Cystic lesions of the pancreas have been detected more frequently than before because of the development of imaging technologies. Pancreas cysts can be divided into neoplastic or non-neoplastic and neoplastic cyst can be categorized as potentially malignant or non-malignant. Cystic lesion that contains mucinous material has malignant potential. Some factors that can predict the presence of malignant tissue has been identified. Serous neoplasm, congenital cysts and pseudocyst do not have malignant potential. Solid pseudopapillary tumor or endocrine tumor can undergo cystic degeneration and also have malignant potential of low level.

Pancreatic cystic lesions are being recognized with increasing frequency due to the development of imaging technologies. Pancreatic cystic lesions can be divided into neoplasms and non-neoplasms, and neoplasms can be further categorized as epithelial and non-epithelial tumors depending on the cells of origin. A significant percentage of pancreatic cystic neoplasms have malignant potential and surgical resection is mandatory. To make the proper decision regarding the management of pancreatic cystic neoplasms, it is important to understand the clinicopathologic features of these tumors.

Pancreatic cystic lesions are being recognized with increasing frequency because of the widespread use of cross-sectional imaging. Although most pancreatic cysts are incidentally detected, pancreatic cysts represent a wide spectrum of histopathology, and neoplastic cysts are more prevalent than previously estimated. Some histologic types have malignant potential. Considering the incidence of perioperative morbidity in patients undergoing surgical resection, deciding whether to operate on or observe an indeterminate pancreatic cyst is challenging. This clinical dilemma has created the need to develop a minimally invasive approach for treating pancreatic cysts. EUS-guided pancreatic cyst ablation has been investigated in several clinical trials and has the potential to become an alternative to surgical resection.

Hemangioblastoma is uncommon, accounting for 1 to 2.5% of all primary neoplasms of the central nervous system. Ten to 20% of hemangioblastomas occur as part of Von Hippel-Lindau disease(VHL). Multiple hemangioblastomas are seen only with VHL and these are seen in up to half of VHL-associated hemangioblastomas. We have treated two cases of multiple hemangioblastomas associated with VHL disease in a family, sister(case I, 48/female) and her brother(case II, 41/male). Both patients had renal and pancreatic cysts in addition to CNS hemangioblastomas. Their hemangioblastomas were removed totally, and then their neurological symptoms had Improved. With a review of the literature, the authors present a family of VHL-associated multiple hemangioblastomas.

Rapid advancements, access to and use of imaging techniques have increased the frequency of identification of pancreatic cystic neoplasms in clinical practice. However, a diagnostic dilemma among pancreatic cystic neoplasms remains. Solid variant serous cystadenoma is extremely rare and difficult to accurately diagnose preoperatively, as they are commonly mistaken for malignant solid tumors of other types. Here, we present a case of a solid variant serous cystadenoma preoperatively misdiagnosed as a neuroendocrine tumor of the pancreas with a review of the relevant literature.

Pancreatic pseudocysts are focal fluid collections that develop as a result of inflammatory diseases of the pancreas. They are managed conservatively or with a drainage procedure. Their radiological appearance can mimic cystic neoplasms of the pancreas. Pancreatic cystic neoplasms include various neoplasms with a wide range of malignant potential. Here, we report a patient with a pancreatic pseudocyst that presented with macrocystic attributes on endoscopic ultrasound.

Pancreatic cystic neoplasm is a clinically challenging entity. Its incidence estimated up to 45% of the general population. The biological behavior ranges from benign to malignant disease. The strategy for pancreatic cystic neoplasm could be to prevent progression to pancreatic cancer while minimizing the costs. The first step for the correct management is correct diagnosis. In this paper, the radiological differential diagnosis of them will be described.

Epidermoid cyst in the intrapancreatic accessory spleen is extremely rare condition which could be mistaken for pancreatic cystic neoplasm. We report two cases of epidermoid cysts of intrapancreatic accessory spleen which mimicking pancreatic cystic neoplasm. Two patients, both male, aged 47 and 53 respectively were referred to our department for pancreatic mass. One was asymptomatic, whereas the other presented worsening abdominal pain and progressive weight loss. In both cases, the mass with cystic component was detected in the pancreatic tail in a computed tomography scan. Under a suspicion of pancreatic cystic neuronendocrine tumor, they underwent a distal pancreatectomy. Pathologic feature of resected specimens were shown to benign squamous lined cyst with splenic tissue in and around cyst wall which suggested that epidermoid cysts in the accessory spleen. We should take into account the possibility of epidermoid cyst in the intrapancreatic accessory spleen in the differential diagnosis of pancreatic cystic lesion.

Detection of cystic lesions in the pancreas has increased because of the widespread use of high-resolution diagnostic imaging techniques. Therefore, cystic lesions of the pancreas constitute an increasingly important category with a challenging differential diagnosis. Squamoid cyst of pancreatic ducts is a recently recognized type of cystic lesion in the pancreas in which cystically dilated ducts are lined by non-keratinized squamous epithelium. Although it is clinically known as benign cystic lesion, we experienced its malignant behavior and report here with review of the international literatures.
