1.Classification of hand degloving injuries and indiviualized treatment

Objective To evaluate the classification and treatment methods of hand degloving in-juries.Methods Individualized treatment was adopted according to area and depth of degloving inju-ries.The injury was repaired from deep to shallow layer.New techniques such as segment blood vessel re-pair, alternative debridement, local fixation and suturing, local pressure bandaging and sporadic drainageof incision were applied.Results All 76 patients were followed up for 3-48 months, which showed that75 patients were cured, with good shapes of the skins and strong rub resistance.Conclusion Classifi-cation of hand degloving injuries simplifies diagnosis, as facilitates the individulized treatment.Five new techniques mentioned above may help solve the problems encountered during operation.Individulized treatment explores new ways for the treatment of hand degloving injuries.
2.Microsurgical treatment of severe children hand injuries caused by firecrackers

Objective To summarize the clinical experiences in microsurgical treatment of severe children hand injuries caused by firecrackers. Methods 26 cases of children hand injuries caused by a firecracker blast were treated with microsurgical procedures. An emergency operation was done for 23 cases, and a delayed repair was done for 3 cases. Operative procedures differed from case to case, according to the injury type. Results All flaps and reconstructed thumbs survived completely by first intention. Of the 9 cases with 11 fingers replanted, 7cases with 9 fingers survived. The follow up for over two years showed that satisfactory restoration of hand function and shape was achieved and the replanted fingers also showed satisfactory restoration of holding and finger to palm and finger to finger functions. Conclusion Severe children hand injuries caused by a firecracker blast must be treated as soon as possible with microsurgical procedures. Preoperative planning of a proper repair or reconstruction approach is also of great importance.

To evaluate the prevalence and incidence of coronary heart disease (CHD) and associated risk factors with CHD in the elderly, a survey was conducted among old subjects aged 60 to 90 who had health examination in PLA General Hospital from 1996 to 1997. A cohort population without CHD had been followed up until 2000, and the incidence of CHD was calculated as the baseline. The incidence of CHD was calculated by person year method. The risk factors of incidence in CHD of different groups were analyzed with multiple logistic regression at the end of four observation years. 4.Giant cell tumor of tendon sheath in the hand:a clinicopathological,immunohistochemical and flow cytometric DNA analysis

Objective A retrospective study of GCTTS in the hand was undertaken to determine whether specific clinic or pathologic factors were associated with an increased risk of recurrence, and to investigate the relationship between the recurrence of GCTTS and its biological indices. Methods 83 patients with histologically proven GCTTS were treated and followed up. The clinical materials were reviewed. Comparison was made between certain factors to determine which were associated with increased recurrence rates. Results The patients were followed for an average of 55 months. 22 patients (26.5%) recurred. Age, gender, site, the origin of tumors, size, bone erosion or destruction, cellularity and the mitoses were not risk factors for recurrence. Tumors without capsule had higher recurrence rate than that with capsule, and the difference was highly significant. The local recurrence rate of Nm23 positive group was 28.3%, and Nm23 negative group was 19.2%, there was no remarkable difference between the groups. The PCNA-LI was 0.448?0.130 in recurrent group and 0.358?0.147 in nonrecurrent group, the difference was statistically significant. The aneuploidy DNA content was presented in 50% of the recurrent tumors, and in 23.2% of nonrecurrent ones. The SPF was 6.98?4.64 in recurrent group and 4.70?2.49 in nonrecurrent group. These values were also significantly different. PCNA-LI, the aneuploidy DNA content and SPF were significantly higher in group without capsule than those in group with capsule. These values were not different between tumors with and without bone destruction, large and small diameter, high and low cellular tumors, as well as high and low mitoses tumors. Conclusion The high proliferative indices of recurrent GCTTS may explain its aggressive biologic behavior. Tumors without capsule possess higher proliferative indices than the ones with capsule, and they have higher recurrent rate. The proliferative indices of the tumors with and without bone distruction are comparable, and the difference of the recurrence rate among groups is not significant.
5.A Preliminary Study on Indoor Air Pollution in Three Districts of Beijing

Objective To understand the indoor air pollution in Beijing. Methods Based on the data from epi-demiological investigation and air pollution monitoring, the investigation was carried out by choosing 3 districts in Beijing including Dongcheng, Shijingshan and Haidian during Jan-Mar in 2000. 70 houses were selected in each district randomly by cluster sampling, the indoor concentrations of PM10, PM2.5 and SO2 in bedroom and kitchen were monitored. The data on concentrations of PM10, PM2.5 and SO2, and the environmental conditions of each sampling place were all recorded in the unified questionaires. Results The total average concentrations of pollutants in indoor air of 3 districts were 0. 555 mg/m3 for PM10 and 0. 512 mg/m3 for PM2.5 respectively. The total average concentration of 0.052 mg/ m3 for SO2 was below the ralated National Stardard. The concentrations of PM10 and PM2.5 in the bedrooms and kitchens in each district all exceeded the related national standards, and revealed no significant differences between bedrooms and kitchens. 7.Preparation, reliability and validity of health status questionnaire

Objective To develop a set of health status questionnaire in line with health management demands for different physical examination reeeivers. Methods Based on literature review and clinical practice, sleep, pain, anxiety, depression, stress, fatigue, habits and social adaptation could be used as health items. An item database was set according to domestic or abroad established questionnaires. The items were screened to constitute our health status questionnaire. A total of 2866 individuals were randomly chosen to get the reliability or validity test. Results The correlation coefficient of single item with total score was 0.361-0.683. The Cronbach'α of sub-scales was ≥ 0.945, the retested reliability coefficient of was ≥ 0.677, and split-half reliability coefficient was ≥0.835. Eight factors in exploratory factor analysis explained 69.37% of total variance. Confirmatory factor analysis showed high degree of fitting. Conclusion Our health status questionnaire has higher reliability and validity and can be used for health data collection.
8.Comparison on diagnostic criteria for metabolic syndrome in the senile people

Objectives To compare the value of different criteria (WHO, IDF and CDS criteria) on the diagnosis of metabolic syndrome (MS) in the elderly people. Methods 2 456 elderly patients admitted to General Hospital of PLA for routine physical examination during 1996 to 2005 were involved in present study. The value of different criteria (WHO, IDF and CDS criteria) on the diagnosis of metabolic syndrome (MS) was compared. Results The incidences of MS in the involved patients were 27.2%, 27.0% and 42.6%, respectively, according to WHO, IDF and CDS criteria. The diagnostic coincidence of CDS and IDF criteria was 79.0%. For the 1759 cases with the waistline data, the incidences of MS were 32.3%, 37.8% and 53.8%, respectively, according to the above three criteria. The diagnostic coincidence of CDS and IDF criteria was 76.5 %. According to the criteria of WHO, IDF and CDS, central obesity was detectable in a rate of 35.9%, 49.6% and 52.2%, respectively. Hypertension was diagnosed in a rate of 61.6%, 72.4% and 61.6%, and abnormal glucose metabolism was found in a rate of 24.9%, 40.5% and 24.9%, respectively. The most common clinical symptom complex of MS was hypertension, impaired glucose tolerance and abnormal lipid metabolism. Conclusions A higher coincidence exists between CDS and IDF criteria for MS, but the incidence of MS is higher in CDS criterion compared to WHO and IDF criteria. It is indicated that more attention should be paid on the measurement of the waistline in the elderly people. It seems more appropriate that the waistline data in CDS criterion should be modified from 85cm to 90cm for the elderly people, however, this should be verified by more epidemiological investigation.

10.Molecular Basis of Activation and Replenishment on Satellite Cell Shed Light on Myopathy

Satellite cells, as the major stem cells, are responsible for postnatal skeletal muscle growth, regeneration and maintenance. As a result, satellite cells have great potential as therapeutic agents. Activation of satellite cells in vivo is a key link in the muscle regeneration processes. And their replenishment is necessary to keep the capacity of skeletal muscle to regenerate after recurrence of muscle damage. Cellular and molecular regulation of activation and replenishment for satellite cells issues were reviewed. Hoping through the points of nitric oxide-hepatocyte growth factor (NO-HGF), myostatin and Notch signaling and the niche of satellite cells to overcome the recent obstruction in cell therapy in clinic myopathy, such as Duchenne muscle destrophy.
