Objective Selecting doxorubicin and tetrandrine as model drug to prepare complex liposomes, study the methods of preparation, and research its release property in vitro. Methods The formulation of tetrandrine-doxorubicin complex liposomes was optimized by three different kinds of methods. And the optimum formula was selected through the orthogonal test according to the entrapment efficiency. Results Tetrandrine-doxorubicin complex liposomes were prepared by (NH4)2SO4-gradient method combined with pH gradient method. One optimum recipe was founded that tetrandrine-doxorubicin complex liposomes/ egg phosphatidyl choline was 1∶20, egg phosphatidyl choline/cholesterol was 3∶1, pH value was 7.6, incubation temperature was 50 ℃, concentration of (NH4)2SO4 was 250 mmol/L. The doxorubicin completely released within 24 h, and the tetrandrine released within 16 h. Conclusion Tetrandrine-doxorubicin complex liposomes have high entrapment efficiency with fine-looking, which is better for the further studies

Purpose To investigate the clinical efficacy of percutaneous radiofrequency ablation and microwave ablation in the treatment of primary hepatic carcinoma.Materials and Methods Ninety-two patients with primary hepatocellular carcinoma (116 lesions) were randomly divided into 46 cases of radiofrequency ablation group and 46 cases of microwave ablation group, which were treated with radiofrequency ablation and microwave ablation, respectively, the ablation points and ablation time, complete ablation rate, local tumor progression rate, postoperative complications, hospitalization time, hospitalization expenses, recurrence rate and relapse-free survival rate 1 year, 2 years and 3 years after surgery were compared and analyzed. Results There was no significant difference between the two groups in average ablation points (P>0.05), and the ablation time of microwave ablation group was significantly shorter than that of radiofrequency ablation group (P<0.01). There was no significant difference in complete ablation rate and local tumor progression rate between the two groups (P>0.05). The difference of complete ablation rate and local tumor progression rate between the two groups of tumor <3 cm, tumor 3-5 cm and tumor near the vessel were not statistically significant (P>0.05). There were no significant differences in the incidence of fever, pain in liver area and average length of hospitalization between the two groups (P>0.05). The average hospitalization cost of microwave ablation group was significantly lower than that of radiofrequency ablation group (P<0.01). There was no statistically significant difference of the cumulative recurrence rate for 1 year, 2 years, 3 years after surgery between the two groups (P>0.05). There was no significant difference between the two groups in the overall relapse-free survival rate and relapse-free survival rate among tumors with different size of the two groups (P>0.05).Conclusion Using microwave ablation for the treatment of primary hepatocellular carcinoma shows a clinical efficacy and safety as good as radiofrequency ablation. Radiofrequency ablation may have better therapeutic effect for smaller tumors, but microwave ablation may have some advantages for medium or large tumors and tumors located near the vessels.

Objective: To investigate the mechanism that glutamine downregulates the cytokine expression in lipopolysaccharide-stimulated human peripheral blood mononuclear cells. Methods:PMBC were extracted from healthy volunteer by density gradient centrifugation, and stimulated by lipopolysaccharide. The release of TNF-α and IL-10 was analyzed via enzyme-linked immunosorbent assay and p-P38 via western blot. Results:Gln led to an increase in HSP70 expression, and decreased TNF-α、IL-10 release at 4 h after LPS stimulation. The expressions level of TNF-α、IL-10 and P-P38 were decreased after using SB203580.Conclusion:These results indicate that the effect of Gln could attenuate cytokine release in PBMC and might may be related to the inhibition of P38 MAPK.

OBJECTIVE:To provide reference for rational use of human serum albumin for surgical inpatients. METHODS:The utilization of human serum albumin for surgical inpatients in a hospital during Jan.-Mar. 2014 was analyzed and evaluated by UHC Guidelines for the Use of Album,Nonprotein Colloid, and Crystalloid Solutions(2010 edition)and European Immune Globu-lin and Albumin Use Recommendation. RESULTS:Among the 556 patients, totally 895 human serum albumin application were con-ducted,mainly involving development of gastrointestinal surgery(29.7%),hepatobiliary surgery(25.9%)and cardiothoracic sur-gery(13.1%). The main reasons were correcting hypoalbuminemia(62.9%),followed by albumin supplemented during major sur-gery(7.9%)and alleviating ascites in patients with cirrhosis(4.4%);only 95 applications(10.6%)were considered appropriate. The most prevalent inappropriate reason was for correcting hypoalbuminemia. CONCLUSIONS:Human serum albumin in the surgi-cal inpatients in the hospital shows a large amount, and low consistent rate between indications and guidelines. The rational stan-dardized utilization of human serum albumin should be strengthened.

Chinese Journal of Infectious Diseases 2012;30(4):221-225
ObjectiveTo investigate the changes of serum apolipoprotein E (ApoE) in children with infectious diseases.MethodsA total of 279 pediatric patients with infectious diseases were enrolled in this study, including 65 patients with sepsis,47 patients with bacterial meningitis,67 patients with bacterial pneumonia, 47 patients with aseptic meningitis and 53 patients with mycoplasmapneumonia. TheserumApoEcollectedfromallpatientswasdetectedby immunoturbidimetric assay (IA).The septic mouse model was established by intraperitoneal injection of group B Salmonella typhimurium.Mouse serum ApoE levels were detected by IA,and the hepatic ApoE mRNA and protein expressions of mice were detected by quantitative real-time reverse transcription-polymerase chain reaction (RT-PCR) and Western blot, respectively.Data in two groups were compared by independent-sample t test.ResultsSerum ApoE levels in patients with bacterial infections were increased dramatically, which was (59.8±23.5) mg/L in patients with sepsis (t=-5.118,P<0.01),while no significant differences were found in patients with aseptic meningitis and myeoplasma pneumonia. Moreover,a high level of serum ApoE was detected in septic mouse model, while the hepatic ApoE mRNA and protein expressions of the mice were both decreased, with mRNA decreased 71% at 3 hour (t=5.022,P<0.01) and 73% at 24 hour (t=4.181,P<0.01).Conclusions Serum ApoE levels in bacterial infections increase dramatically, while its hepatic expression in septic mouse model is decreased, which indicates that the elevated serum ApoE level is not related to the changes of hepatic ApoE expression.
