1.Soil-transmitted helminth infections among malaria patients determined by microscopy and real-time PCR methods at two district hospitals in Sarawak, Malaysia

Malaria is still endemic in Sarawak and Sabah. Numerous studies have indicated
that patients with malaria are commonly co-infected with helminthes particularly in endemic
regions. The aim of this study was to investigate the incidence of soil-transmitted helminth
(STH) infection among malaria patients using microscopy and multiplex real-time PCR at two
district hospitals in Sarawak. A total of 94 patients who were clinically-suspected to have
malaria were confirmed to be infected by both microscopy and multiplex real-time PCR. By
the molecular method, 23.4%, 74.5% and 2.1% of the samples were positive for Plasmodium
falciparum, P. vivax and mixed P. falciparum and P. vivax, respectively. Among the malaria
patients, 48.9% were found to be co-infected with STHs. In comparison, microscopic
examinations showed that 6.4% of the malaria patients were infected with STHs. From the
real-time PCR positive samples, 31.9% had single helminth infections while 17% had mixed
infections. In conclusion, this study showed that almost half of the malaria patients at the
two Sarawak hospitals were co-infected with helminth. Future studies should be specifically
designed to determine if there is any correlation between the two infections in terms of
incidence and intensity.
2.Recurrent Glossal Leiomyoma

Oral leiomyomas are rare benign tumour of smooth muscle.
The first case of oral leiomyoma was reported by Blanc in
1884 and since then more cases has been published following
advancement in immunohistochemical study. This tumour
has an excellent prognosis and recurrences are extremely
rare. We report a case of a recurrent glossal leiomyoma in a patient with HIV infection and the lesion recurred one year after the first excision.
3.Polygonum minus ethanolic extracts attenuate cisplatin-induced oxidative stress in the cerebral cortex of rats via its antioxidant properties

Objective: To explore the protective effect of Polygonum minus ethanolic extract on cisplatin-induced neurotoxicity. Methods: In vitro test, total phenolic content assay and DPPH assay were performed to determine the antioxidant activity of Polygonum minus. For in vivo test, 30 male Sprague-Dawley rats were randomly divided into 5 groups: the control group, cisplatin 10 mg/kg, Polygonum minus 100 mg/kg, Polygonum minus 200 mg/kg and Polygonum minus 400 mg/kg. The control group and the cisplatin group were given distilled water whereas Polygonum minus groups received the respective dose of Polygonum minus extract orally for 14 d. On day 15, a single intraperitoneal administration of normal saline was given to the control group; while 10 mg/kg of cisplatin was given to the cisplatin group and Polygonum minus groups. Body weight, signs of illness, daily activity and mortality were observed at least once daily throughout the experimental period. On day 18, the anterior part of the brain was collected and processed for histological and ultrastructural analyses (right hemisphere). The remaining part (left hemisphere) of the brain was assayed to determine malondialdehyde and catalase levels for oxidative stress analyses. Results: Polygonum minus ethanolic extract possessed high phenolic content (977.6 mg GAE/g) and 95.9% DPPH radical scavenging activities. No mortality was observed in all groups. Rats in the cisplatin group were weak and less active compared to Polygonum minus treated rats. In the cisplatin group, disorganised cellular layers of the cerebral cortex were observed whereas rats treated with low and mid doses of Polygonum minus extract had normal cerebral cortex as in the control group. Mild ultrastructural changes were observed in rats treated with low and mid doses of Polygonum minus extract. Meanwhile, low and mid doses of Polygonum minus extract significantly reduced malondialdehyde level whereas low and mid doses of Polygonum minus extracts groups significantly increased catalase activity compared to the cisplatin group. Conclusions: Polygonum minus ethanolic extract at 100 and 200 mg/kg attenuates cisplatin-induced oxidative stress in the cerebral cortex via its antioxidant activity.
4.Alteration of cell cytoskeleton and functions of cell recovery of normal human osteoblast cells caused by factors associated with real space flight.

Experiments involving short-term space flight have shown an adverse effect on the physiology, morphology and functions of cells investigated. The causes for this effect on cells are: microgravity, temperature fluctuations, mechanical stress, hypergravity, nutrient restriction and others. However, the extent to which these adverse effects can be repaired by short-term space flown cells when recultured in conditions of normal gravity remains unclear. Therefore this study aimed to investigate the effect of short-term spaceflight on cytoskeleton distribution and recovery of cell functions of normal human osteoblast cells. The ultrastructure was evaluated using ESEM. Fluorescent staining was done using Hoechst, Mito Tracker CMXRos and Tubulin Tracker Green for cytoskeleton. Gene expression of cell functions was quantified using qPCR. As a result, recovered cells did not show any apoptotic markers when compared with control. Tubulin volume density (p<0.001) was decreased significantly when compared to control, while mitochondria volume density was insignificantly elevated. Gene expression for IL-6 (p<0.05) and sVCAM-1 (p<0.001) was significantly decreased while alkaline phosphatase (p<0.001), osteocalcin and sICAM (p<0.05) were significantly increased in the recovered cells compared to the control ones. The changes in gene and protein expression of collagen 1A, osteonectin, osteoprotegerin and beta-actin, caused by short-term spaceflight, were statistically not significant. These data indicate that short term space flight causes morphological changes in osteoblast cells which are consistent with hypertrophy, reduced cell differentiation and increased release of monocyte attracting proteins. The long-term effect of these changes on bone density and remodeling requires more detailed studies.
5.Optimal protein extraction methods from diverse sample types for protein profiling by using Two-Dimensional Electrophoresis (2DE)

There is a great diversity of protein samples types and origins, therefore the
optimal procedure for each sample type must be determined empirically. In order to obtain a
reproducible and complete sample presentation which view as many proteins as possible on
the desired 2DE gel, it is critical to perform additional sample preparation steps to improve
the quality of the final results, yet without selectively losing the proteins. To address this, we
developed a general method that is suitable for diverse sample types based on phenolchloroform
extraction method (represented by TRI reagent). This method was found to yield
good results when used to analyze human breast cancer cell line (MCF-7), Vibrio cholerae,
Cryptocaryon irritans cyst and liver abscess fat tissue. These types represent cell line,
bacteria, parasite cyst and pus respectively. For each type of samples, several attempts were
made to methodically compare protein isolation methods using TRI-reagent Kit, EasyBlue Kit,
PRO-PREPTM Protein Extraction Solution and lysis buffer. The most useful protocol allows the
extraction and separation of a wide diversity of protein samples that is reproducible among
repeated experiments. Our results demonstrated that the modified TRI-reagent Kit had the
highest protein yield as well as the greatest number of total proteins spots count for all type
of samples. Distinctive differences in spot patterns were also observed in the 2DE gel of
different extraction methods used for each type of sample.
6.Combined Assessment of TGF-beta-1 and Alpha- Fetoprotein Values Improves Specificity in the Diagnosis of Hepatocellular Carcinoma and Other Chronic Liver Diseases in Malaysia

Transforming growth factor beta-1 (TGF-β-1) is a
multifunctional cytokine involved in the regulation of
growth and differentiation of both normal and transformed
cells. The main aim of this study was to determine whether
TGF-β-1 or alpha fetoprotein (AFP) or the combination of the two is a better indicator for hepatocellularcarcinoma (HCC).
Serum TGF-β-1 and AFP were measured by ELISA in 40
healthy subjects, 23 patients with hepatocellular carcinoma
(HCC), 70 patients with hepatitis B, 26 patients with hepatitis C and 16 patients with liver cirrhosis (LC). Patients with liver diseases showed significantly higher serum TGF-β-1 values (>3 fold) compared to control subjects. As for serum AFP,
significant elevation was only observed for HCC cases.
Serum TGF-β-1 exhibited higher percent sensitivity compared
to serum AFP in all liver diseases. Combination of serum TGF-β-1 and AFP increased specificities in all cases studied. In conclusion, serum TGF-β-1 is a more sensitive marker for HCC when compared to serum AFP and its specificity is increased when combined with serum AFP.
7.Twin reverse arterial perfusion sequence.

Twin reverse arterial perfusion sequence occurs in approximately one percent of monochorionic twins. This condition is always fatal for the recipient twin and carries a high mortality rate for the pump twin. Various treatment options are described, but management is continually evolving with the publication of new data. We report an acardiac acephalic monochorionic twin who was diagnosed at 31 weeks gestation. Serial ultrasonographical examinations of the normal pump twin showed intrauterine growth restriction but with no evidence of heart failure. A healthy pump twin was delivered by caesarean section at 34 weeks.

8.Lower Levels of Serum Adiponectin and the T Allele of rs1501299 of the ADIPOQ Gene Are Protective against Polycystic Ovarian Syndrome in Jordan.

BACKGROUND: Polycystic ovary syndrome (PCOS) is a common reproductive disorder. Obesity, which is linked with lower adiponectin levels, increases a woman's risk of developing PCOS; however, the association between adiponectin and PCOS is controversial. Adiponectin levels could be affected by single nucleotide polymorphisms (SNPs) in the ADIPOQ gene. This study aimed to test the relationship between serum adiponectin and PCOS in Jordan and the association between the rs2241766, rs1501299, and rs266729 SNPs in the ADIPOQ gene and PCOS. METHODS: One hundred and fifty-four women with PCOS and 149 age- and body mass index–matched normally menstruating controls were recruited. Serum adiponectin levels were measured using enzyme-linked immunosorbent assay. Genotyping was performed using polymerase chain reaction–restriction fragment length polymorphism analysis. RESULTS: Serum adiponectin levels were significantly lower (P=0.0064) in PCOS women and rs1501299 (+276 G/T) genotype distributions were significantly different (P=0.01) between them and normally menstruating women. Multivariate analysis revealed that adiponectin levels remained significantly lower in PCOS women (P=0.001; odds ratio [OR], 0.9; 95% confidence interval [CI], 0.84–0.96). The GT genotype of rs1501299 increased the risk of PCOS (P < 0.001; OR, 5.46; 95% CI, 2.42–12.33) and increased the risk of PCOS by three-fold (P < 0.001; OR, 3.00; 95% CI, 1.36–6.60) relative to the TT genotype. The GG genotype increased the risk of PCOS as well (P < 0.001; OR, 3:00; 95% CI, 1.36–6.60). CONCLUSION: PCOS is associated with lower serum adiponectin levels independent of age and body mass index. The T allele of the rs1501299 (+276 G/T) SNP of the ADIPOQ gene protects against PCOS.

10.Public health status of Myanmar refugees in South East Asia: A Malaysian case study

The United Nations High Commissioner for Refugees (UNHCR) reports over 80 million people
are displaced worldwide with approximately 26.3 million categorized as refugees and over
a million residing temporarily in South East Asia. Despite the lack of national legislative
framework in place for refugees and asylum seekers (RAS), Malaysia hosts approximately
178,140 as registered with UNHCR and the majority originate from Myanmar. In this review,
we examine refugees from South East Asia, particularly from Myanmar that have contributed
to the largest influx of refugees to this region with a focus on their health status. The
present study traces barriers to the health care of refugees in the country of asylum and also
the challenges faced by these communities in accessing health services.
