4.Is Helicobacter Pylori the Pathogen of Chronic Tonsillitis?.

BACKGROUND AND OBJECTIVES: Tonsillar tissue is a component of mucosa-associated lymphoid tissue (MALT), which has evolved to protect vulnerable mucosal surfaces. Helicobacter pylori, implicated as an etiological factor in duodenal ulcer and gastritis, induces the appearance of lymphoid aggregates in the stomach. Therefore, we investigate the possibility that tonsils can be extragastric reservoir and target tissue for H. pylori. SUBJECTS AND METHOD: Campylobacter-Like Organism (CLO) test and immunohistochemical (IHC) analysis for H. pylori was performed on 98 patients undergoing tonsillectomy. Chronic tonsillitis group (62 patients) and controlled group (36 patients) were enrolled in this study. Tissue samples were collected from excised palatine tonsils. Two millimeter diameter tissue piece that had been obtained from palatine tonsil specimens were placed in the CLO test kit. Remnant tonsils were sent to the Department of Pathology for IHC analysis. RESULTS: Using the CLO test, H. pylori was detected in 60 (61.2%) of the 98 patients, 39 (62.9%) of 62 in chronic tonsillitis group and 21 (58.3%) of 36 in control group. Using the IHC analysis, it was detected in 54 (55.1%) of the 98 patients, 35 (56.5%) of 62 in chronic tonsillitis group and 19 (52.8%) of 36 in control group. There are no significant differences between chronic tonsillitis group and control group with regard to both CLO & IHC analysis. CONCLUSION: This result suggests that palatine tonsil represents an extragastric reservoir for H. pylori infection, but not a target tissue.

5.Study of voice changes pre- and post-tonsillectomy.

OBJECTIVE:
To evaluate changes in acoustic features of voice after tonsillectomy.
METHOD:
Thirty-three patients presenting for tonsillectomy were recruited. Voice recordings obtained from patients before and 1 month after surgery, were analyzed using the Kay Elemetrics, multi-dimensional voice processing software. The following parameters were estimated: the first formant (F1, B1), average of fundamental frequency (F0), Jitter percent (Jitter), Shimmer, noise-to-harmonics ratio (NHR).
RESULT:
There were significant difference in the first formant frequency and bandwidth before and 1 month after surgery. The larger the tonsil was, the more changes were seen in the formant frequency. No significant differences were found for F0, Jitter, Shimmer, NHR decreased after the surgery.
CONCLUSION
The modifications in the anatomical structure and volume of the vocal tract, induced by the tonsillectomy can change the formant frequency and bandwidth. The larger the tonsil was, the more changes were detected in the formant frequency.

6.Low-temperature radiofrequency technology treatment of spontaneous tonsillar hemorrhage: a case report.

Patient, 30-year-old, male,was admitted to our hospital because of discontinuously spit fresh blood without any inducing factors for three days. In the course, the patient suffered mild dry sensation of pharyngeal, poor spirit condition, fatigue, poor sleep, poor appetite and was with black stool 2 times. Physical examination: T36. 6°C, R 21/min, P98/min, BP135/90 mmHg (1 mmHg = 0.133 kPa). Bilateral tonsils were III hypertrophy and with scar shape surfaces. The left tonsil's surface had longitudinal small blood vessels markedly dilated. His oropharynx, laryngopharynx and laryngeal did not be find any obvious bleeding sites. Laboratory findings: WBC 13.82 x 10(9)/L, N 0.8084, L 0.1632, Hb 81.00 g/L, HCT 25.20; PT 9.60 s, APTT 25.50 s, TT 15.80 s, FIB 1.900 g/L. After 3 hours of admission,the patient spit out fresh blood again,checked the body to see:the left peri-tonsil with fresh blood and found a slowly bleeding site at the 1/3 junction of the middle lower part of left tonsil's rear surface, the size was about 0.5 cm x 0.6 cm. We finally diagnosed spontaneous tonsillar hemorrhage and successfully managed with low-temperature radiofrequency technology.

7.The influence of tonsillar and adenoidal hypertrophy on mental behavior of children.

OBJECTIVE:
To investigate the influence of tonsillar and adenoidal hypertrophy on mental behavior of children.
METHOD:
Mentality and behavior of total sixty-one children suffering from tonsillar and adenoidal hypertrophy were tested with Achenbach Child Behavior Checklist (CBCL) edited by Gong Yaoxian in 1986.
RESULT:
The mentality and behavior of the children suffering from tonsillar and adenoidal hypertrophy were distinctly abnormal compared to control children (P<0.01). The behavioral abnormalities of boys appeared as divisiveness, social difficulties, compel, hyperkinetic syndrome and attack, and the girls appeared as hyperkinetic syndrome and attack, which was significantly different from the control children (P<0.01). The second test was performed three months after tonsillectomy and adenoidectomy, and compared with the first one. Boys were found that the divisiveness, hypochondria, compel and unwell of body cursory mark were evidently lower than the first test (P<0.05). Girls were found that divisiveness, hypochondria, disobeyed discipline and attack mark were evidently lower than the first test (P<0.05). The abnormal rates of mentality and behavior positively correlated with the age of children.
CONCLUSION
Tonsillar and adenoidal hypertrophy evidently affected the mentality and behavior of children. The psychological and behavioral intervention were necessary for children suffering from tonsillar and adenoidal hypertrophy.

10.One case of pediatric obstructive sleep apnea hypopnea syndrome with pulmonary hypertension.

The clinical manifestation included snoring and mouth breathing for 2 years, repeated coughing and shortness of breath in action for more than 1 year. Physical examination of oral cavity showed tonsils were in grade III. The endoscopy showed 2/3 of postnaris were blocked by the adenoids. The preoperative ultrasonic cardiogram revealed the right atrial and right ventricular dilatation, pulmonary artery widened. The preoperative polysomnography (PSG) showed apnea-hypopnea index (AHI) was 28.5 events an hour, and the lowest oxygen saturation (LSaO2) was 39%. The patient was diagnosed as severe obstructive sleep apnea hypopnea syndrome with pulmonary hypertension. The postoperative PSG showed the AHI was 11.7 events an hour, and the LSaO2 was 86%. The ultrasonic cardiogram at 5 months after surgery didn't show any abnormalities.
