Objective:To investigate the first aid diagnosis and treatment of acute inhalation acetonitrile poisoning and to improve the ability of clinicians to diagnose and treat the disease.Methods:Retrospective analysis was performed on the diagnosis and treatment of 10 cases of acute occupational poisoning caused by acetonitrile gas, and clinical experience was summarized.Results:All the poisoned patients were cured and discharged after rescue and cooperation from various specialties.Conclusion:Acetonitrile poisoning treatment early to take the right first aid measures is the key, while safety education, training and enterprise supervision can not be ignored.

Objective To investigate the effect of evolocumab combined with levocarnitine after emergency percutaneous coronary intervention (PCI) in patients with acute ST-segment elevation myocardial infarction (STEMI). Methods A total of 120 STEMI patients were selected and randomly divided into treatment group (60 cases) and control group (60 cases). The control group was given STEMI basic treatment plus levocarnitine, and the treatment group was given evolocumab on the basis of the control group. The therapeutic effectiveness, coronary artery microcirculation [B-type natriuretic peptide (BNP), left ventricular ejection fraction (LVEF)], coronary arteriole status[Thrombolysis in Myocardial Infarction (TIMI) risk score, preprotein converting enzyme subtilysin 9 (PCSK9) and low density lipoprotein cholesterol (LDL-C)] and incidence of adverse reactions were compared between the two groups before and after treatment. Objective To analyze the clinical characteristics of IgG4-related disease (IgG4-RD)so as to improve the understanding of IgG4-RD in China.Methods IgG4-RD patients were recruited from Peking Union Medical College Hospital between January 2011 and January 2016.All patients were followedup for more than 6 months.The demographic characteristics,symptoms,organ involvements,laboratory examinations and treatment efficacy were evaluated and analyzed.Results A total of 346 patients were finally enrolled,including 230 males (66.5%) and 116 females (33.5%).The mean age of disease onset was (53.8 ± 14.2) years old.The mostly common involved organs were lymph nodes (56.4%) and submandibular glands (52.6%).Other affected organs and manifestations included:swelling of the lacrimal glands (46.5%),autoimmune pancreatitis (38.4%),pulmonary involvement (28.0%),sclerosing cholangitis (25.4%),naso-sinusitis (23.4%),parotid gland swelling (21.7%),retroperitoneal fibrosis (19.9%),large arteries involvement (9.5%),kidney involvement (obstructive nephropathy caused by retroperitoneal fibrosis was excluded) (6.9%),skin lesions (6.4%).Rare features consisted of thyroid glands,pituitary glands,gastrointestinal tract,pachymeningitis,pericardium,sclerosing mediastinitis and orchitis.The majority of patients had multi-organ involvement,such as 74.3% patients with 3 and more,18.2% and 7.5% patients with 2 and single organ involvement respectively.The average IgG4-RD responder index (IgG4-RD RI) was 13.21 ±5.70.History of allergy was found in 172 (49.7%) patients.As to the laboratory tests,elevated serum IgG4 levels were confirmed in 285 (94.1%) patients,which was positively correlated with IgG4-RD RI.There were 33.5% patients receiving monotherapy of glucocorticoid,52.6% treated with glucocorticoids combined with immunosuppressive agents,4.9% patients with immunosuppressant only,and 9.0% patients with mild disease not receiving medication.The majority (336,97.1%) patients improved the above regimens.Conclusion IgG4-RD is a systemic fibro-inflammatory disease with multiple organ involvement.The mostly common involved organs include lymph node,submandibular glands,and pancreas.Glucocorticoids and immunosuppressive agents were effective for IgG4-RD.
5.Potential profile analysis and nursing implications of self-management in elderly patients with venous leg ulcers

Objective To explore the status quo and potential profile of self-management in elderly patients with venous leg ulcers,analyse the differences and influencing factors in different profiles,and provide references for clinical nursing.Methods Convenient sampling method was used to select patients who were followed up in the department of vascular surgery and vascular department of 3 tertiary A hospitals in Shanghai from October 2022 to May 2023 as the study subjects.General data questionnaire,Chronic Wound Patient Self-management Behavior Questionnaire,Health Literacy Management Scale,Brief Illness Perception Questionnaire,Social Support Rating Scale,Chronic Disease Self-Efficacy Scale and Geriatric Depression Scale-15 were used for investigation.Mplus 8.3 was used for potential profile analysis,and SPSS 25.0 was used for univariate analysis and multiple Logistic regression analysis.Results A total of 316 valid questionnaires were collected,and the patients were divided into 3 potential profiles according to their self-management level:low management-inefficient wound response,middle management-highly effective compliance and high management-efficient wound response,accounting for 25.63%,46.84%and 27.53%respectively.Multiple Logistic regression analysis showed that duration of illness,depression,health literacy,self-efficacy and illness perception were factors influencing the potential profile of self-management in elderly patients with venous leg ulcers.Conclusion The self-management level of elderly patients with venous leg ulcers is at a medium level and has obvious classification characteristics.Clinical nurses can take targeted nursing measures according to different characteristics of patients to improve their self-management ability.
6.Association of gene polymorphisms in microRNA with blood pressure responses to salt and potassium intake

Objective To investigate the relationship of miRNA gene polymorphisms with blood pressure(BP)responses to the sodium and potassium diet intervention.Methods In 2004,we recruited 514 participants from 124 families in seven villages of Baoji,Shaanxi Province,China.All subjects were given a three-day normal diet,followed by a seven-day low-salt diet,a seven-day high-salt diet,and finally a seven-day high-salt and potassium supplementation.A total of 19 miRNA single nucleotide polymorphisms(SNPs)were selected for analysis.Results Throughout the sodium-potassium dietary intervention,the BP of the subjects fluctuated across all phases,showing a decrease during the low-salt period and an increase during the high-salt period,followed by a reduction in BP subsequent to potassium supplementation during the high-salt diet.MiR-210-3p SNP rs 12364149 was significantly associated with systolic BP(SBP),diastolic BP(DBP)and mean arterial pressure(MAP)responses to low-salt diet.MiR-4638-3p SNP rs6601178 was significantly associated with SBP while miR-26b-3p SNP rs115254818 was significantly associated with MAP responses to low-salt intervention.In addition,miR-26b-3p SNP rs115254818 was significantly correlated with SBP,DBP and MAP responses to high-salt intervention.MiR-1307-5p SNPs rs1 1191676 and rs2292807 were associated with SBP and MAP responses to high-salt diet.MiR-4638-3p SNP rs6601178,miR-210-3p SNP rs12364149,miR-382-5p SNP rs4906032 and rs4143957 were significantly associated with SBP response to high-salt diet.In addition,miR-26b-3p SNP rs115254818 was significantly associated with SBP,DBP and MAP responses to potassium supplementation.MiR-1307-5p SNPs rs11191676,rs2292807,and miR-19a-3p SNP rs4284505 were significantly associated with SBP responses to high-salt and potassium supplementation.Conclusion miRNA gene polymorphisms are associated with BP response to sodium and potassium,suggesting that miRNA genes may be involved in the pathophysiological process of salt sensitivity and potassium sensitivity.
7.Contents Determination of Spinosin and Jujub oside A in the Seads of Ziziphus jujuba and Its Quality Grading Standard

OBJECTIVE: To establish a method for simultaneous determination of spinosin and jujuboside A in the seads of Ziziphus jujuba, and to investigate its quality grading standard. METHODS: HPLC-ELSD method was adopted. The separation was carried out on Inertsil ODS-SP column with mobile phase consisted of acetonitrile-water (gradient elution) at the flow rate of 1.0 mL/min. The column temperature was 30 ℃, the temperature of drift tube was 90 ℃, the flow of carrier gas was 2.9 L/min and injection volume was 20 μL. The thickness, width, length and 100-grain quality of the medicinal materials were used as indicators to investigate the appearance traits. SPSS 22.0 software was used to analyze the correlation of the contents of spinosin and jujuboside A, its appearance traits with the quality constant of TCM, and establish a quality classification standard for the seads of Z. jujuba. RESULTS: The linear range of spinosin and jujuboside A were 1.03-6.18 μg/mL (r=0.999 7), 1.05-6.30 μg/mL (r=0.999 8); the limits of quantitation were 0.171, 0.174 μg/mL, respectively; the limits of detection were 0.052, 0.053 μg/mL, respectively. RSDs of precision, stability and reproducibility tests were all lower 2%. The recoveries were 99.01%-102.97% (RSD=1.39%, n=6), 97.94%-101.03% (RSD=1.13%, n=6), respectively. Correlation analysis results showed that the length, width, 100-grain quality spinosin content and jujuboside A content of the medicinal materials were positively correlated with the quality constant of TCM. The results of quality classification for 30 batches of medicinal materials showed that S1-S4 and S7-S12 were first-class products; S5, S6, S13-S17 and S20-S30 were second-class products; S18 and S19 were third-class products. CONCLUSIONS: Established content determination method is simple, precision, accurate and stable, and can be used for simultaneous determination of spinosin and jujuboside A in the seads of Z. jujuba. Established quality grading standard of the seads of Z. jujuba can be used to evaluate the quality.

【Objective】 Corin, a transmembrane serine protease that can cleave atrial natriuretic peptide precursor (pro-ANP) into atrial natriuretic peptide with smaller bioactive molecules, participates in the pathophysiological process of hypertension and cardiac hypertrophy. The purpose of this study was to explore the relationship of Corin gene variation with blood pressure responses to sodium and potassium dietary interventions. 【Methods】 In 2004, we recruited 514 participants from 124 families in 7 villages of Baoji, Shaanxi Province, China. All the subjects received a 3-day normal diet, a 7-day low-salt diet, a 7-day high-salt diet, and finally a 7-day high-salt and potassium supplementation. Fifteen single nucleotide polymorphisms (SNPs) of Corin gene were selected for final analysis. 【Results】 SNPs rs12509275 were significantly associated with diastolic blood pressure (DBP) response to low-salt diet, while rs3749584 was associated with pulse pressure (PP) response to low-salt diet.SNP rs3749584 and rs10517195 were significantly associated with PP response to high-salt diet. In addition,rs17654278 were significantly associated with systolic blood pressure (SBP) response to high-salt and potassium supplementation, rs2271037 was significantly correlated with DBP responses to high-salt and potassium supplementation, and rs4695253, rs12509275, rs2351783, rs36090894 were significantly associated with PP response to high-salt and potassium supplementation. 【Conclusion】 Corin gene polymorphisms were associated with blood pressure response to sodium and potassium, suggesting that Corin gene may be involved in pathophysiological process of salt sensitivity and potassium sensitivity.
