1.Subcutaneous tissue calcification in a patient with rheumatoid arthritis.

Subcutaneous tissue calcification in rheumatic diseases usually occurs in connective tissue diseases, such as systemic lupus erythematosus, scleroderma, and dermatomyositis. Domestic cases of calcification in rheumatoid arthritis have not been reported. The mechanism of subcutaneous tissue calcification may differ depending on the cause and it can develop on all parts of the body. Calcification occurring in rheumatic diseases is a major mechanism of tissue damage caused by chronic inflammation. No standard therapy for calcification has been established; however, many studies have reported on medical and surgical treatment. We report on subcutaneous tissue calcification in a rheumatoid arthritis patient tissue calcification on both sides of the buttocks, the upper limbs, and the lower limbs.

2.Anterior Uveitis and Ankylosing Spondylitis.

Recently ankylosing spondylitis have been found to be the most common one among the joint diseases which is associated with anterior uveitis. About 10 to 60% of patients with Marie-Strumpells' ankylosing spondylitis have anterior uveitis. Ankylosing spondylitis is mostly found in young male and characterized by makedly elevated sedimentation and radiological changes; earliest finding in sacro-iliac joints are subchondral sclerosis or demineralization of the bone near the sacro-iliac joints. The joint margins gradually become less distinct until ankylosis develope. The vertebral bodies show "squaring" and the longitudinal ligaments show calcification and ossification on x-ray film (Bamboo spine). The uveitis associated with ankylosing spondylitis is a mild to severe non-granulomatous type (sometimes granulomatous) involving the anterior segment exclusively. Usually it affects one eye at a time. Recurrent attacks may lead to permanent damage depending on the severity and frequency of the attacks and the adequacy of treatment. The main treatment is the administration of steroids. A 35 year old man was seen in our clinic and found to have recurrent anterior uveitis with: hypopyon in his right eye and ankylosing spondylitis while was confirmed by char;acteristic radiological findings. Pathients with acute recurrent non-granulomatous iridocyclitis without apparent cause should have an x-ray of the lumbosacral spine and sacro-iliac joints.

3.Biomicroscopic Observation of Plica Semilunaris and Caruncle.

The plica semilunaris is a narrow crescentic fold of conjunctiva, corresponding to the third eyelid or nictitating membrane of the lower vertebrates. The caruncle is a small, soft, pink ovoid body, situated in the lacus lacrimalis to the medial side of the plica semilunaris. Plica semilunaris and caruncle were described morphologically by many investigators in foreign countries but Korea. The shape of plica semilunaris, the extent of exposure of the caruncle and hairs on the caruncle, in 960 normal Korean eyes with no history of external diseases, were observed biomicroscopically. The results were as follows: 1. In primary position, lateral free margin of the plica semilunaris was devided morphologically into 5 forms. among which 2nd form was most frequently seen but 3rd and 4th forms were rare. The 1st form was seen below age 20, whereas 5th form over 21. 2. In abducting position, form of the plica semilunaris was devided into 8 forms, 6th form was most frequent in male but 1st form in female. This discrepancy between male and female was of statistical significance. 3. The extent of exposure of the caruncle in palpebral fissure was devided into 6 forms depending on the ages. Below the age of 20, the 1st, 2nd and 3rd forms, caruncle is exposed less than half in size, were observed mostly. Over the age of 21, 4th, 5th and 6th forms, caruncle is exposed more than half in size, were seen mostly, 5th form was frequent in male and 4th form in female. It was statistically significant that the more is the extent of exposure of the caruncle, the older they are. 4. In the caruncle, the hairs were one to ten in number mostly, black in color mostly, directed toward medial canthus or forward, situated in lower portion near the medial canthus.

4.The Assessment of Stress between White and Blue Collar Workers by using Psychosocial Well-being Index.

This study was conducted to evaluate the degree of stress state and the risk factors related to in 967 white collar workers and 275 blue collar workers buy using Psycosocial Well-being Index. The results obtained were as follows: 1. In the white collar workers and blue collar workers, young age, low education and low income groups had high scores of stress, while in white collar workers, female had high scores of stress but in blue collar workers male had high scores of stress. 2. According to psychosocial well-being index, mild stress state were 73.9% and 53.1%, high risk stress state were 8.9% and 44.4%, and healthy state were 17.2 and 2.6% in white collar and blue collar workers respectively. 3. The total stress score was highly associated with the factors of depression, social performance and self-confidence, general well-being and vitality, and sleeping disturbance and anxiety in order. 4. In reliability test of stress factors, Cronbach's coefficients of depression, social performance and self-confidence, general well-being and vitality, and sleeping disturbance and anxiety were 0.89, 0.81, 0.79, and 0.74 respectively. In conclusion, it suggested that age, sex, marital status, income, education, sleeping time, smoking and exercise habit were associated with stress score, all of above factors should considered to occupational health.

5.Needs, Uses and Evaluation of Internet Health Information Among Students in a Provincial University.

OBJECTIVE: This study aims to investigate the needs, uses and evaluation of Internet health information among students in a provincial university. METHODS: Nine hundred and sixty one student from seven different classes of S University located in Chungnam province participated in this study from 2nd to 9th May, 2005. A self-reported questionnaire was used to collect data. Collected data were analyzed on SPSS 11.0. RESULTS: 58.2% of them have needed some health information before and 84.6% of them needed the health information offered by a University. 67.2% of them use the Internet more than any other resource to acquire Health Information. 90.9% of them used a search engines to get health information on the Internet. 57.6% of them have searched for Internet health information to get information about specific disease or medical problem. The most frequently searched topics were LASIK(laser in-situ Keratomileusis), diabetes, and contraception. 39.6% of them evaluated the Internet health information was reliable and 55.8% of them satisfied with the Internet health information. CONCLUSION: Universities and their in-campus health centers are needed to offer the trustworthy Internet health informtaion on their official homepages so that the student can refer to the reliable and qualified Internet health information.

6.Delayed recovery of neuromuscular blockade by rocuronium in a hypothermic patient during a laparoscopic procedure: A case report.

Perioperative hypothermia can occur frequently and lead to various complications. Laparoscopic procedures do not require a large incision or exposure of the viscera, but a prolonged and high pressure procedure can lead to unexpected hypothermia. Hypothermia prolongs the time-course of non-depolarizing neuromuscular blocking agents by reducing the plasma clearance significantly. Rocuronium, a new aminosteroidal neuromuscular blocking agent, can be influenced by reductions in plasma clearance and prolong the duration of action. Therefore, patients must receive ventilator care in the ICU. Aggressive measures were taken for hypothermia, and the patient was discharged without any complications. We report a case of delayed of neuromuscular blockade by rocuronium in a hypothermic patient during a laparoscopic procedure with a brief review of literatures.

7.Ankylosing spondylitis associated with balanced reciprocal X-1 translocation.

A number of research papers have reported more frequent occurrence of rheumatic/autoimmune disease among patients with hypogonadism or a chromosomal anomaly with potential X-chromosome defects. A 30-year-old female patient came to the hospital with a main cause of bilateral buttock pain, which began two years ago and worsened seven days ago. Ankylosing spondylitis with invasion of both sacral-iliac joints was observed. On magnetic resonance imaging, although the uterus was observed normally, an ovary was not observed. In a chromosome test, balanced reciprocal X-1 translocation of 46,X,t(X;1)(p10;q10) was diagnosed. Here, we report on the first case involving ankylosing spondylitis accompanied by balanced reciprocal X-1 translocation.

10.Health related practices and morbidity among adult in rural area.

This research was carried out to determine the performance rate of health related practices, to measure the agreement between morbidity by doctor's diagnosis and morbidity by 'subject' self-reported and the degree of association between health related practices and morbidity rate by doctor's diagnosis, to identify their effects on morbidity among rural area populations. The data were gathered by volunteer residents(over the age of 20) of Haman Myeon, Haman Gun, Kyeongsangnam Do in Korea, from June 10, 1993 to June 12, 1993 (369 male and 516 female). Face to face interview, lab, chest P-A, EKG and physical examination were completed. Descriptive statistics, agreement analysis and multiple logistic regression procedures were employed for analyses. The results of the study were summarized as follows : 1) Age adjusted morbidity rates by doctor's diagnosis and self-reported were 38.5% (male:37.3%, female:36.5%), 26.4% (male:33.3%, female:27.5%), respectively. Kappa coefficient between morbidity by doctor's diagnosis and morbidity by self-reported was 0.21 (male:0.21, female:0.22). 2) The frequency of disease by doctor's diagnosis was as follows: hypertension (15.3%), gastritis (9.6%), diabetes mellitus (8.5%), liver disease (8.1%), and degenerative arthritis (6.2%) in the study population. 3) Order of health practice performance rate was as follows: Males- normal body weight (62.1%), non-heavy alcohol consumption (57.5%), 7-8 hours of sleeping (50.1%), non-smoking (21.7%), and exercise (19.8%). Females- non-heavy alcohol consumption (97.3%), non-smoking (84.7%), normal body weight (57.8%), 7-8 hours of sleeping (45.0%), and exercise (9.9%). 4) There was no significant relationship between health related practice and morbidity except exercise among health related practices. 5) Health related practice index which was recategorized by high, medium, and low had effects on the probability of developing morbidity.
