1.Advances in genomics for ischaemic stroke

Ischemic stroke is one of the major diseases of causing adult disability and death.Age,sex,smoking,hypertension,and diabetes,etc.are the risk factors for ischemic stroke,but they can only partially explain the reasons of stroke onset.Twins,families,and single-gene genetic stroke studies have shown that ischemic stroke has obvious genetic predisposition.In recent years,genomics has made remarkable progress in ischemic stroke study and has shown that many new single nucleotide polymorphisms or genes are associated with ischemic stroke and its risk factors.However,the contribution of these genetic genes for ischemic stroke is still too small and the repetitive studies are needed for further confirmation.
2.Study on Quality Standard for Kangyuan Granula

Objective To establish the quality standard for Kangyuan Granula. Methods Radix Astragali, Radix Paeoniae Alba, Fructus Lycii and Radix et Rhizoma Glycyrrhizae in this prescription were identified by TLC. The content of the Paeoniflorin was determined by HPLC. The column of AichromBond-AQ-C18 (4.6 mm?150 mm, 5 ?m) was used, the mobile phase was Water-Acetonitrile (85∶15), at the flow of 1.0 mL/min, the column temperature of 30 ℃, and peaks were detected at 230 nm. Results The characteristic of identification by TLC was distinct and highly specific. The linear range of Paeoniflorin was 0.0545~0.545 ?g, r =0.999 5, the average recovery was 97.90%, RSD=1.06% (n =6). Conclusion The method was easy to operate with accurate result and good reproducibility. It can be used effectively for the quality control of this preparation.
3.New progress of molecular targeted therapy for breast cancer

Currently targeted therapy of breast cancer therapy has become the hot research field and a kind of brand-new biological treatment mode after three traditional pattern of the surgery, radiotherapy and chemotherapy for breast cancer. Molecular targeted therapy is aimed at the target that may cause cancer cells, such as protocarcinogenic genes and tumor-suppressor genes, cell signaling pathways, cytokines and receptors, antiangiogenesis etc. It can reverse malignant biology behavior to inhibit tumor cell growth from the molecular level and has the advantages of high specific effects and low side effects. This paper focuses on the drug of the molecular targeted therapy for breast cancer and the latest progress of targeted therapy.
4.Method of Bacterial Endotoxin Test of Fluorescein Sodium Injection

OBJECTIVE: To establish a bacterial endotoxin test method for fluorescein sodium injection.METHODS: Based on the bacterial endotoxin test method stated in Chinese Pharmacopoeia 2005 edition(second part),an interference test was conduced on different batches of fluorescein sodium samples using tachypleus amebocyte lysate from different companies.RESULTS: At a concentration of no more than 0.75 mg?mL-1,fluorescein sodium showed no interfere with the bacterial endotoxin test.CONCLUSION: It is feasible for bacterial endotoxin test of fluorescein sodium injection be conducted in a limit value of 0.15 EU?mL-1.

6.Surgical techniques of pelvic autonomic nerves protection in laparoscopic total mesorectal excision for the treatment of rectal neoplasms

Laparoscopic radical resection of rectal neoplasms is still under clinical research currently due to the following 2 reasons. Firstly, compared with open surgery, the longterm efficacy of laparoscopic surgery remains unclear; secondly, the pelvic autonomic nerves are difficult to be exposed and easy to be damaged during the surgery under laparoscope. Till present time, our department has completed 800 cases of laparoscopic radical resection of colorectal neoplasms, in which rectal neoplasms accounted for 70%. For most cases, the pelvic autonomic nerves have been exposed and protected properly. This paper summarized the skills to protect the pelvic autonomic nerves in the laparoscopic total mesorectal excision used for the treatment of rectal neoplasms.
7.The surgical treatment of pectus excavatum in infants

Objective:To review the surgical experience with pectus excavatum deformities in infants.Methods:Retrospective study was carried out on 43 infants with pectus excavatum deformities from January 1994 to October 2001.There were 31 male patients and 12 female patients.40 patients with PE underwent modified sternal elevation and 3 sternal turn-over operation.Results:Except one case of associated pneumonia and another of wound infection,the other cases had normal contours.The follow-up period was 6 months~7 years postoperatively.Except that two had logical notch and one had a flat contour in thorax,all patients had satisfactory results.Conclusion:Pectus excavatum deformities can be early operated.Correction of pectus excavatum by using sternal elevation procedure and fixation with Lucite results in an excellent cosmetic outcome.

9.THE ULTRASTRUCTURAL CHANGES IN THE MYOCARDIA IN PATIENTS SUFFERING FROM PRIMARY DILATED-CONGESTIVE CARDIOMYOPATHY

The histopathological and ultrastructural changes in myocardial biopsy material of 6 patients suffering from primary dilated-congestive cardiomyopathy (DCM P) were studied with following findings: 1. In the hypertrophic myocardial cells there were proliferation and hypertrophy of mitochondria, many of which acummulated beneath the cell membrane, producing finger-like projections as well as increasing the cellular volume. The contractile elements of myocardia did not increase in number and showed so- me degenerative changes. 2. Degeneration of myocardia menifested mainly as condensation of myofila-ments, and seemed to be the contributing factor to impairment of myocardial contraction. Hypertrophy and degeneration seemed to be two different stages of the same pathology as transitional changes from one to another were found. 3. Edema, which might proceed to cellular lysis or necrosis, was another manifestation of degeneration of the myocardium. These findings gave the morphologic basis of impairment of myocardial contraction in primary dilated-congestive cardiomyopathy.
10.Troponin and acute coronary syndromes:more clinical studies needed

Monitoring cardiac troponin Ⅰ(cTnI)or T(cTnT)for the detection of myocardial injury and for risk stratification in acute coronary syndromes(ACS)has been endorsed by the laboratory medicine community(IFCC,AACC,NACB)and the cardiology community(ESC and ACC).The clinical laboratories has long been argued on the best cut-point values for troponin assay-some believe it should be set at the 99~(th)percentile of the reference population,while others believe it should be at the level where the particular assay's coefficient of variation(CV)is less than 10%,with the 10% CV varying by test manufacturer.Besides,there are numerous cTnI measurement systems,confusion regarding cTnI measurements has resulted large variability demonstrated among the cTnI assays.The aim of this opinion is to reflect on some concepts related to using cTn in clinical practice.
