2.Analysis of follow-up results one year after initial screening in high-risk area of nasopharyngeal carcinoma

Objective:To observe the differences in the dynamic change of the EB virus antibody between general population and first-degree relatives (FDR) of nasopharyngeal carcinoma patients during follow-up study one year after initial screening and discuss the difference among the nasopharyngeal carcinoma detections. Methods: Serologic data of all subjects that participated in the fol-low-up study were collected. Changes in EB virus antibodies were investigated and correlation of these changes with gender and age level was analyzed. Differences in the nasopharyngeal cancer detection rate of different populations were also compared. Results:NA1/IgA negative conversion rate was higher in the family group than in the control group (χ2=20.28, P<0.001). This rate was also higher in both male and female family groups than in the male and female control groups (χ2=22.59, P<0.001;χ2=4.03, P<0.05, respectively). NA1/IgA positive conversion rate was lower in the family group than in the control group (χ2=7.79, P<0.05). Likewise, this rate was lower in both male and female family groups than in the male and female control groups (χ2=9.46, P<0.05;χ2=0.74, P=0.39, respective-ly). VCA/IgA negative conversion rate was higher in the family group than in the control group (χ2=1.90, P<0.001). This rate was also higher in the male and female family groups than in the male and female control groups (χ2=7.50, P<0.05; χ2=no expression, P=0.108, determined by Fish exact test, respectively). VCA/IgA positive conversion rate was higher in the family group than in the control group (χ2=0.10, P=0.70). This rate was again higher in both male and female family groups than in the male and female control groups (χ2=0.02, P=0.90,χ2=0.51, P=0.48, respectively). Ten cases from the control group manifested nasopharyngeal carcinoma;the same disease was not observed in the family group. Nasopharyngeal carcinoma detection rate was significantly higher in the control group than in the family group, but the difference was not statistically significant (χ2=1.05, P=0.31). Conclusion:a. Reactivation of the EB virus is not closely linked with genetic factors. b. The detection rate of NPC in FDR was lower compared with the general population after initial screening;thus, the rule of selective follow-up is not applicable for FDR.
3.Microwave ablation for colorectal liver metastases

Colorectal liver metastases (CRLM) are one of the main causes of death among the patients with colorectal cancer,yet most patients with CRLM are unable to get radical resection.Fortunately,thermal ablation can yield long-term survival efficacy in this population of CRLM patients.Currently,thermal ablation is the first-line treatment among other ablation technologies,including radiofrequency ablation and microwave ablation (MWA).However,recent advancements in microwave ablation techniques increase consideration of this method in the treatment of CRLM,such as:reliable,fast,efficient and rarely affected by heat-sink effect.This article aims to review the mechanism,efficacy,complications of MWA in treating CRLM.
4.Effect of proinflammatory cytokines on growth factor expression of type n alveolar epithelial cells from neonate piglet lungs

Objective To establish a method of isolation, purification and identification of type Ⅱ alveolar epithelial cells (AEC- K ) from neonate piglet lungs of 1 ~ 3 days old and to investigate effects of proinflammatory cytokines on expression of growth factors (GFs). The yield, viability and purity of AEC- Ⅱ obtained using different enzyme digestion and purifying methods were compared. Methods After the first 24-hour culture of AEC- Ⅱ ,the media containing interleukin (IL)-1β,IL-6 and IGF-Ⅰ at different concentrations were used to culture AEC-Ⅱ for another 48 hours. And then the cells were counted and the expressions of insulin-like growth factor (IGF-Ⅰ ), platelet-derived growth factor ( PDGF), surfactant proteins (SP) -A and SP-B mRNA were determined by real time PCR. Results A significantly higher yield of AEC-Ⅱ was achieved by digesting the lung with 30 unit/ml elastase and 0.1 % trypsin at 37 t for 20 min, the yield was (5.33 ±0.54) × 106 after adjusted by the weight of lung and heart (P <0.01). The number of purified AEC-II obtained by immune adherence method was (38.0 ±28.0) × 106 perpiglet which was higher than by the method of percoll. The optimal phenotype maintenance time of AEC- Ⅱ was the first 24~96 hours in the primary culture. With increasing concentrations of IL-1 β and IL-6, there were decreased proliferation and expression of SP-A and IGF-Ⅰ mRNA in the cultured AEC- Ⅱ ,but SP-B mRNA expression was not affected. Both AEC-Ⅱ proliferation and expression of SP-A, SP-B mRNA decreased significantly after cultured with anti-IGF-Ⅰ. Conclusion In a new model of cultured AEC-Ⅱ from neonate piglets, IL-1β and IL-6 inhibited AEC- Ⅱ proliferation and SP-A mRNA expression through IGF-Ⅰ -dependent mechanisms.
5.Effects of brucine combined with glycyrrhetinic acid or liquiritin on rat hepatic cytochrome P450 activities in vivo.

Abstract: The activities of four CYP450 enzymes (CYP3A, 1A2, 2El and 2C) and the mRNA expression levels of CYP1A2, 2El, 2Cll and 3A1 in rat liver were determined after Wistar rats were orally administered with brucine (BR) at three dosage levels (3, 15 and 60 mg.kg-1 per day) and the high dose of BR combined with glycyrrhetinic acid (GA, 25 mg.kg-1 per day) or liquiritin (LQ, 20 mg.kg-1 per day) for 7 consecutive days. Compared with the control, brucine caused 24.5% and 34.6% decrease of CYP3A-associated testosterone 6beta-hydroxylation (6betaTesto-OH) and CYP2C-associated tolbutamide hydroxylation (Tol-OH), respectively, and 146.1% increase of CYP2El-associated para-nitrophenol hydroxylation (PNP-OH) at the high dose level. On the other hand, (BR+GA) caused 51.4% and 33.5% decrease, respectively, of CYP2El-associated PNP-OH and CYP1A2-associated ethoxyresorufin-O-de-ethylation (EROD) as compared with the high dose of BR group. Meanwhile, (BR+LQ) caused 41.1% decrease of CYP2El-associated PNP-OH and 37.7% increase of CYP2C-associated Tol-OH. The results indicated that the co-administration of BR with GA or LQ had effect on mRNA expression and activities of the CYP450 enzymes mentioned above to some extent, and the in vivo antagonism of LQ on BR-induced CYPs adverse effects and the in vivo inhibitory action of GA on CYP2E1 and 1A2 might play an important role in the detoxification of Radix Glycyrrhizae against Strychnos nux-vomica L.
6.Mesenchymal stem cell transplantation for spinal cord injury:a Meta-analysis

BACKGROUND:In recent years, the effectiveness of stem cel transplantation in the treatment of spinal cord injury has been validated in animal models, and mesenchymal stem cel transplantation for treatment of spinal cord injury has been studied most widely. Currently, there are a number of relevant clinical studies that have shown a good prospect. OBJECTIVE:To evaluate the efficacy and safety of mesenchymal stem cel transplantation for spinal cord injury in human with a system review. METHODS:PubMed database, EMBASE database, Cochrane Library, ISI Web of knowledge, CBM database, VIP database, CNKI database and Wanfang database were searched from their start year up to July 2015 for relevant randomized clinical trials on the treatment of spinal cord injury with mesenchymal stem cel transplantation. The key words were“spinal cord injury, paraplegia, cel transplantation, transplantation, mesenchymal stem cel , bone marrow transplantation, stem cel , randomized control ed trial”in English and Chinese, respectively. RESULTS AND CONCLUSION:A total of 260 articles were retrieved, including 6 randomized clinical trials (252 cases). In the aspects of ASIA touch sensation score, overal Frankel score and daily life activity training score, the patients undergoing mesenchymal stem cel transplantation were significantly superior to those in the control group (P<0.05). In addition, ASIA motor function score and residual urine volume were also improved in the patients undergoing mesenchymal stem cel transplantation, but there was no statistical difference (P>0.05). Compared with the control group, low fever was more common in the patients undergoing mesechymal stem cel transplantation (P<0.05). Another side effect was lower limb numbness, but there was no difference from the control group (P>0.05). These findings suggest that mesenchymal stem cel transplantation has limited efficacy in the treatment of spinal cord injury and cannot induce severe complications, but there is a need for high-quality randomized control ed trials to prove the efficiency and safety of mesenchymal stem cel transplantation for the treatment of spinal cord injury.
7.Uvulopalatopharyngoplasty and hyoid suspension for obstructive sleep apnea hyponea syndrome.

OBJECTIVE:
The aim of this study was to investigate the clinical efficacy of uvulopalatopharyngoplasty (UPPP) with hyoid suspension for patients with obstructive sleep apnea hypopnea syndrome (OSAHS).
METHOD:
Thirty-eight OSAHS patients underwent UPPP with hyoid suspension. Review the sleep monitoring after 6 months and 1 year and compare the AHI, LSaO 2 and ESS score.
RESULT:
The average AHI decreased, and blood oxygen saturation increased significantly afer operation.
CONCLUSION
UPPP with hyoid suspension is an available and relatively safe surgical approach in OSAHS patients.

8.Evaluation on efficacy of trastuzumab combined with neoadj uvant chemotherapy in Her-2 positive breast cancer patients and analysis of influencing factors of prognosis

Objective:To assess the efficacy of trastuzumab combined with neoadjuvant chemotherapy in the treatment of human epidermal growth factor receptor-2 (Her-2)positive breast cancer patients,and to explore its influencing factors of prognosis of breast cancer.Methods:The clinical materials of 112 Her-2 positive breast cancer patients were collected and they were divided into combined treatment group (trastuzumab + neoadj uvant chemotherapy)and single chemotherapy group (without trastuzumab ). SPSS 1 9.0 software was employed to calculate and analyze their clinical characteristics.The survival rate and prognosis were analyzed by Kaplan-Meier method,Log-rank test, and Cox regression. Results:Among the 112 Her-2 positive breast cancer patients, 23 cases were treated with trastuzumab,89 cases were treated without trastuzumab.The disease-free survival (DFS)in combined treatment group and single chemotherapy group had significant difference (P=0.012).And there was no significant difference of overall survival (OS)between two groups (P=0.064).The HR negative group had 18 (32.7%)patients with 5-level of Miller and Payne (MP)classification which was higher than HR positive group (5/46,10.9%)(P=0.009).In univariate analysis,the tumor size,node status at diagnosis and node status after operation were the influencing factors of DFS in the Her-2 positive breast cancer patients;the tumor size,node status at diagnosis,MP classification and node status after operation were the influencing factors of OS in the Her-2 positive breast cancer patients.The result of multivariate analysis indicated that the tumor size, node status at diagnosis,and with or without radiotherapy were the independent influencing factors of DFS and OS in the Her-2 positive breast cancer patients. Conclusion:The breast lesions in the Her-2 positive breast cancer patients with negative HR are relieved after treated with trastuzumab combined with neoadjuvant chemotherapy. The combination of trastuzumab and neoadj uvant chemotherapy can significantly improve the DFS and has a positive influence in the prognosis of Her-2 positive breast cancer patients.
9.Effects of Valproic Acid and Molecular Hydrogen on Phenotypes of Microglia Treated with Hypoxia

Objective To investigate the effect of VPA and molecular hydrogen(H2)on phenotypes of microglia treated with hypoxia. Methods Mouse hypoxic BV2 microglia were treated with VPA or H2. The levels of phenotypic markers of supernatant and cells were detected by ELISA, flow cytometry and real?time PCR,respectively. Results Hypoxia significantly increased mRNA level of M1 marker(iNOS)and reduced mRNA levels of M2 markers(CD206 and TGF?β)in BV2(P<0.05). Besides,the ratio between the mRNA levels of M1 increased(P<0.05). VPA significantly reduced protein level(CD16/32)and mRNA production(iNOS)of M1 markers in hypoxia?treated BV2(P<0.05). The ratio be?tween the mRNA levels of M1 markers and M2 markers(CD16:CD206,CD32:CD206,iNOS:CD206 and iNOS:TGF?β)were also significantly decreased(P<0.05). H2 significantly reduced both protein levels(TNF?α,CD16/32 and iNOS)and mRNA production(iNOS)of M1 markers and increased secretion of M2 marker(IL?10)in hypoxia?treated BV2(P<0.05). The ratio between the mRNA levels of M1 markers and M2 markers(CD16:CD206,iNOS:CD206 and iNOS:TGF?β)were also highly declined(P<0.05). Conclusion Hypoxia can induce microglial cells toward pro?inflammatory phenotype. Both VPA and H2 can inhibit hypoxia?induced inflammatory effect on microglia.
10.Analysis of the medical service capacity of primary healthcare in Zhejiang province

Objective To analyze the medical service capacity of primary healthcare in Zhejiang province since the ongoing healthcare reform and put forward suggestions. Methods Key indicators of healthcare resources and medical service utilization from 2009 to 2015 reflecting the primary healthcare were identified,for a quantitative analysis in terms of the structure-process-outcome dimensions. Results In terms of structural service capacity,the average headcount growth rate of primary healthcare′s technical personnel was 5. 7% per year; the personnel competence structure kept improving; the hospital beds at primary institutions and their ratio among all were slightly decreased,with better devices and informatization. In terms of procedural service capacity,the proportion of primary institutions with contract-based services amounted to 89. 9%,with the standard contract signing rate up to 18. 8%. In terms of consequential service capacity,the average growth rate of the number of outpatient visits at primary institutions was 6. 3%. The amount and proportion of inpatients were slightly decreased,while the hospital bed utilization ratio was increased slightly. Conclusions It is necessary to further strengthen the training and introduction of primary healthcare professionals. The functional orientation of hospitals at various levels should be clarified, encouraging contract-signing of general practitioners, promote the medical insurance payment reform featuring the capitation payment at primary level,and improve the income distribution and incentive mechanism.
