2.Association of HbA1C with serum uric acid among inpatients with type 2 diabetes mellitus

[Summary] Data from 1 589 consecutive inpatients with type 2 diabetes mellitus from January 2012 to March 2015 were collected. The patients were divided into five groups according to the quintile of HbA1C . The association between serum uric acid ( SUA) and HbA1C was tested using a general linear model after adjusting for age, body mass index ( BMI) , systolic blood pressure, and creatinine. Linear regression analysis was used to analyze the association between SUA and HbA1C in patients with HbA1C<9. 0% and HbA1C≥9. 0%, respectively. The results showed that BMI, waist circumference, triglycerides, and the incidence of fatty liver were elevated with increased serum uric acid level. SUA was negatively associated with HbA1C level in inpatients with type 2 diabetes. However, SUA should be measured after glycemic control in men with HbA1C≥7. 0% and women with HbA1C≥9. 0%.
3.Association of carotid artery and lower-extremity artery atherosclerosis and coronary heart disease or cerebral infarction in type 2 diabetes

cerebral infarction as compare with patients at Grade 1-2 in both vascular locations, whereas the risk was not significantly increased in patients at Grade 3-4 in only one vascular location. Conclusions The simple method of assessing the degree of arterial atherosclerosis can be used to evaluate carotid artery and lower-extremity artery atherosclerosis in patients with type 2 diabetes. Patients with plaques or stenosis in both vascular locations were with a significantly increased risk of coronary heart disease or cerebral infarction if they were evaluated concurrently.
4.PI3 K/Akt/Sirt1 signaling pathway mediated hydrogen sulfide postconditioning-induced protection against I/R injury

Aim To explore the role of PI3 K/Akt/Sirt1 pathway in cardioprotection of hydrogen sulfide ( H2 S ) postconditioning against ischemia/reperfusion ( I/R) injury. Methods Langendorff perfusion appa-ratus was used to build an isolated rat myocardial I/R model. Isolated rat hearts were subjected to 30 min global ischemia followed by 60 min reperfusion after 20 min of equilibrium. 60 male SD rats were randomly di-vided into 5 groups(n=12):control group(Control), ischemia/reperfusion group( I/R) , H2 S postcondition-ing group( H2 S) , inhibitor LY294002 group( LY) and H2 S with inhibitor group( H2 S+LY) . The left ventric-ular diastolic pressure ( LVEDP ) , the left ventricular developed pressure(LVDP), the maximum rate of in-crease or decrease of left ventricular pressure ( ± dp/dtmax ) were registered at the end of 20 min equilibri-um, 30 and 60 min of reperfusion separately. Triphe-nyl tetrazolium chloride( TTC) staining was used to de-termine the myocardial infarct size. The levels of Sirt1 and PGC-1 mRNA were tested using real-time PCR. The expressions of Sirt1 and PGC-1αwere detected with Western blot analysis. Immunohistochemical method was used to determine the location of Sirt1 . Results There were no differences in equilibrium hemodynamics observed between the experimental groups(P>0. 05). At the end of reperfusion, compared with I/R group, H2 S group had obviously ameliorated functional recov-ery and significantly decreased the myocardial infarct size(26. 9 ± 4. 9)% vs(48. 9 ± 5. 6)%(P <0. 05). Meanwhile, the expression of Sirt1 and PGC-1α in-creased significantly. However,LY294002 reversed the cardioprotective effects provided by hydrogen sulfide postconditioning and reduced the level of Sirt1 and PGC-1α, the percentage of Sirt1-positive nuclei. Con-clusion PI3 K/Akt/Sirt1 signaling pathway mediates the hydrogen sulfide postconditioning-induced protec-tion against I/R injury.
5.Different fixations for intertrochanteric fracture affect proximal femur stress:a finite element analysis

BACKGROUND:The mechanical characteristics of intramedul ary and intramedul ary fixations for treating intertrochanteric fracture were hot research of finite element, but there was few comparative research on tension side and pressure side of proximal femur. In our study, we found that the distribution of the stress zone was important indication to researches on intertrochanteric fracture and internal fixation design. OBJECTIVE:To explore the effects of intramedul ary and extramedul ary fixations for treating intertrochanteric fracture on pressure side and tensile side of proximal femur. METHODS:CT scan and Mimics modeling of volunteer’s femur were conducted, and the model Evans type I intertrochanteric fracture was constructed and treated with proximal femoral nail antirotation and dynamic hip screw respectively. The stress condition of proximal femur in weight status was simulated. The Abaqus 6.13 was used for finite element analysis, and the tension side and pressure side of proximal femur in both fixations were also analyzed. RESULTS AND CONCLUSION:(1) The stresses of compressive side and tensile side in proximal femoral nail antirotation group were both less than that in dynamic hip screw group. The stress of proximal femur in proximal femoral nail antirotation group was more similar to that in physiological status. (2) These findings verified that for treating intertrochanteric fracture, intramedul ary fixation exerted better mechanical behavior than extramedul ary fixation. The protection of compressive side and tensile side of proximal femur had achieved by intramedul ary fixation.
6.The expression and role of CMTM family in tumor

CMTM family is silenced and down-regulated in several kinds of tumors.Its aberrant expression has a strong association with the development,progression and metastasis of tumors.Thus,CMTM family is potential tumor suppressor genes.Epigenetics mechanism is the essential mechanism of the aberrant expression in this gene family.The discovery of this research gives a new direction to the clinical treatment of tumor.
7.Research and implementation of gray-scale blood flow imaging system of high frequency ultrasound

Objective To study the method of gray-scale blood flow imaging and image processing in condition of high frequency ultrasound,and the implementation of the system.Methods On the base of research of scattered signals of red blood cells in high frequency ultrasound,20 MHz ultrasound mechanical and linear scanning probe was used to transmit a number of pulses on a scan line.Pulse-echo subtraction method was used to obtain the blood flow information.At the end,simulated blood vascular was used to conduct flow imaging,and the obtained images were analyzed.Results Experiment results showed that clear blood flow images were obtained using this system.The noise from perivascular tissue could be filtered and the signals from blood flow could be enhanced after image processing.Conclusions In the detection of superficial blood vessel,blood flow signals can be obtained even using single pulse emitting via high frequency ultrasound.The blood flow imaging system can be implemented after image processing.
8.Biomechanical analysis of fracture fixation with external fixator in vivo.

OBJECTIVETo investigate the different ways of measuring the main axial strain during treatment with an external fixator and to find the suitable compression loaded by the external fixator at an early stage.
METHODSEighteen healthy big-ear rabbits were randomly divided into two groups according to different measuring methods: Group A and Group B. In Group A, a strain gauge was affixed to the external tibial cortex with 502 glue, and in Group B, a bone cement-coated strain gauge was installed on the internal tibial cortex. Groups A and B were divided into two subgroups A1, A2 and B1, B2, respectively, according to the pressure of half of and the same as the body weight. A Z-shaped left mid-shaft tibial osteotomy was performed and fixed by an external fixator.
RESULTSThe scaler curves of Group A changed dramatically during the early stage. The trendlines of the internal and external cortex went consistently after reaching the stable stage while the latter strain value was higher than the former. The time for Group B reaching the stable stage was short, but its absolute strain value was less than that of Group A. Before they were pressed to the stable stage, the declined speed of Subgroup A1 was more slowly than that of Subgroup A2 while the results of Subgroups B1 and B2 were same. Group A had an ascending trend after it declined while Group B didn't have. After they reached the stable stage, both Subgroups A1 and A2 had a declining trend while Subgroup A2 was more quickly than Subgroup A1, Subgroup B1 was kept at a definite level while Subgroup B2 fluctuated.
CONCLUSIONSThe axial strain under external fixator can be measured by bone cement coated-strain gauge in vivo. The data may suggest that half of the body weight load was suitable for external fixator.

9.Effect of allograft compound vertebra on vertebral reconstruction in rabbits.

OBJECTIVETo study the effect of allograft compound vertebra on vertebral reconstruction in rabbits so as to provide biomechanical direction for manufacturing and selecting vertebral reconstruction materials.
METHODSTwenty-five healthy New Zealand white rabbits were divided randomly into three groups: normal group (Group A, n equal to 5),iliac bone graft group (Group B, n equal to 10) and allograft compound vertebra group (Group C, equal to 10). After C4 was resected, iliac bone implantation and allograft bone cage transplantation were fulfilled in Group B and Group C, respectively. Every 5 rabbits from Group B and Group C were selected to test the biomechanical strength and biological activity one and two months postoperatively.
RESULTSNo significant statistical difference was found between Group A and Group C one and two months postoperatively (P larger than 0.05). The biomechanical strength of Group B was much weaker than that of Group A and Group C one month postoperatively (P less than 0.05), but at two months postoperatively, no statistical difference was found among the three groups. The biological activity and vertebral moulding ability of Group C were better than those of Group B at one and two months postoperatively.
CONCLUSIONSCompound vertebra, which is made up of allograft cortical bone cage and autogenous cancellous bone, shows instantaneous and permanent biomechanical stability and biological activity, therefore, it is an ideal material for vertebral reconstruction.

10.Comparison of therapeutic effect between percutaneous kyphoplasty and pedicle screw system on vertebral compression fracture.

OBJECTIVETo compare the clinical efficacy of percutaneous kyphoplasty (PKP) with pedicle screw system (PS) in the treatment of vertebral compression fracture(VCF).
METHODSEighty-six patients with VCF were treated either by PKP (Group A, n equal to 30) or PS (Group B, n equal to 56). The anterior, intermediate, and posterior heights of the vertebrae body, visual analogue pain scale (VAS) before and after operation, the duration of operation, and amount of blood loss between two groups were compared.
RESULTSNo statistical difference was noted regarding the vertebral height between two groups. Significant difference was seen in VAS, duration of operation and amount of blood loss between the two groups (P less than 0.01).
CONCLUSIONSPercutaneous kyphoplasty has the similar therapeutic efficacy with pedicle screw system in treatment of VCF with a minimal invasion, less operation time and blood loss. For those with posterior wall destruction, PS is deemed favorable.
