1.The effects of Xuebijing injection on immune function of patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the clinical effects of Xuebijing injection on immune function of patients with acute exacer-bation of chronic obstructive pulmonary disease(AECOPD) .Methods 103 patients with AECOPD were divided into 3 groups ,in-cluding control group(33 cases) ,the treatment group 1(36 cases) and treatment group 2(34 cases) .The conventional treatments of COPD including anti-infection ,relieving cough ,expectorant ,antispasmodic ,anti-asthmatic and aerosol therapy were adopted in all patients ,based on the similar conventional therapy in the control group ,50 mL Xuebijing injection with NS 250 mL(1/d ,course of 7 d) were added in the treatment group 1 ,and 100 mL Xuebijing injection with NS 250 mL (1/d ,course of 7 d)were added in the treatment group 2 .Serum CD3 ,CD4 ,CD8 ,CD4/CD8 and IgA ,IgG ,IgM were tested before treatment as well as 4 days and 8 days after treatment .The condition changes in all patients were evaluated .Results After 8 days of treatment ,the total treatment effi-ciency and serum CD3 ,CD4 ,CD4/CD8 ,IgA were significantly changed in the two treatment groups compared with the control group(P<0 .05) ,but serum CD8 ,IgG ,IgM showed no significant difference(P>0 .05);After 4 days of treatment ,the total efficien-cy and serum CD4 ,CD4/CD8 were significantly changed in the treatment group 2 which injection dose was increased compared with the control group(P<0 .05) .The total treatment efficiency and immune function showed no significant difference between the two treatment groups(P>0 .05) .Conclusion Xuebijing injection can improve the immune function and treatment efficiency of AECO-PD patients .Appropriately increase the dose can achieve better clinical results in a short period .In conclusion ,Xuebijing injection is a feasible and effective method for the clinical treatment of AECOPD .
2.Effect of community-based nutrition education program on diet quality of the elderly in Jinshan District of Shanghai

Objective To evaluate the effect of a community-based nutrition education program on the dietary quality of the elderly with diet balance index(DBI). Methods Eighty-three volunteers from Zhujing Town of Jinshan District of Shanghai with age ≥60 years were served as study objectives,and a total of 70 people completed the program.They were given nutritional guidance bimonthly in combination with measures such as community publications,panel discussions and individual interviews.Measurements of dietary intakes,which lasted for 3 d, were performed before intervention and 12 months after intervention,respectively.Diet quality was evaluated with DBI scoring system,and the parameters were compared before and after intervention. Results There were significant differences in DBI-Total Score(DBI-TS),DBI-Lower Bound Score(DBI-LBS)and DBI-Diet Quality Distance(DBI-DQD)before and after intervention for these 70 people (P<0.05).The proportion of subjects with adequate milk and soy product consumption increased from 7.1% before intervention to 20.0% after intervention,that with proper amount of salt intake from 22.9%to 51.5%.and that with diverse diet from 20.0%to 49.9%.The major DBI pattern changed from pattern B (47.1%) and pattern E(34.3%)to pattern B (34.3%) and pattern A (32.9%). Conclusion The community-based nutrition education program for the elderly can improve the diet quality of the elderly.
3.Comparative proteomics analysis of taxol resistance in human ovarian cancer

Objective To analyze the differential expression of protein in taxol-resistance from elderly females with ovarian cancer, by compared proteomics analysis and identified the candidated marker in the clinical tissues. Methods Soluble fraction proteins in two SKOV3 taxol-resistant cells, SK-TR30 and SK-TR2500 and one A2780 taxol-resistant cell, A2780-TR paired taxol-sensitive ovarian cancer cells were separated and prepared by two dimensional gel electrophoresis (2-DE). The differentially expressed proteins were selected and identified by matrix-assisted laser desorption/lonization time of flight mass spectrometry(MALDI-TOF-MS) and database search. 2-DE profiles with high resolution and reproducibility were obtained. Then highly expressed protein-cofilin1 which exist in two forms of de-phosphorylate was detected and compared between human ovarian cancer specimens including 22 elderly female with chemosensitive and 21 elderly female with chemoresistant by immunohistochemistry. Results Among the identified proteins, there were sixteen over/under-expressed proteins in taxol-resistant cell lines as compared with taxol-sensitive cell lines. In particular, overexpression of cofilin1 and destrin were displayed in all three taxol-resistant cell lines. The expression of phosphorylation of cofilin (P<0.05), but not cofilin1, displayed much higher in chemoresistant than in chemosensitive group. Conclusions Cofilin1 may play a role of taxol resistance by phosphorylation of ovarian cancer cell. However, the mechanisms need to be elucidated further.

Methods Fifty-seven patients with acute splenic injury were examined by CT and DSA,treated with splenic arterial embolization and then undertook follow up. 5.Psychosocial factors and the development of breast cancer

Objective To investigate the impacts of psychosocial factors on the development of breast cancer.Methods Thirty-seven patients with confirmed breast cancer and 37 healthy women were enrolled in this study to complete psychological assessment,including ENRICH marital inventory,Life Event Scale(LES),Social Support Rate Scale(SSRS),and Trait Copying Styles Questionnaire(TCSQ).Results In comparison with the healthy controls,the patients experienced more stress events and negative response,and lowered quality of marriage over the last 5 years.The logistic regressive analyses indicated that the important contributors to breast cancer were stress events,objective social support and negative coping styles.Conclusion Stress events,insufficient access to available psychosocial support and negative responses are important risk factors of breast cancer in women.
6.Effect of baclofen on recovery of motor function of stroke patients with hemiplegia

ObjectiveTo explore the best applying time and effect of baclofen on recovery of motor function of stroke patients with hemiplegia.Methods83 stroke patients with hemiplegia were divided into group A and group B according to the degree of muscle tension, every group was divided into the treatment group and control group. Patients of the four groups were treated with routine rehabilitation training, and baclofen was added to the patients of two treatment groups. Motor ability and activities of daily living (ADL) of patients were evaluated before and after one month treatment.ResultsAfter one month treatment, motor ability and ADL of all patients were improved, but in group A, there were not significant differences existed between treatment group and control group (P>0.05), while, there was a significant difference existed in group B(P<0.01).ConclusionBaclofen is necessary to stroke patients with hemiplegia only when the muscle tension increased obviously (≥Asworth 3).
7.Cost-utility Analysis of Alendronate Sodium Preventing Osteoporotic Fractures in Postmenopausal Women

OBJECTIVE:To evaluate the economics of alendronate sodium preventing osteoporotic fractures in postmenopausal women,and to provide reference for osteoporotic and osteopenia patients to select economical and effective therapy plan. METH-ODS:Using Markov economic model,TreeAge Pro 2011 software was used for cost-utility analysis using RCTs literature data of basic medication(calcium and vitamin D3)and basic medication combined with alendronate sodium as baseline data. Using quality adjusted life years(QALYs)and incremental cost utility ratio(ICUR)as economics evaluation indexes,the stability of established model was investigate by single factor sensitivity analysis. RESULTS:QALYs of the basic treatment group was 0.704 years,and QALYs of drug combination group was only 0.708 years. However,ICUR of two groups reached 714252.44. For the late period of hip fracture and other fractures,drug combination group was in high cost and low benefit,obviously had no economic advan-tage. For vertebral fractures,the cumulative probability of fracture in drug combination group was significantly lower than basic treatment group,and ICUR of the two groups was only 13902.17,with economic advantage. For hip fracture,the cumulative prob-ability of fracture in basic treatment group was the same as drug combination group,and ICUR was 19109.00,with economic ad-vantage. For wrist fractures,curative effect and economy needed further study. CONCLUSIONS:For the low-risk population with osteoporotic fractures,calcium and vitamin D3 are more economical in preventing osteoporosis fractures. For the high-risk popula-tion with vertebral and hip fractures,alendronate sodium combined with basic treatment is effective and economical.

Objective To investigate the clinical effecl of trabeculectomy with scleral tunnel in the treatment of refractory glaucoma. Design Prospective, randomized and comparative clinical study. Participants 87 patients (98 eyes) with refractory glaucoma. Methods The patients were randomly assigned to receiving trabeculectomy with or without scleral tunnel. The tunnel group (50 eyes) underwent trabeculectomy with an additional deep scleral tunnel of 5.0mm?1.5mm beneath the superficial scleral flap. The control group (48 eyes) underwent conventional trabeculectomy. The average follow-up period was 6 to 12 months posloperatively. Main Outcome Measures Visual acuity, intraocular pressures (IOP), filtering blebs, operative and postoperative complications. Results (1) No significant differences in visual acuity were found between two groups. (2) The postoperative IOPs were significantly lower than the preoperative IOPs in both groups, while the IOPs on the 7th day after the surgery between the two groups were not different significantly.
