2.Evaluation of recurrent gastric malignancy with computed tomography

Purpose:To evaluate the role of CT in diagnosing local recurrence after surgery of gastric carcinoma. Methods:Two groups of 51 patients after surgery of gastric carcinoma were prospectively studied by means of CT performed with the water as oral contrast,hypotonic agent and dynamic or spiral CT technique. 15 patients proved to be without local recurrence measured the thickness and observed the CT findings of anastomosis stoma ,36 patients proved to have local recurrence ,those patients included male 21 and female 15.Results:90.19%(46/51)anastomosis stoma was clearly demonstrated ,and 32 lesions was detected and confirmed the diagnosis in 29 of 32 cases . The CT findings of local recurrence after gastric resection for gastric cancer were irregular gastric wall thickening ( n =14),local mass ( n =6) and mixed lesions ( n =9).Conclusions:Using the optimal technique can improve the detectability and accuracy of CT for diagnosing the local recurrence after surgery for gastric carcinoma.
3.Clinical correlative study on high mobility group box 1 protein after traumatic acute lung injury

Objective To study the changes of serum high mobility group box 1 protein ( HMGB-1 ) in patients with traumatic acute lung injury (ALI) and determine its correlations with MODS,acute physiology and chronic health evaluation ( APACHE Ⅱ ) scores and whether it can predict the incidence of MODS and mortality.Methods Forty cases of ALI were divided into MODS group (n =13) and nonMODS group (n =27) according to the MODS evaluation standard.The serum HMGB-1 was determined in the control group ( 10 healthy persons) and the MODS and non-MODS groups at days 1,4 and 7.The MODS and APACHE Ⅱ scores were also evaluated.Results Between the groups,the HMGB-1 level at days 1,4 and 7 in non-MODS group was higher than that in the control group,while lower than that in MODS group (P<0.05).Within the non-MODS group,the expression of HMGB-1 showed a high level at day 4 compared with that at day 1 ( P <0.01 ) ; the HMGB-1 showed a low level at day 7 with clinical symptom improvement compared with that at day 4,and was even lower than that at day 1 ( P < 0.01 ).Within the MODS group,the serum HMGB-1 level was significantly increased,and continued for several days; the HMGB-1 was decreased slightly at day 7 in comparison with that at day 4,but was still higher than that at day 1.The difference of MODS and APACHE Ⅱ score was significant,with the dynamic change of HMGB-1 level at days 1,4 and 7 after traumatic ALI.The correlative analysis showed that HMGB-1 expression level was remarkably related with MODS score and APACHE Ⅱ score.Conclusions HMGB-1 shows a high expression in patients with traumatic ALI.As a late mediator of inflammation,HMGB-1 usually increases relatively late and lasts for a long duration.HMGB-I level and concurrency MODS are closely related.Routine test of serum HMGB-1 level and joint assessment of MODS and APACHE Ⅱ score are contributive to the prediction of organ dysfunction after traumatic ALI.
4.Analysis of IL-17 and IL-10 expressions of patients with hepatic cystic echinococcosis in cyst stages

Objective To explore the significance of IL-17 and IL-10 in patients with different types of hepatic cystic echinococcosis ( CE ) in cyst stages , to investigate the values of evaluation of hydatid cyst activity . Methods 33 patients with hepatic hydatid disease and 21 healthy people (control group) were enrolled, divided into 4 groups, according to WHO ultrasonic classification for CE:Control group (n=21), CEⅠ,Ⅱgroup (active, n=14), CEⅢgroup (transitional, n=10), and CEⅣ,Ⅴgroup (inactive, n=9). The serum concentrations of IL-17 and IL-10 were measured by enzyme-linked immunosorbent assay. Results Serum levels of IL-17 in CEⅢgroup were significantly higher than other groups (P<0.05). There were no significant difference in serum IL-17 levels between CEⅠ,Ⅱgroup [(38.57 ± 3.83) pg/mL] and CEⅣ,Ⅴgroup [(40.22 ± 5.15) pg/mL]. Serum levels of IL-10 were significantly higher in CEⅢgroup [(14.65 ± 2.15) pg/mL] than CEⅠ,Ⅱgroup [(11.38 ± 0.71) pg/mL] (P<0.05), while CEⅠ,Ⅱgroup were significantly higher than CEⅣ,Ⅴgroup [(7.60 ± 0.46) pg/mL] (P<0.05). There were no significant difference of IL-17/IL-10 ratio between CEⅠ, Ⅱgroup (3.44 ± 1.04) and CEⅢgroup (4.60 ± 2.40), and there were no significant difference of IL-17/IL-10 ratio between CEⅢgroup and CEⅣ,Ⅴgroup (5.39 ± 1.95). IL-17/IL-10 ratio in CEⅠ, Ⅱgroup was significantly lower than CEⅣ, Ⅴgroup (P<0.05). The serum concentrations of IL-17 has no correlation with IL-10 in different cyst stages. Conclusions The synergy effect of IL-17 and IL-10 may play the certain role to promote the parasite progress. Neither IL-17 nor IL-10 level could be as an independent marker of echinococcal cysts activity, while IL-17/IL-10 ratio reflect the hydatid cyst activity in certain degree, but as a echinococcal cysts activity marker, further evidence should be needed.

6.Total subadventitial cystectomy for the treatment of 30 patients with hepatic hydatid cyst

Objective This study was to evaluate a new radical resection of hepatic hydatidosis. MethodSince 1999, total subadventitial cystectomy through the intraadventitial space of the hepatic hydatid cyst was performed on thirty patients with 45 liver hydatid cysts. Results The procedure was successful in all cases with no intraoperative rupture of the cysts. The average blood loss per cyst was 20?ml. There was no operative mortality, nor postoperative complications. Twenty one cases were followed up for an average period of 11 months with no recurrence. Conclusions The results indicate that the new procedure is a safe and radical alternative for the treatment of hepatic hydatidosis.
7.Study on quality of life and its influencing factors of urinary incontinence in elderly women

Objective To investigate quality of life and its influencing factors among elderly women with urinary incontinence(UI) living at nursing homes in Beijing. Method A randomized method was used to select six districts in Beijing. All the nursing homes in these 6 districts were included in our study,642 eligible elderly women participated in the survey. Quality of life of 244 women who were identified with different type UI were assessed. Bivariate analysis and multiple analysis were performed. Results The average score of quality of life was 8.5 ± 6.6. Scores of social embarrassment, activity & limiting behavior, psychosocial impacts were 1.8 ± 1.4,4.0 ± 3.5 and 3.3 ± 2.7 respectively. In bivariate analysis, significant differences were found by type of UI, severity of UI,course of UI, health condition, income level and self-care ability. In multiple analysis after adjustment of other variables, UI course≥5 years and inability to take care of herself were risk factors of quality of life.Conclusion Quality of life of elderly women with UI is poor, specific nursing and treatment should be conducted.
8.The influence of whole nursing model on health-related knowledge awareness and satisfaction rate of patients with liver hydatidosis receiving pericystectomy

Objective To observe the influence of whole nursing model on health-related knowledge awareness and satisfaction rate of patients with liver hydatidosis receiving pericystectomy.Methods 180 patients in hospital receiving resection of liver hepatic cyst were randomly divided into the observation group and the control group with 90 patients in each group.The control group was given usual care,while the observation group received whole treatment model.The patients' health-related knowledge awareness and satisfaction rate were compared between two groups.Results The rate of health-related knowledge awareness and satisfaction about nursing work in the observation group preceded the control group.Conclusions This whole treatment model can boost the awareness rate of health-related knowledge and nursing satisfaction,which not only provides the care with full,comfortable and efficient services,but also advances the self-care capacity and cognition of disease of patients.
9.Effects of curcumin on mRNA expression of cytokines related to Tou-like receptor 4 signaling in THP-1 cells

Objective To investigate the effects of curcumin on mRNA expression of cytokines related to Toll-like receptor (TLR) 4 signaling in THP-1 cells.Methods After pretreatment with different concentrations (50,25,12.5 mg/L) of curcumin or dexamethasone for 12 hours,THP-1 cells were stimulated by lipopolysaccharide (LPS.1 mg/L) for 4 hours followed by the collection of cells.Then total RNA was isolated from these cells and subjected to reverse transcription-polymerase chain reaction (RT-PCR) for the detection of mRNA expression of tumor necrosis factor receptor-associated factor 6 (TRAF6),interleukin-1 receptorassociated kinase (IRAK1) and nuclear factor (NF)-κB.THP-1 cells without pretreatment or stimulation served as negative control,and those only stimulated with LPS served as LPS group.Results After stimulation with LPS (1 mg/L) for 4 hours,the mRNA expressions of TRAF6,IRAK1 and NF-κB were significantly upregulated in THP-1 cells compared with negative control cells (f=38.69,39.13,23.99,all P<0.01).Curcumin of 50 mg/L and 25 mg/L significantly inhibited the mRNA expressions of TRAF6.IRAK 1 and NF-κB upregulated by LPS with an inhibition rate of more than 50% (all P<0.0 1).Conclusions Certain concentrations of curcumin can inhibit the mRNA expressions of TRAF6.IRAK1 and NF-κB.which demonstrates the regulatory effect of curcumin on the mRNA expressions of TLR4 signaling pathway-associated cytokines.
10.Investigation on prevalence and risk factors of urinary incontinence among aged women at nursing home in Beijing

Objective To evaluate prevalence and risk factors of urinary incontinence(UI)among aged women living at nursing homes in Beijing,and provide references of relative health strategy.Methods Totally 642 women aged 60 years and over were randomly selected at nursing homes from 6 districts in Beijing.A questionnaire survey was conducted using International Consultationon Incontinence Questionnaire Female Lower Urinary Tract Symptoms Module (ICIQFLUTS) recommended by International Consultation on Incontinence (ICI).Results The prevalence rate of UI was 38.0% (244/642) among whom 38.5% (94/244) of participants knew their health conditions,7.8% (19/244) visited hospital and 6.6% (16/244) received medical or operational treatment.The proportions of respiratory disease ( 27.0 % vs.14.6%),digestive disease (23.4%vs.15.3%),urological disease(24.6%vs.8.5%),cardio-cerebralvascular disease(74.2% vs.53.5%),neurological disease ( 9.4 % vs.5.0%),obstetric disease ( 27.5% vs.10.1%),menstrual disorder (21.4%vs.14.4%),getting married before aged 21 years (51.5%vs.40.8%),being pregnant more than 4 times(50.2%vs.40.9%),the experience of both cesarean and natural birth (4.0%vs.0.8%),constipation(40.2% vs.29.4%),fecal incontinence ( 13.1% vs.4.0%),uneasy movement ( 22.5%vs.10.6%) and drinking (8.6%vs.3.5%) were higher in women with UI than in women without UI (P<0.05 or P<0.01).Eight risk factors that were entered into logistic regression model included getting married before 21 years old (OR =1.5,95 % CI:1.0-2.3,P<0.05),being pregnant more than 4 times(OR=1.7,95%CI:1.1-2.4,P<0.05),menstrual disorder(OR=1.7,95%CI:1.0-2.7,P<0.05),respiratory disease(OR=1.6,95 %CI:1.0-2.6,P<0.05),urological disease(OR=2.4,95 %CI:1.4-4.1,P< 0.01 ),cardio-cerebralvascular disease ( OR =2.0,95 % CI:1.3-3.0,P < 0.01 ),obstetric disease(OR=3.4,95%CI:2.0-5.6,P<0.01),moving uneasily(OR=2.5,95%CI:1.5-4.0,P<0.01).Conclusions UI is a common disease among women residing at nursing homes.Specific measures including intervention and clinical treatment targeting risk factors should be conducted.
