1.Diagnosis of coronary arterial anatomy of patients with arterial switch operation using echocardiography

Objective To evaluate the accuracy of preoperative diagnosis on the anatomy of coronary artery of patients with arterial switch operation(ASO) using echocardiography, to conclude the diagnostic methods. Methods From March 2007 to March 2008,50 patients were performed ASO after the diagnosis with eehoeardiography. Prior to the operation, evaluation was conducted to all of anatomical pattern of the coronary artery,and the comparison was made to the observation in the operation. There were 23 patients with complete transposition of great artery(TGA), 20 patients with double outlet of right ventricle of abnormal great artery(DORV), 2 patients with corrected transposition of the great arteries(CTGA), 1 patients with double outlet of left ventricle of abnormal great artery (DOLV), 2 patients with TGA complicated with total anomalous pulmonary venous connection (TGA + TAPVC), 2 patients with TGA complicated with pulmonary valve stenosis(TGA + PS). Results Among 50 patents,there were 40 patients with normal coronary artery,and 10 patients with abnormal coronary arteries. Except for 2 patients failing to being successfully diagnosed due to bad condition of image,48 patients could be revealed the origin and the direction of the major coronary arteries, the visibility proportion was 96.0%. Compared to the observation in the operation,the total echocardiography accuracy for 48 patients was 95.8%. Among the 40 patients with normal coronary arteries,39 patients were diagnosed accurately except 1 patient failed to be showed clearly by echo. In the 10 patients with abnormal coronary arteries,4 patients with single abnormal coronary artery, 2 patients with 2 coronary artery originating from the same coronary sinus, 2 patients with left anterior descending coronary artery originating from right coronary artery, 2 patients with 2 coronary artery originating from two adjacent coronary sinus. One patient failed to be diagnosed by echo for the bad image,2 patients was misdiagnosed by echo, the accuracy of echo was 77.8%. Conclusions Through multi-section scanning,echocardiography can accurately diagnose anatomical pattern of coronary artery for most of ASO,thus providing comprehensive and accurate diagnosis information for ASO.
2.Research Status on Intraoperative Regional Chemotherapy of Advanced Gastric Cancer

Objective To explore the feasibility,safety,efficacy and mechanism of intraoperative regional chemotherapy of advanced gastric cancer. Methods The related literatures were reviewed and analyzed. Results Compared with systemic chemotherapy,intraoperative regional chemotherapy of advanced gastric cancer could increase blood drug concentration of cancerous tissue,reduce the systemic toxic side effects,increase survival rate and improve the quality of life. Conclusion Intraoperative regional chemotherapy,as an adjuvant treatment of advanced gastric cancer,has been gradually applied to clinic because of the definite curative effect,which is worth popularizing. However,it needs systemic researches and accumulation of cases.
4.Treatment and prognosis of 135 nasal non-Hodgkin′s lymphoma patients

Objective To evaluate the effects of nasal non-Hodgkin′s lymphoma(N-NHL) treated with chemotherapy alone, radiotherapy alone, chemotherapy plus radiotherapy and autologous peripheral blood stem cell transplantation(APBSCT) combined with total body irradiation(TBI);and to analyze the impact factors of prognosis. Methods 135 patients were treated between 1980 and 2000. All were confirmed by histopathology as N-NHL, including 122 T cell in origin, 12 B cell and 1 NK cell in origin. The main radiotherapy portal was set in front of the nose with a spade-like protrusion, supplement with a portal next to the ear on one side or both sides. Combined portal in facial cervical area was first used when there was invasion of the oropharynx. The median dose to the nasal cavity was 56.0Gy with a range of 35.2 to 75.5Gy, with added 30Gy to the primary lesion in two patients. Six patients received TBI combined with APBSCT, with 8Gy in the TBI group. Chemotherapy, given before or during after radiotherapy or alone, consisted of 2-6 cycles of COP, COPP, COMP, CHOP or COBDP. Prognostic factors were analyzed with Cox model. Results The local control rate was 12%,69%,76% and 83% in chemotherapy alone, radiotherapy alone, chemotherapy plus radiotherapy and APBSCT combined with TBI, respectively(P=0.057).The 5-year survival rate was 9%,52%,63% and 83%,respectively(P=0.032). Multi-factor analysis showed that tumor extension and treatment methods were the most important prognostic factors besides Ann-Arbor stage, but gender, pathology, age and symptoms had little effect on prognosis .Conclusions Chemotherapy plus radiotherapy group achieves a better survival rate than radiotherapy alone. It is helpful to evaluate prognosis to make more detail subareas on basis of local extensions in Ann Arbor staging system.For some N-NHL patients with good financial condition, APBSCT combined with TBI is a good choice.
5.Compare the curative effect of Non-Hodgkin lymphoma by four types of therapy

Objective To evaluate the curative effect of nasal non-Hodgkin lymphoma(N-NHL) by four types of therapy such as pure chemotherapy, pure radiotherapy, chemotherapy combined with radiotherapy, and APBSCT combined with TBI. Methods One hundred and thirty five patients with nasal NHL were treated between 1980 and 2000. All patients received radiotherapy alone or chemotherapy alone or radiotherapy combined with radiotherapy or TBI combined with APBSCT. The median radiation dose to the nasal cavity was 56.0 Gy with a range of 35.2 to 75.5 Gy. Six patients received TBI combined APBSCT. The TBI dose was 8 Gy. Two patients received 30 Gy in nasal of the six patients. The chemotherapy regimen consisted of 2-6 cycles of COP, COPP, COMP, CHOP, COBDP. Results The local control rate and 5-year survival rate of the four groups of pure chemotherapy, pure radiotherapy, combined chemotherapy and radiotherapy and APBSCT combined with TBI were 12 %, 69 %, 76 %, 83 % and 9 %, 52 %, 63 %, 83 %. For the four groups, the best is APBSCT, then combined chemotherapy and radiotherapy group ,then pure radiotherapy, the last is pure chemotherapy. 6.Practice and Exploration for the Teaching of New Drug Clinical Research

OBJECTIVE:To Provide reference for improving teaching levels of New Drug Clinical Research and training for pharmaceutical research talents. METHODS:The experiment teaching in 2013 graduate student of the Fourth Military Medical Uni-versity was taken as a pilot,different teaching methods were introduced in various chapters of New Drug Clinical Research,and ap-praisal system was perfected. RESULTS & CONCLUSIONS:Some appropriate methods are established,for example,conductive teaching,problem-based learning,spot-teaching and bilingual-teaching. The appraisal system including usual performance and re-search proposal is established,it has improved enthusiasm and initiative of students,promoted teaching quality,obtained certain achievements,which provides reference for the training methods on the cultivation of innovative and professional pharmaceutical tal-ents.
8.Double micro-catheter technique embolization for cerebral aneurysm

Objective To explore the clinical effect of double micro-catheter technique for detachable coil treatment of intracranial an-eurysms. Methods From January 2010 to April 2014,there were a total of 83 patients with cerebral aneurysms treated with double micro-catheters,and the ratio of neck/body was 1/3~1. The double micro-catheters were positioned within aneurysm. The detachable coils were in-troduced to satisfactory position by different ways. There was always one undetached coil to hold the stability of detached coils till aneurysm compactly embolized. Results 83 cases were instantly compactly embolized with double micro-catheter technique. No coils out of aneurysm. There were two cases of ischemic complication during clinical treatment,one of them was died. One case ruptured during operation. Conclu-sion The double micro-catheter technique may be an optional and safe method during embolization of some cerebral aneurysms.
10.Glycolysis abnormality in neoplasm cells and the targeted therapy

Cancer cells display a decreased mitochondrial respiration along with an abnormal activation of glycolysis to satisfy the demand of the malignant growth even in the presence of oxygen.This phenomenon may be caused by mitochondrial dysfunction,hypoxia of microenvironment and the abnormal activation of upstream regulators of glycolysis pathway.Treatment targeted at the glycolysis pathway and its upstream regulators may provide a new therapeutic strategy of cancer.
