4.Clinical analysis of otogenic Mouret abscess: a case report.

Mouret abscess is a rare extracranial complication of suppurative otitis media. It is generally believed to be a deep neck abscess caused by inflammation leading to the rupture of the bony tip of the mastoid tip. The location of Mouret abscess is deep. The symptoms are insidious at the onset, but may eventually spread to the surrounding tissue, and even lead to mediastinal abscess, cavernous sinus thrombosis, meningitis, dyspnea and other serious complications. At present, with the popularization of antibiotics, the occurrence rate of Mouret abscess is very low, and only sporadic cases have been reported.In this paper, a case of Mouret abscess caused by cholesteatoma was analyzed to explore Mouret abscess in terms of the route of infection, clinical manifestations, imaging features, diagnosis and treatment.

5.Cochlear Pathology in Experimental Otitis Media with Effusion ofGuinea Pig Caused by Endotoxin.

BACKGROUND AND OBJECTIVES: Injection of endotoxin into the middle ear of experimental animals results in otitis media with effusion and transient reversible hearing loss, which occurs in the higher concentration of endotoxin than the concentration found in human middle ear effusion. The purpose of this study was to investigate the histopathologic changes of the cochlea in guinea pigs with experimental otitis media with effusion induced by E.coli endotoxin. MATERIALS AND METHODS: Concentrations of lipopolysaccharides (LPS) from E.coli, such as 0.1 mg/ml, 1 mg/ml, and 10 mg/ml, were instillated into the middle ear cavity of 30 normal guinea pigs. Histopathologic changes of the cochlea were observed at 1 week after endotoxin instillation, using both light and electron microscope. Six normal guinea pigs were used as a control group whose middle ears were instillated with the physiologically sterile saline. RESULTS: There were deformation in the organ of Corti, partial loss of stereocilia in the hair cells, and vacuolation in stria vascularis at a concentration of 10 mg/ml lipopolysaccharides from E. coli. Mild morphologic abnormality of the cochlea was observed at the concentrations of 1 mg/ml and 0.1 mg/ml. CONCLUSION: A higher concentration of LPS in the middle ear cavity may produce severe inner ear damage, possibly resulting in the sensorineural hearing loss.

9.The eosinophilic otitis media's research progress.

The eosinophilic otitis media(EOM) is an intractable disease characterized by the presence of a highly viscous yellow effusion with extensive accumulation of eosinophils in the middle ear; granulation tissue can been discovered in the middle ear cavity; most of patients have association with bronchial asthma; resist to conventional treatment for otitis media; EOM patients show gradual deterioration of hearing and sometimes become deaf suddenly; effective treatment involves use of topical and oral steroids. This article summarizes the progress of the EOM's diagnosis and treatment.

BACKGROUND AND OBJECTIVES: Impedance audiometry is a clinically useful method for measuring the physical volume of pneumatized middle ear and mastoid medial using a hermetically sealed probe tip. This study was performed to evaluate the obstructive pathologies of middle ear and mastoid by measuring their volumes in patients of chronic otitis media. MATERIALS AND METHODS: The volume of middle ear and mastoid was measured in 37 ears with dry eardrum perforation by chronic otitis media. For control, we measured the volume in 23 ears with the traumatic tympanic membrane perforation. A comparison was made between the volumes and pneumatization in the mastoid X-ray, the soft tissue density in the computed tomography of temporal bone, and the obstruction site at operative findings. RESULTS: The mean volume of the traumatic tympanic membrane perforation was 6.75+/-0.42 ml. The mean volume of chronic otitis media was 3.36+/-2.19 ml and the volume of the sclerotic group was significantly smaller than that of the pneumatic group in the mastoid X-ray. The pathological group had significantly smaller volume than the non-pathological group at operative findings. CONCLUSIONS: The impedance audiometry enables a rapid and valuable evaluation of the presence or absence of obstructive pathologies in the middle ear and mastoid in chronic otitis media with dry tympanic perforation.
