1.Retrospective analysis of 58 patients with Takayasu' s arteritis

Objective To evaluate the clinical features and curative effect of patients with Takayasu's arteritis.Methods The clinical data of fifty-eight patients,which were divided into different groups according to gender,clinical classifications,disease activity,and so on,were retrospectively analyzed,to compare whether the results of clinical features,laboratory tests or prognosis had significant differences.Results The data suggested the fact that stroke as first manifestation(2/9 vs 0/49,P =0.022) and incidence of hypertension (9/9 vs 28/49,P =0.037),as well as critical hypertension(8/9 vs 18/49,P =0.011) were more common in male patients than in female patients.The active Takayasu's arteritis patients showed that the level of erythrocyte sedimentation rate (ESR),C-reactive protein (CRP),platelet and fibrinogen was elevated,while the level of Albumin/Globulin ratio was decreased.Electrophoresis showed that the elevation of globulin mainly based on globulin α1 and globulin α2 (P < 0.05).The reduction of complement C3 is more common in inactive group than in active group.Conclusion Stroke and hypertension are more common in male Takayasu's arteritis patients than in female patients.The active Takayasu' s arteritis patients showed elevated level of ESR,CRP,platelet and fibrinogen,while the level of Albumin/Globulin ratio was reduced.
2.Comparing inhibition effects of four antihistamines on chemokines and cytokines expression of cultured nasal polyps in vitro

Objective To investigate the inhibition effects of different antihistamines on the expressions of chemokines and cytokines released by nasal polyps in vitro.Methods The fresh nasal polyps resected under endoscope from the patients admitted to our hospital were cultured with mizolastine,cetirizine,loratadine,fexofenadine,dexamethasone,or ciclosporin A respectively for 24 h,with the addition of histamine and arachidonic acid.The total RNA of polyps was obtained and purified with tripure isolation reagent.The mRNA expression levels of monocyte chemoattractant protein-1(MCP-1),monocyte chemoattractant protein-3(MCP-3),regulated on activation,normal T cell expressed and secreted(RANTES),eotaxin were analyzed by RT-PCR.The concentrations of interleukin(IL)-4,IL-5 and tumor necrosis factor-?(TNF-?)in cultured supernatant was detected by ELISA.Results The MCP-1 mRNA expression in nasal polyps treated by mizolastine was weaker than those by loratadine and fexofenadine;the MCP-3 mRNA expression by mizolastine was weaker than those by cetirizine and loratadine;the RANTES mRNA expression by mizolastine was weaker than those by loratadine and fexofenadine;the eotaxin mRNA expression by mizolastine was weaker than that by cetirizine(all P
3. Study of microbial transformation process of Strychnos nux-vomica alkaloid salts by Cunninghamella blakesleeana

Objective: To enhance the transformation rate of brucine and strychnine in Strychnos nux-vomica and establish a high efficient microbial transformation system by Cunninghamella blakesleeana. Methods: Based on the growth and metabolic rule of C. blakesleeana, through single-factor experiments, by using brucine sulphate and strychnine nitrate as substrates and the transformation rate of each substrate as index, several influential factors were investigated. Results: The optimal transformation condition was inoculation amount at 4%, fermentation time at 2.5 d, substrate concentration at 20 mg/L, transformation time at 3 d, culture temperature at 28°C, initial pH of medium at 6.5, and shaking speed at 150 r/min. Under the above condition, the average transformation rates of brucine sulphate and strychnine nitrate were approximate 77.75% and 77.10%, respectively. Conclusion: The average transformation rates of brucine sulphate and strychnine nitrate are increased by about 17% and 22%, respectively by C. blakesleeana, and this microbial transformation system could meet the need of the study on "toxicity attenuation and efficacy reservation" of S. nux-vomica.
4.Diagnosis and management of impalpable testes

Objective To review the management of impalpable testes. Methods Ninty-eight impalpable testes in 82 patients underwent surigcal exploration and all the cases were studied retrospectively. Results 17 testes were found on ultrasonography and 3 of 4 cases were found on ECT pre-operatively.Ninty-eight testes in 82 patients were explored.Eighty-five testes were found,52 in the inguinal canal,33 in the abdomen.Another 13 testes were absent.Ninty-two(94%) testes were found on inguinal exploration or limited laparotomy when necessary. Conclusions Inguinal exploration,with lower abdominal laparotomy if necessary remains the main form of management.If the testes could not be found still laparoscopy should then be performed.
5.Pleural needle biopsy in diagnosis for pleural disease and its safety

Objective To evaluate clinical significance of pleural needle biopsy ( PNB) in diagnosis of pleural disease and its safety. Methods Totally, 594 cases with established pleural effusion, who were undergone PNB at least once in the First Hospital of Changzhou, Jiangsu province from June 1989 to September 2002, were studied retrospectively and analyzed. Results Sensitivity of PNB was 74% (302/ 408) for tuberculous pleurisy and 70. 2% (113/161) for malignant pleurisy. No characteristic pathological changes were found in PNB for the other 25 patients with pleural effusion due to other causes ( SLE, rheumatoid arthritis, leukemia, lymphoma, Castleman disease and Meigs syndrome) . Complications associated with PNB were pneumothorax (69. 0% ) , bleeding (31. 0% ) , fever ( 19. 0% ) and seeding of malignant cells (5. 0% ), and no special treatment was required for them, but one case of tension pneumothorax recovered uneventfully after 5 days of closed drainage. Conclusions PNB was a safe, simple and effective means of diagnosing tuberculous and malignant pleural effusion, and should be attempted extensively at grassroots hospitals. Seeding of malignant cells was uncommon, but should be kept in mind. Repetition of PNB could improve its sensitivity in diagnosis for malignant pleural effusion, with extremely few fatal complications.

8.Effect of Compound Duzhenggan Jiegu Plaster for External Use on the Healing of Closed Distal Radius Frac-ture

OBJECTIVE:To investigate the effect of Compound duzhenggan jiegu plaster for external use on the healing of closed distal radius fracture. METHODS:68 cases of closed distal radial fracture were randomly assigned into treatment group(34 cases) and control group (34 cases). Treatment group was treated with Compound duzhenggan jiegu plaster for external use,and control group was treated with Dieda pills for external use. The changes of X-ray score,BMD and serum ALP content were com-pared between 2 groups after 30 d and 60 d treatment. RESULTS:In 2 groups,there were statistically significant difference in X-ray score,BMD and serum ALP content between 30 d and 60 d treatment(P<0.01). After 60 d treatment,there were statistical-ly significant difference in X-ray score,BMD and serum ALP content between 2 groups(P<0.01 or P<0.05). CONCLUSIONS:Compound duzhenggan jiegu plaster for external use in the treatment of closed radius distal fracture can promote the increase of BMD and serum ALP content,improve fracture repair and healing,shorten the healing time. The mechanism should be further re-searched.
9.The application of Herbert screw in the treatment of navicular bone fracture

Objective To investigate the treatment of Navicular bone fracture with Herbert screw.Methods Twenty-three patients with Navicular bone fracture were collected.They were all conducted with CT scan diagnosis and type,the small incision and targeting wire was used in operation.The Herbert screw was used to fix with pressure,plaster was also used to fix.After operation,the activity training was executed gradually.Results We conducted follow-up of 6-18 months in 23 patients,with an average of 12 months.All patients got bone-healing,the healing time ranges from 7 to 10 weeks,with an average of 9 weeks.There was one case of screw penetrating cortical bone,and one case of mild deformity healing,among the other 23 cases there were no complications including infection,nonunion,necrosis,osteoarthritis'etc.Conclusion The treatment of Navicular bone fracture with Herbert screw fixation can get firm fixation,function training and healing early,and it's worth of clinical promotion.
10.Wnt signaling pathways in osteogenic differentiation of mesenchymal stem cells

BACKGROUND:Studies have shown that Wnt signaling pathways play an important role in the osteogenic differentiation of mesenchymal stem cells.
OBJECTIVE:To review the mechanism and regulation of the Wnt signaling pathways, as wel as Wnt signaling pathway effects on osteogenic differentiation of mesenchymal stem cells.
METHODS:A computer-based search of PuMed database and CNKI database from September 1998 to March 2013 was performed to search related articles. The key words of“Wnt, mesenchymal stem cells, Wnt signaling pathways, osteoblastic differentiation, canonical wnt signaling pathway, non-canonical signaling pathway”in English or Chinese were used to search the articles in the title and the abstract. A total of 31 articles were included to review.
RESULTS AND CONCLUSION:Wnt signaling pathways play a critical role in the osteogenic differentiation of mesenchymal stem cells. Canonical Wnt signaling pathway, non-canonical Wnt signaling pathway, and their mutli-factors were involved in regulating the proliferation and differentiation of mesenchymal stem cells. The osteogenic differentiation of mesenchymal stem cells can be promoted effectively via specific induction of Wnt signaling pathways. Wnt11, FZD6, sFRP2, sFRP3 and Ror2 expressions increase, while Wnt9a and FZD7 decreases during the regulation. However, the relations of factors in Wnt signaling pathways and how to use the mechanism of Wnt signaling for promoting mesenchymal stem cells faster, more accurate differentiation need further studies.
