1.Subcutaneous Angiolymphoid Hyperplasia of the Inguinal Region and Anteromedial Side of the Proximal Thigh: A case report

Subcutaneous angiolymphoid hyperplasia is an uncommon disease characterized by eosinophilia, proliferation of the capillary vessels and lymphoid tissue and infiltration of the inflammatory cell, especially eosinophils. The etiology of the disease are obscure, but probably trauma, lower grade infection, nervous factor & hormonal status etc. The predilection sites are face, ears, scalf & neck, but rarely reported in the extremities. We experienced a case of subcutaneous angiolymphoid hyperplasia of the inguinal region and anteromedial side of the proximal thigh in a 10 year-old man who complained pain and tenderness on the above region. The patient was treated by excision with satisfactory result.

2.Clinical Study of Acetabular Fracture

It appears that frequency of fractures of the aceta ulum is increasing with the number of automobiles on our roads and when they occur they seem to pose difficulties of management. Because, the anatomy of this region is complicated, making surgical approaches difficult. Second, the patients often have major associated injuries, making early operative treatment hazardous. Third, the fractures are often severly comminuted, leading many surgeons to beleave that operative reduction is impossible. Finally, and unfortunately, the fractures are often missed. The aim of treatment must be to restore the fractured acetabulum to its normal anatomy followed by early motion to promote healing and restore function. Undisplaced acetabular fractures have a good prosis with conservative treatment but patients with displaced fractures of the acetabulum not reduced by manipulation and traction should be considered candidates for open reduction. The author experienced 72 cases of acetabular fracture patients who were admitted to the department of Orthopaedic Surgery of Catholic Medical College and Center from January 1979 to August 1983. The results of 48 patients who were followed up over 6 months period were as follows; l. Among 72 cases(44 were male and 28 were female), the most common causes of acetabular fracture was pedestrian struck by car. 2. 56 were treated conservatively and 16 were treated surgically. The result were as follows; Excellent-15(31%), Good-23(48%), Fair-8(17%), Poor-2(4%). 3. 72 cases were classified by Letournel classification. The most common type was posterior wall fracture(14 cases), and second most was T-shaped fracture(13 cases). 4. If the grossly displaced fragments are present they should be reduced and fixed surgically if surgical approach can be done. 5. It is essential to understand the pathologic anatomy of the acetabulum in order to approach the acetabular fracture sefely and with maximum ease.

3.EEG Abnormalities in Henoch-Schonlein Purpura.

Schonlein-Henoch purpura is a generalized small vessel vasculitis characterized by nonthrombocytopenic purpura, arthritis, abdominal pain and nephritis. In 1914 Osler described an allergic purpura associated with hemiplegia. After then Lewis et al. reported the cases of Schonlein-Henoch purpura associated with convulsion, coma, confusion, intracranial hemorrhage, and chorea, CNS complication has been reported in 1-8% of children and subsided spontaneously in most cases. Headache is a remarkable sympton and appears nonspecific nature. In 1991 Ostergaard and Storm reported that headache occured during the first week following skin rash and frequently showed abnormal EEG findings. We investigated prospectively the presence of a possible cerebral and renal involvement in the case of Schonlein-Henoch purpura. EEG abnormality demonstrated in 52.6% of all cases, and headache or irritability in 47.4% of all cases. A significant association was found between abnormal EEG finding and presence of headache, but was not found between EEG findings and presence of renal involvement and hypertension. Patients with abnormal EEG had no Past or famity history of febrile convulsion or ididopathic epilepsy.

4.CT Findings of Palpable Neck Masses in Children.

PURPOSE: We performed this study to assess the value of CT in the differential diagnosis of palpable neck masses in children. MATERIALS AND METHODS: We retrospectively reviewed the CT scans of the palpable neck masses in 30 children. The masses were proved histopathologically and classified into cystic, solid, and inflammatory mass and their CT findings were analyzed. RESULTS: Twelve cases were cystic masses, 4 were solid masses, and 14 were inflammatory lesions. Cystic masses included cystic lymphangiomas (n=6), branchial cleft cysts (n=3), thyroglossal duct cysts (n=2), and ranula (n=l). Cystic lymphangiomas showed insinuating appearances into adjacent structures and 4 cases occurred in the posterior cervical space. All branchial cleft cysts were round cystic masses with smooth wall and displaced the submandibular gland anteriorly and the sternocleidomastoid muscle posteriorly. Two thyroglossal duct cysts occurred centrally adjacent to the hyoid bone and 1 ranula in the submental area. Solid masses were juvenile hemangioma, pleomorphic adenoma in submandibular gland, neurilemmoma, and fibromatosis colli. Juvenile hemangioma showed well-enhancing mass with indistinct margin and the other solid masses had well-defined margin with their characteristic location. Inflammatory lesions were abscess (n=4), deep neck infections with lymphadenopathy (n=4), submandibular gland inflammation (n=3), and tuberculous lymphadenitis (n=3) and they showed strand-like enhancement in adjacent subcutaneous tissues. Tuberculous lymphadenitis had multiple lymph node enlargement with internal low attenuation areas and showed less surrounding strand-like enhancement than suppurative lymphadenopathies. CONCLUSION: Most neck masses in infants and children were of congenital or inflammatory origin. CT is useful for the evaluation of the child presenting with a neck mass, because it can differentiate various forms of neck masses and is able to reveal the relationship of the masses to the adjacent structures with their characteristic location.

5.Transcatheter Instillation of Urokinase into Loculated Pleural Effusion: Analysis of Treatment Effect.

PURPOSE: To evaluate the indication for intracavitary Urokinase(UK) in the treatment of Ioculated pleural effusion. MATERIALS AND METHODS: We analyzed CT and US in 31 patients who were treated with intracavitary UK in Ioculated pleural effusion. In each patient, a single chest catheter (10-12F) was insected under imaging guidance. When the amount of drainage was less than 100ml/day, UK was instilled through the catheter until less than 50ml/day was drained. On follow-up chest radiographs of more than 1 month, we classified the results of treatment into 3 groups:(1) completely effective (lung expansion, over 80%);(2) partially effective (20-80%); (3) ineffective (below 20%) group. Sonographic pattern of pleural fluid was classified into anechoic, septated, and honeycomb appearances and the thickness of parietal pleura was measured on CT. RESULTS: Sixteen patients were completely effective, nine were partially effective, and six were ineffective. patients with completely or partially effective outcome had anechoic and linear septated appearance on had less than 4mm of parietal pleural thickness on CT. Of six ineffective patients, US showed linear septated in one patient and honeycomb appearance in five patients and the thickness of parietal pleura on CT was 3 mm in one patient, 4ram in two patients, 5mm in one patient, and 6mm in two patients. CONCLUSION: UK instillation through percutaneous catheter was an effective method in the treatment of Ioculated pleural effusion. However, we found near complete reaccumulation of pleural fluid when honeycomb appearance of pleural fluid on US or more than 5mm parietal pleural thickness on CT was observed, which might suggest that we should consider the other kinds of treatment method in those patients.

6.The Clinical Significancy of the Osteomedullography and Bone Scanning with Radioactive Isotopes in Open Fractures of the Shaft

In the orthopedic Department of Jeonbug National University Hospital, twenty-five open tibial shaft fractures were reviewed and analyzed with osteomedullography and bone scanning with radioactive isotopes from June 1981 to October 1983. The results are as follows. 1. The cases of injury were mostly traffic accident(70.8%) and majority was found between second decade and third decade. Males were 11 times more frequent than females. 2. In the shape and location of fractures, comminuted fracture(60%) was most common, and fracture of middle one third(48%), lower one third(28%) were in order. 3. In the open tibial fractures, Grade I. of soft tissue injury was most frequent(64%). As inthe plate fixation was required, we should it placed atypically on the posterior surface of the tibia. 4. In the osteomedullography at 3 months after treatment, the rate of positive finding was 80 and intraosseous crossing vein(Kaski sign 1) was observed most frequently. 5. In the profile of the bone scanning with radioactive isotopes, the rate of single-peak uptake was 76% and twin-peak uptake was 24%. 6. In the negative Osteomedullogram and twin-peak uptake on the profile of the bone scanning, bone graft was necessary. 7. In the case of difficult diagnosis for union process of tibia fracture, osteomedullography and bone scanning with radioactive isotopes were considered useful method of early diagnosis.

This study was conducted to study the job interest and job value of medical freshmen. The job interest test and job value test by the Korea Employment Information Service of the Ministry of Employment and Labor were conducted on 149 medical freshmen enrolled in a medical school at 2019 and 2020. Among six types of job interest, investigative type was most suitable for the medical freshmen, followed by realistic type, enterprising type, social type. Among 13 types of job values, job stability, body and mind space, recognition, and financial compensation were higher values than the others in order. We expect that this result will be helpful to establish the career identity of medical students, and to construct the system of curriculum or counseling programs for career developing of them.

9.Ex Vivo Sentinel Node Mapping in Gastric Cancer.

PURPOSE: Lymph node analysis is essential for staging gastric cancer. Intraoperative lymphatic mapping and sentinel lymphadenectomy have not yet been investigated for most gastrointestinal neoplasms. The purpose of this study is to evaluate the usefulness of ex vivo lymphatic mapping in patients with gastric cancer. METHODS: 42 patients with gastric cancer underwent ex vivo lymphatic mapping and sentinel lymph node (SN) biopsy after standard surgical resection from March 2002 to September 2002. Within 5 minutes of resection, stomach specimens were injected submucosally around the tumor with isosulfan blue dye. Blue lymphatic channels were identified and followed to the blue-stained SN (s) which were harvested. The specimen was fixed in formalin and subsequently analyzed in the usual fashion. RESULTS: At least one SN was identified in 39 patients (92.9%). The average number of SNs identified was 2.5 (range, 1~6), and the average number of nodes in each gastric cancer specimen was 23.4 (range, 13~55). 14 patients had lymph nodes containing metastatic disease. 9 patients had metastasis in both sentinel and nonsentinel node. In 5 patients the sentinel nodes was negative for disease, whereas the nonsentinel lymph nodes contained metastatic disease (false negative rate=35.7%). Of these 5 patient, one may have skip metastasis and four had metastasis on same rerional lymph node group. CONCLUSION: Ex vivo SN mapping of the stomach is technically feasible, but it is too early to provide a useful approach to evaluate lymph node metastasis.

10.Methylenetetrahydrofolate Reductase C677T and A1298C Polymorphisms in Colorectal Cancer.

PURPOSE: Recently, the role of vitamins, folate in particular, has been emphasized in the maintenance of health. Folate deficiency is known to give rise to developmental delay, pre-mature vascular disease, neural tube defects, acute leukemia, atherosclerotic vascular disease, delivery defects, breast cancers and gastrointestinal neoplasia. Methylenetetrahydrofolate reductase (MTHFR) is an essential enzyme in folate metabolism, and influences DNA synthesis and DNA methylation. Generally, a low folate level is known to be associated with gastrointestinal neoplasms. Also, the amino- acid-changing and enzyme-activity-reducing nucleotide polymorphism (677C-->T/Ala222Val) has been described in the MTHFR polymorphism and it brings about low enzyme activity, which may reduce DNA methylation and uracil misincorporation into DNA. These processes may be critical for the oncogenic transformation of human cells. Two common single nucleotide polymorphisms (SNPs) resulting in amino-acid changes (677C T/Ala222Val and 1298A C/Glu428Ala) have been described in MTHFR. We investigated the relation between the MTHFR C677T and A1298C polymorphisms derived from colorectal cancers and from controls in the Korean population. METHODS: One hundred forty-eight (148) individuals with colorectal cancer and 288 healthy persons were analyzed. Blood sampling of each group was performed by using a PCR- RFLP analysis, and MTHFR polymorphism genotypes of 677C/C, 677C/T, 677T/T, 1298AA, 1298AC, and 1298CC were obtained. RESULTS: The genotype frequencies of MTHFR C677T polymorphisms were 25.0% (CC), 48.0% (CT), 27.0% (TT), and 75.0% (CT+TT), respectively, in case patients and 39.2% (CC), 36.8% (CT), 24.0% (TT), and 60.8% (CT+TT) in controls. The genotype frequencies of MTHFR A1298C polymorphisms were 56.1% (AA), 372% (AC), 6.8% (CC), and 43.9% (AC+CC), respectively, in case patients and 55.6% (AA), 40.3% (AC), 4.2% (CC), and 44.4% (AC+CC) in controls. The 677TT and the 677CT genotypes were associated with significantly increased risks for colorectal cancer (adjusted OR=1.77 and 95% CI=1.02~3.04 in TT; adjusted OR=2.07 and 95% CI=1.28~3.35 in CT) than was the 677CC, genotype but the the 1298CC and 1298 AC genotypes were not associated with significantly increased risks for colorectal cancer (adjusted OR=1.75 and 95% CI= 0.71~4.26 in CC; adjusted OR=0.95 and 95% CI=0.62~1.45 in AC). CONCLUSIONS: The MTHFR C677T polymorphism may be influenced by colorectal cancer, but the role of the MTHFR A1298C polymorphism needs careful interpretation and confirmation in larger studies.
