoms. There are few reports about retronychia in the English literature, and we report 3 cases of retronychia here.

Deep palmoplantar warts (Myrmecia) are frequently tender papules or plaques caused by HPV-1. Myrmecia are usually misdiagnosed as paronychia or digital mucous cysts. They occur not only on the palms and soles but also on the lateral aspects and tips of the fingers and toes. We report a rare case of deep palmoplantar wart occurring on proximal nail fold, unusual site of right 5th finger in an 11-year-old girl.

5.A Case of Chronic Paronychia Accompanied by Cutaneous Ossification.

Cutaneous bone formation may be primary or secondary. If it is primary, there are no preceding cutaneous lesions. If it is secondary, bone forms through metaplasia within a preexisting lesion caused by inflammation, traumatic injury, and a neoplastic tumor. Paronychia is inflammation of the proximal nail fold and presents as painful periungual erythema, sometimes with associated purulence. Chronic paronychia is most commonly related to mechanical or chemical factors. Here, we report a case of chronic paronychia accompanied by cutaneous ossification in a 33-year-old woman who presented with a 2-year history of recurrent paronychia on the left first finger.

6.A Case of Green Nail Syndrome.

Green nail syndrome is characterized by greenish discoloration of the nail. It is caused by Pseudomonas aeruginosa and mostly associated with paronychia in persons whose hands are frequently exposed to water. Trauma of the nail fold or nail plate is another predisposing factor of this infection. Here we report a case of green nail syndrome in a 27-year-old man who develops asymptomatic greenish discoloration and fragile thickening of the nail plate on his left great toe after trauma.

7.Green Nail Syndrome Treated with the Application of Tobramycin Eye Drop.

Green nail syndrome (chromonychia) is a nail disorder characterized by onycholysis and green-black discoloration of the nail bed. This condition is often associated with chronic paronychia. Pseudomonas aeruginosa is the most commonly identified organism in cultures from the affected area. Despite the various treatment options available, removal of the nail is still necessary in many cases. A 35-year-old man presented with dark-greenish discoloration of the nail plate and onycholysis on the left thumbnail. He had been treated with oral antifungal and antibiotic agents for several months; however, the lesion showed no improvement. The diagnosis of green nail syndrome was established after a positive bacterial culture, and on the basis of the antibiotic sensitivity test result, tobramycin eye drop (Tobrex(R)) was then prescribed. Three weeks later, the nail discoloration almost vanished but the onycholysis remained. Herein, we recommend the application of tobramycin eye drop as an easy and safe treatment option for green nail syndrome.

9.A Probable Case of Alternarial Onychomycosis.

Alternaria is a common saprophyte that is not usually pathogenic in humans. Alternarial onychomycosis is very rare and it is difficult to make a diagnosis of onycholycosis by moulds. In Korean literature, clinical findings of onychomycosis by Scopulariopsis brevicaulis, Aspergillus sp., Fusarium sp. were reported. But clinical finding of alternarial onychomycosis has not been reported. We report a probable case of alternarial onychomycosis in a 67-year-old Korean farmer. Some of the infected nails showed paronychia, onychia, loss of nail plate, black discoloration of proximal part of the nail plate and proximal nail folds. And some of them showed combined findings of proximal subungual onychomycosis and distal and lateral subungual onychomycosis. Repeated microscopic findings showing typical macroconidia and brownish hyphae of Alternaria were observed on three consecutive KOH smears with one-week interval. But cultures were not successful. Extraction of nail plates with oral antifungal and antibiotic treatment was only partially effective.

10.Metastatic Bone Tumor in Hand: Report of 2 Cases.

The metastatic malignant tumor of the hand is rare condition, and has difficulty in diagnosis at presentation due to simulating other diseases such as osteomyelitis, felon, paronychia, rheumatoid arthritis, and so forth. The treatment of them is very limited and the prognosis is poor. We experienced and going to report two cases of metastaic tumor of the hand, one was from rectal cancer which associated with rheumatoid arthritis and another was from brochogenic carcinoma.
