8.Post-operational surgery of colorectal cancer for metastatic tumor on caudate lobe of liver:report of 18 cases

Objective To evaluate the techniques and effects of post-operational surgery of colorectal cancer for metastatic tumor on caudate lobe of liver. Methods For the patients admitted from Jan. 1999 to Jan. 2007 to the Navy General Hospital of PLA and undergone metastasectomy on caudate lobe of liver after colorectal cancer operation, the operative procedure and effects were studied retrospectively. Results A total of 18 cases of metastasectomy on caudate lobe of liver were successfully performed. The mean diameter of the metastases was 6.3cm (3.2-11.3cm). The types of operation included total caudate lobectomy (6 cases), total caudate lobectomy combined with extended hepatectomy (9 cases) and partial caudate lobectomy (3 cases). The mean operative time was 198min (154-360min) and the average intraoperative blood loss was 975ml (400-3250ml). No intraoperative death occurred, while the remarkable complications were found in 5 cases (27.8%). The survival rates of follow up for 1, 3 and 5 years were 83.3%, 61.1% and 27.8%, respectively. Conclusions The metastases on caudate lobe of liver after colorectal carcinoma resection usually involve all the hepatic portals. Surgical resection, though sophisticated in technique, remains to be a safe and effective treatment, and the first choice for treating metastases.
9.Evaluation on function of knee reconstruction with LARS artificial ligament of the ACL

Objective To explore the clinical effect of ligament advanced reinforcement system ( LARS) artificial ligament reconstruc-tion of the anterior cruciate ligament ( ACL) in the operation. Methods Sixty patients with ACL injury in our hospital from September 2011 to May 2013 were randomly divided into LARS group and autologous hamstring tendon ( group ST/G) group with 30 cases in each group,the Lysholm score,IKDC score,the difference of clinical effect and bone tunnel of patients in two groups before treatment and at 3rd,6th,9th, 18th months after treatment were compared. Results The time of walking with crutch,ambulation and restore movement in LARS group were obviously shorter than those in ST/G group(P<0. 05). The Lysholm score,IKDC score,the difference of clinical effect and bone tunnel of patients in two groups after treatment were better than those before treatment(P<0. 05). The difference of Lysholm score and IKDC score be-tween two groups before operation was not significant (P>0. 05). The Lysholm score and IKDC score of LARS group at 3rd,6th,9th months after operation was higher than that of ST/G group,and at 18th months after operation,the difference of Lysholm score and IKDC score be-tween two groups was not significant (P>0. 05). At the last follow-up,the distribution effect of LARS group was higher than that of ST/G group (P<0. 05). The excellent and good rate of LARS group (93. 33%) was higher than that in ST/G group(76. 67%),but the difference was not significant (P>0. 05). There were 6 cases of patients with bone tunnel enlargement in LARS group,14 patients had bone tunnel en-largement in ST/G group. The bone tunnel enlargement rate was significantly higher than that of the LARS group (P<0. 05). Conclusion Ligament advanced reinforcement system of artificial ligament in anterior cruciate ligament reconstruction with autologous hamstring tendon re-construction can significantly speed up the recovery of the patients,and improve the postoperative curative effect and reduce the incidence of postoperative bone tunnel enlargement.
10.Correlation between revascularization effect and calcium content of a kind of man-made compound bone on the femoral head defects and necrosis of canines

[Objective]To observe the effect of composite(AACB/BMP/bFGF)of allogeneic antigen-extracted cancellous bone(AACB),bone morphogenetic protein(BMP)and basic fibroblast growth factor(bFGF)on the repair of defective and necrotic areas of the femoral head,and evaluate the revaseularization the femoral head defect and necrosis model using liquid nitrogen,and randomly divided into 3 groups with 12 femoral heads in each group,which were implanted with nothing(group A),AACB/BMP(group B)and AACB/BMP/bFGF(group C)respectively.Every femoral head was implanted with about AACB 0.33 g,BMP 12.5 mg and bFGF 2000 U.Two canines were sacrificed respectively at 3th,6th,and 12th weeks after operation in every group.Histologically,immunohitochemical vessel staining,vessel count and vessel surface area image analysis,calcium content and the correlation between revascularization effect and calcium content were investigated in the femoral heads after the operation.[Result]Histology,the detects were still infilled by fibrillar tissue in group,A;there were a great deal of new bone formed in group B;and the detects were repaired completely in group C 12 weeks after operation.Measurement of vessel count and vessel surface area showed a large quantity of new vessel proliferated around the infilling material in group C on the 3rd week.The vessel count and vessel surface area continuously increased on the 6th week and 12th week.They were more than those of group B and group A at every definite time point(P
