Objective To explore the mechanism of combined treatment with methotrexate (MTX) and Ga-Al-As laser irradiation for rheumatoid arthritis (RA) and to assess the effectiveness of Ga-Al-As laser therapy for RA. Methods Twenty-two patients with RA were randomly and evenly divided into two groups: the treatment group treated with Ga-Al-As laser irradiation combined with MTX and the control group treated with MTX only. Ten age-matched normal subjects were observed as normal controls. The amount of CD4 + CD25 + regulatory T cells in peripheral blood (PB) of the normal controls and that in the PB and synovial fluid (SF) of the 22 patients before and after therapy were counted by flow cytometry. Meanwhile, the amount of prostaglandin E2 (PGE2) in synovial fluid of the patients was measured before and after treatment by enzyme-linked immunosorbent assay(ELISA). Results After combined treatment the clinical symptoms of the patients were improved significantly, and the amount of PGE2in SF decreased significantly. The count of CD4 + CD25 + regulatory T cells in PB of RA patients was ( 3.84 ±3.20) % , compared to ( 10.05 ± 7.04) % in healthy individuals. The count of CD4 + CD25 + regulatory T cells in SF of RA patients was ( 14.89 ± 12.30) % , much higher than that in PB. The count of CD4 + CD25 + regulatory T cells in SF decreased significantly in treatment group compared to control group (P ＜0.05). Conclusion Ga-Al-As laser irradiation eombined with MTX can effectively improve the clinical symptoms of RA patients. It may be related to the decrease of amount of PGE2 and count of CD4 + CD25 + regulatory T cell in PB and SF.

Objective To observe the effect of ritodrine hydrochloride on peripheral blood and tocolysis rate of patients with threatened premature labor.Methods89 cases of patients with threatened premature labor were selected from October 2015 to September 2016 and randomly divided into two groups, research group with 45 cases treated with ritodrine hydrochloride and control group with 44 cases treated with epsom salt.The peripheral blood, extinction time of uterine contraction, extended days of pregnancy, postpartum hemorrhage, neonatal birth weight and tocolysis rate were compared between two groups.ResultsAfter treatment, CRH, NO, PGE2 and IL-8 of control group were obviously higher than research group, the difference was statistically significant (t=10.826,3.839,7.534,8.075,P<0.05).The extinction time of uterine contraction in control group was obviously longer than research group, while extended days of pregnancy and neonatal birth days in research group were obviously longer than control group, the difference was statistically significant (t=4.566,7.855,3.237,P<0.05).There was no significant difference in postpartum hemorrhage between two groups (t=0.889,P>0.05).The overall efficacy of tocolysis in control group was 70.5%, while that in research group was 86.7%, which was obviously higher than the control group, the difference was statistically significant (χ2=7.801,P<0.05).ConclusionRitodrine hydrochloride can effectively control uterine contraction factor in peripheral blood and extend gestational days of patients with threatened premature labor, so as to improve tocolysis rate and birth quality.

Objective To investigate the effect of Propess on cervical collagen remodeling and cervical ripening in full-term pregnancy.Methods 94 cases of mid-term pregnant women enrolled in obstetrics and gynecology of our department from January 2016 to January 2017 were divided into two groupaccording to the random number table method.47 cases of the control group, taking oxytocin intravenous infusion;observation group of 47 cases, taking propess suppository vaginal medication.Labor time, medication to labor time, cervical Bishop score, mode of delivery, postpartum hemorrhage and neonatal situation of two groups were compared.Results Compared with the control group, the Bishop score of the cervix was increased, the total effective rate of cervical ripening was improved, the labor was shortened, the time of medication was shortened, and the delivery rate of vaginal delivery was improved(P<0.05), while the two groups were bleeding, neonatal Apgar score and the incidence of fetal distress were not statistically significant.Conclusion It can promote cervical collagen remodeling, promote cervical ripening, thereby reducing the rate of cesarean sectionwith taking propess in the vaginal.

Objective: To construct a prediction model for sleep disorders in shift workers of a chemical fiber enterprise, so as to provide the basis for early identification and prevention of sleep disorders in shift workers. Methods: Shift workers were sampled from a chemical fiber enterprise in Tongxiang City, Zhejiang Province using a cluster sampling method from August 2022 to July 2024. Demographic information, length of service and average weekly working hours were collected through questionnaire surveys. Depressive symptoms, anxiety symptoms and sleep disorders were evaluated using the Pittsburgh Sleep Quality Index, Patient Health Questionnaire and Generalized Anxiety Disorder Questionnaire, respectively. The shift workers were randomly divided into a training set and a validation set at a ratio of 7∶3. Predictive factors were selected using a multivariable logistic regression model based on the training set, and a nomograph model for prediction of sleep disorders in shift workers was established. The predictive values of the model were evaluated using the receiver operating characteristic (ROC) curve and calibration curve based on the training set and validation set. Results: Totally 673 shift workers were included, with a median age of 32 (interquartile range, 12) years. There were 493 males, accounting for 73.25%. There were 471 (69.99%) workers in the training set and 202 (30.01%) workers in the validation set. There were 274 workers with sleep disorders, accounting for 40.71%. The equation for the prediction model was ln[p/(1-p)]=-8.391+1.906×average weekly working hours+1.822×depressive symptoms+1.667×anxiety symptoms. The area under the ROC curve was 0.769 (95%CI: 0.661-0.835) for the training set and 0.655 (95%CI: 0.593-0.737) for the validation set, and Hosmer-Lemeshow test showed a good fitting effect (both P>0.05). Conclusion The nomograph model constructed by average weekly working hours, depressive symptoms and anxiety symptoms can be used to predict the risk of sleep disorders in shift workers of a chemical fiber enterprise.

OBJECTIVETo explore the auricular point specificity of the electric determination in stroke so as to provide the evidence for the objective study on the auricular point diagnosis in stroke.

METHODSThirty patients of stroke were selected in an observation group; 30 patients of non-stroke internal medicine disorder were in a control group A; 30 persons of healthy examination were in a control group B. In the observation group.and the control group A, beside the conventional treatment, acupuncture was applied to all the cases. In the observation group, Jianyu (LI 15), Quchi (LI 11), Shousanili (LI 10), Hegu (LI 4), Waiguan (TE 5), Liangqiu (ST 34), Zusanli (ST 36), Sanyinjiao (SP 6) and Taichong (LR 3) were selected on the affected side. In the control group A, Yin-tang (GV 29), Baihui (GV 20), Sishencong (EX-HN 1), Fengfu (GV 16), Dazhui (GV 14), Fengchi (GB 20), Tianzhu (BL 10), Jiaji (EX-B 2) C4-C7 were selected. Acupuncture was given once a day in the two groups. The; treatment of one week made one session, and continuously 2 sessions were required. In the control group B, the convention physical examination was provided without any medication and acupuncture involved. The CLRH-A auricular point detector was used to detect the entire auricular point area in the subjects and the positive reaction sites of auricular points in the subjects of each group before and after treatment separately. The auricular point positive detecting score and positive point frequency were observed in the subjects.

RESULTSBefore treatment, the auricular points detecting score in the observation group was obviously higher than that in the control group A and control group B (both P<0. 05). The difference in the score between the control groups was not significant in' comparison (P>0. 05). In two weeks of treatment, the auricular points detecting score in the observation group was reduced apparently (P<0. 05). The difference was not significant before and after treatment in either the control grolip A or the control group B (both P>0.05). In the observation group, the first 15 points with high frequencies of positive reaction were naogan (AT3,4i), pizhixia (AT4), e (AT1), xin (CO15), gan (CO12), nie (AT2), zhen (AT3), shen (CO10), jiaogan (AH6a), pi (CO13) shenmen (TF4), wei (CO4), neifenmi (CO18), shenshangxian (TG2p) and erjian (HX6,7i). The differences were significant in the frequencies of positive reaction at 9 auricular points as compared with the control group A and the control group B respectively, named naogan (AT3,4i), pizhixia (AT4), e (AT1), xin (CO15), gan (CO12), nie (AT2), zhen (AT3), shen (CO10) and jiaogan (AH6a) (all P <0. 05).

CONCLUSIONThe pathological degree of positive reaction about auricular points in stroke patients is higher than those in the patients of non-stroke internal medicine disease and the healthy people. It is verified that the auricular diagnostic method is specific in stroke. naogan (AT3,4i), pizhixia (AT4), e (AT1), xin (C15), gan (GO12), nie (AT2), zhen (AT3), shen (CO10) and jiaogan (AH6a) are correlative with stroke.

Meanwhile, Western medicine considers stress as a kind of information that forms memory trace in central nervous system. Combining the TCM theory with the modern psychological theory on stress, and from the points of views of the emotion regulation, the formation, consolidation, extraction and extinction of fear memory, and the plasticity of amygdala-hippocampus-prefrontal cortex nervous pathway, as well as the results of pharmacological studies of prescriptions and herbs for invigorating the kidney, the authors explained in this article the pathological mechanisms of kidney deficiency due to attack of fear, and revealed the material basis, the action modes, and the neurobiological mechanisms of the kidney in controlling and regulating emotional activity.

This study aims to evaluate the bioactivity of five components of the traditional Chinese medicine complex prescription Jiangzhi granules against hepatocellular steatosis.

A method of derivatization at injection port following three phase hollow fiber microextraction with tetramethylammonium hydroxide (TMAH) as a dual-function reagent, for the simultaneous determination of salicylic acid, p-hydroxybenzoic acid, cinnamic acid, vanillic acid, p-coumaric acid and ferulic acid in beers by gas chromatography was developed. Phenolic acids were extracted from aqueous samples to a thin layer of organic solvent ( hexyl acetate) phase impregnated into the pores of the hollow fiber wall, and then back extracted to an acceptor solution (TMAH) located inside the lumen of the hollow fiber. Upon injection, the phenolic acids were derivatized to their methyl esters in the GC injection port. Several parameters related to the derivatization and extraction efficiency were optimized. The optimized conditions were as follows: hexyl acetate was used as the extraction solvent, an aqueous solution of TMAH (5% W/ V) was used as the derivatization reagent and acceptor phase, the pH value of donor phase was 2. 0, the concentrations of NaCl was 25% W/ V, the stirring rate was 500 r/ min, the extraction time was 40 min. Under the optimal conditions, the linear range of phenolic acids was 0. 50 -15. 00 mg / L, the limits of detection were 0. 05 -0. 18 mg / L. The proposed method was applied to the determination of the phenolic acids in beers. Vanillic acid, p-coumaric acid and ferulic acid were found in the beer samples, others were not detected. The spiked average recoveries were 90. 1% -106. 8% and RSD% was less than 5. 9% (n =3). The method is suitable for the determination of phenolic acids in beers.

Objective To explore the difference involved in the activation of inflammation pathway and the plasma level of inflammatory factors in the subjects with different sorts of insulin sensitivity. Methods The study was carried out in 38 women, consisting of obesity (n = 22 ) and control (n = 16 ) groups according to body mass index. The insulin sensitivity was assessed by homeostasis model assessment of insulin resistance (HOMAIR). Plasma concentrations of interleukin-6 (II-6) and IL-1β were determined by enzyme immunoassay. Western blot analysis was used to examine total protein expression and phosphorylation levels of IκB kinase (IKK) ,inhibitor of nuclear factor-κB ( IκB ) in peripheral blood leukcocytes. Electrophoretic mobility shift assay (EMSA)was used to detect the binding activity of NFκB. Results The levels of fasting plasma insulin[62.2 ( 20.0-127. 0) pmol/L vs 19. 15 ( 14. 2-47. 8 ) pmol/L, P<0. 01], HOMA-IR[2. 32 ( 0. 76-5.49 ) vs 0.70(0.53-1.7),P<0.0l], HbA1 C[(5.42±0. 45 ) % vs ( 5.08 ±0. 38) %, P<0. 05], triglyceride[( 1.75 ±0. 68 vs 1.22 ±0. 58 )mmol/L, P<0. 05], plasma IL-6[3. 15 (0. 03-22. 2) pg/ml vs 1.26 (0. 74-6.06 ) pg/ml, P<0. 01], and IL-1 β[6. 53 ( 0. 84-36 ) pg/ml vs 3. 16( 1.48-8. 86 ) pg/ml, P<0. 01]in obesity group were significantly higher than those in control group. Compared with control group, the levels of IKKo, IKKβ expression and IκBα serine phosphorylation in obesity group were markedly increased, while the expression of IκBα was significantly reduced. Accompanied with the degradation of IκBα protein, the binding activity of NFκB in obesity group was significantly increased. Conclusions The plasma levels of IL-6 and IL-1β were significantly raised in obesity group. The activation of IKK-IκB-NFκB pathway is closely associated with the genesis and development of insulin resistance in obese subjects.
