Objective To investigate the application of laparoscopic cholecystectomy(LC) in the liver,cross ditch right notch identification,prevention of bile duct injury anatomy of cystic duct.Methods 224 cases with liver transverse groove had right notch,all operations in the liver transverse groove right notch on the separation of anatomic posterior Calot's triangle LC operation.Results 224 cases of LC were performed smoothly,the entire group without the occurrence of bile duct injury,hemorrhage,conversion to open surgery,postoperative all patients recovered were well cured.Conclusion Liver transverse groove notch on the right posterior Calot's triangle anatomic helps to avoid LC operation structure and variation of extrahepatic bile duct,which can effectively prevent the biliary duct injury.

Objective:To evaluate the efficacy of 5-LOX inhibitor zileuton on the proliferation and apoptosis of human gastric carcinoma cell line SGC-7901,and to explore the mechanisms of apoptosis induced by zileuton.Methods:The proliferation of SGC-7901 cells was detected by thiazolyl blue tetrazolium bromide(MTT)assays.TUNEL staining was used to evaluate the apoptosis of SGC-7901 cells.The changes of Bax,Bcl-2 and caspase-3 mRNA expression were studied by reverse transcription polymerase chain reaction.Bax,Bcl-2 and caspase-3 protein expression were analyzed by immunocytochemistry in SGC-7901 cells treated with zileuton.Results:Zileuton can decrease the proliferation significantly in a dose-dependent and time-dependent manner in SGC-7901 cells.TUNEL staining showed that the apoptosis rate was elevated with the increase of zileuton concentration(P

Objective To evaluate the clinical efficacy of procedure for prolapse and hemorrhoids (PPH)plus Milligan-Morgan methods for circumferential mixed hemorrhoid.Methods A total of 74 cases with circumferential mixed hemorrhoid were treated by PPH plus Milligan-Morgan methods.Results The anal form of all cases were changed correctly.There was no complication such as anal edema,incontinence,anal stenosis,postoperative massive bleeding and infection.Conclusion PPH plus Milligan-Morgan methods is a safe and effective way for circumferential mixed hemorrhoid.This way can decrease postoperative complications and improve the curative effect.

Objective To examine the ability of cancer metastasis to lymph nodes by study of the h(?)o,(?)thology of early gastric cancers. Methods Eighty-one cases of early gastric cancers with lymph node(?)etastasis were used as metastatic group. Another randomly selected 81 cases of early gastric cancer without lymph node metastasis were used as control group. Both groups consisted of 11 cases of intramucosal cancer and 70 cases of submucosal cancer. The location of cancers in tissue was further classified as two categories; the superficial lesions and invasive fronts. In addition, the gastric cancers were hislopathologically divided into three subtypes: differentiated, undifferentiated and the mixed type. The histopathological classification and location of cancers in tissue were similar between the two groups. ObjectiveTo investigate the effects of intense pulsed light(IPL) and radiofrequency radiation on the appearance,histopathological manifestation,dermal depth and collagen content in BALB/c mouse skin.MethodsThe back of BALB/c mice was irradiated with intense pulsed light(IPL group,n =9),radiofrequency (RF group,N =9),IPL and radiofrequency (IPL+RF group,n =9) respectively,for 4 sessions at 1-week intervals followed by 2 sessions at 2-week intervals.Three mice remaining untreated served as the control group.The appearance of skin on the back of mice was observed at different time points after irradiation.Some mice were sacrificed and skin specimens were obtained from the back at week 4,8 and 12 after the first irradiation.Hematoxylin and eosin staining was performed to observe the histopathological manifestation and measure the dermal depth of mouse skin,and Masson staining to detect the expression of collagen fiber in mouse skin.ResultsNo local adverse effects were observed in the back skin of mice after irradiation with IPL or radiofrequency.Since the 8th week after the first irradiation,the dermal layer had appeared to be thickened,with an increase in the content of collagen fiber and extraeellular matrix,in the IPL group,RF group and IPL+ RF group compared witb the control group,and the increase was more obvious in the IPL+RF group than in the IPL group and RF group (all P ＜ 0.05 ).Increased collagen fiber was densely arranged in irradiated mice.There was no apparent thickening of skin epidermis after irradiation.ConclusionsBoth IPL and radiofrequency can stimulate an increase in dermal collagen production in mice,and a synergistic effect exists between IPL and radiofrequency in the biostimulation of collagen synthesis.

Objective To investigate the applicetion value of combined ultrasonic scalpel in laparoscopic cholecystectomy(LC).Methods 140 patients with acute and chronic cholecystitis,gallbladder stone patients,received intraoperative ultrasound combined electrosurgical knife,suction devices auxiliary red line LC.The clinical effects were observed.Results 138 cases were completed cholecystectomy.Blood loss ( 8.16 ± 1.85 ) ml,operation time(38.13 ±4.35) min,Postoperative hospital stay(3.25 ± 1.18)d,4 cases of intraoperative rupture of the gallbladder,Hepatobiliary burns in 1 case,2 cases because of dense adhesions around the gallbladder and gallbladder more extensive anatomical triangle unclear laparotomy.Conclusion The rational use of combined high-frequency ultrasonic scalpel knife,suction devices auxiliary red laparoscopic cholecystectomy,surgical conditions that meet medicalrecords,are safe and reliable,quicker recovery can be achieved by minimally invasive medical treatments.

Object A study on the ingenol-type diterpene esters of Euphorbia kansui T. N. Liou ex T. P. Wang. Methods Their structures were elucidated by spectral analysis, especially 2DNMR ( 1H- 1HCOSY, HSQC, HMBC and NOESY). Results Six ingenol-type diterpene esters were isolated successfully, two of them were identified as 3-O-(2′E, 4′Z-decadienoyl)-20-O-acetylingenol (Ⅰ) and kansuiphorin C (Ⅱ). Conclusion NMR data of compound Ⅰ and 13 CNMR data of compound Ⅱ were first reported. On the basis of variety of 2DNMR experiments, NMR data of compound Ⅰ and Ⅱ were well assigned.

Objective To explore the histopathological features of renal lesions in patients with prima- ry Sjgren's syndrome(pSS)and the correlation between the findings with clinical manifestations.Methods Kidney and labial salivary gland biopsies from 16 patients with pSS were studied by a semiquantitative assess- ment approach.Results Among these pSS patients,light microscopy showed acute or chronic interstitial nephritis(IN,8/16),diffuse mesangial proliferative glomerulonephritis(MePGN,3/16),mild mesangial prolifer- ative glomerulonephritis(3/16),focal segmental glomerulosclerosis(FSGS,2/16),membranoproliferative glomeru- lonephritis(MPGN,1/16)and membranous nephropathy(MN,2/16).There were 11/16(68.8%)cases had renal tubular acidosis(RTA),8 cases of them presented with IN.In two chronic renal failure cases,one showed MN and another showed IN accompanied with FSGS.Immunofluorescent examination revealed deposition of IgG, IgA,IgM,C3,Clq on the glomerular basement membrane or tubular basement membrane,mesangium focally.It was noted that the interstitial inflammation was focal in 85% of the patients and was always combined with dif- fusely distributed cellular infiltrates.The percentage of obsolescent glomeruli was found correlated with the level of vessel wall thickening(r=0.77,P=0.001)but not with the grade of tubular atrophy and interstitial fi- brosis.Creatinine clearance was correlated with the adjusted chronic tubulointerstitial nephritis(CTIN)score (r=-0.52,P=0.04)and the percentage of obsolescent glomeruli(r=-0.53,P=0.04).No association could be found between the labial salivary gland and the renal focus scores(r=0.28,P=0.30).Conclusions The renal tubular acidosis and IN are the most common findings in pSS patients with renal involvement.Glomerular dam- age is not uncommon,althought mild and is not merely secondary.Timely renal biopsy can help to estimate prognosis and guide therapy.
