3 cases of osteomyelitis in the below of 1/3 of thin bone and Tarsal bone’s were operated to remove completely the died bone and filling fully the focus of osteitis by free gracilis muscle flap with the vessel connection for feeding through microsurgery. Results have been successful in all 3 patients. The necessary techniques include the seperation of free gracilis muscle flap, eradication of died bone and decoration of focus of defective osteitis and stitch filling muscle in the focus.

4.Radiographic Features of Tuberculous Osteitis in Greater Trochanter and Ischium.

PURPOSE: To evaluate, if possible, the radiographic features of tuberculous osteitis in the greater trochanter and ischium, and to determine the cause of the lesions. MATERIALS AND METHODS: We retrospectively reviewed the plain radiographic findings of 14 ptients with histologically proven tuberculous osteitis involvingthe greater trochanter and ischium. In each case, the following were analyzed : morphology of bone destruction, including cortical erosion; periosteal reaction ; presence or abscence of calcific shadows in adjacent softtissue. On the basis of an analysis of radiographic features and correlation of the anatomy with adjacent structures we attempted to determine causes. RESULTS: Of the 14 cases evaluated, 12 showed varrious degrees of extrinsic erosion on the outer cortical bone of the greater trochanter and ischium ; in two cases, bone destruction was so severe that the radiographic features of advanced perforated osteomyelitis were simulated. Inaddition to findings of bone destruction, in these twelve cases, the presence of sequestrum or calcific shadows was seen in adjacent soft tissue. CONCLUSION: Tuberculous osteitis in the greater trochanter and ischium showed the characteristic findings of chronic extrinsic erosion. On the basis of these findings we can suggest that the selesions result from an extrinsic pathophysiologic cause such as adjacent bursitis.

Bone metastasis is the most common diagnosis considered for lytic lesions in the bone. Parathyroid cancer presenting as overt bone disease with diffuse lytic lesions secondary to untreated hyperparathyroidism (HPT) is a rare entity nowadays. We report the case of a 50-year-old Chinese lady who presented with diffuse bone lesions consistent with Brown tumours, vertebral body collapse and hypercalcaemia mimicking bone metastasis and was later diagnosed with parathyroid cancer.

Pustulotic arthro-osteitis is a rheumatic syndrome of unknovn cause, characterised by an inflammatory osteitis of the sternocostoclavicular region and pustuosis palmaris et plantaris. Although many ases of the disease have been reported in Japan, it, has not been reported in Korea so far. Three cases of pustulosis palmaris et plantaris associated i rith sternocostoclavicular hyperostosis or pustulotic arthro-osteitis are presented.

9.Conservative Management of Condensing Osteitis of the Medial End of the Clavicle.

PURPOSE: To assess the clinical features and the outcomes of conservative treatment of condensing osteitis of the medial end of the clavicle. MATERIALS AND METHODS: We reviewed 24 patients among 44 patients with pain and protrusion on the medial clavicle, who were diagnosed as condensing osteitis and followed over one year between April 2003 and August 2008. Plain radiograph and laboratory examination (24 cases), bone scan (12 cases), computed tomography (3 cases), MRI (1 case) and biopsy (1 case) were performed. Patients were followed with UCLA score, VAS (Visual Analogue Scale) and change of protrusion. RESULTS: In all cases, mild sclerosis and expansion were seen at the medial clavicle and sternoclavicular joint was preserved. Laboratory examination showed nonspecific results but various abnormal findings. Protrusion was not changed in 20 cases (83%) and decreased in 4 cases (17%). VAS for pain was improved from 3.4 to 1.2 and VAS for function was improved from 25.5 to 29.1. UCLA score was improved from 28.8 points to 33.7 points. CONCLUSION: Physical and radiologic examination was important for diagnosis of condensing osteitis and laboratory exmination was useful to differentiate it from other diseases. Conservative treatment with rest and nonsteroidal anti-inflammatory drugs showed satisfactory result.

10.Computed tomography of osteitis condensans ilii

The CT is a more accurate technique for detecting sclerotic bony change of osteritis condensans ilii thanplain radiograph. We analysed a comparison between CT and plain radiography of osteitis condensans ilii, acorrelation between osteitis condensans ilii and women of childbearing age. The result were as follow: 1. Theincidence of osteitis condensans ilii is 5.3% on KUB, 11.7% on CT when the width of iliac sclerosis is more than7.5mm as diagnsotic criteria. 2. We observed a osteitis condensans ilii between 19 years and 51 years of age, mostfrequently in fourth decade. 3. The width of iliac sclerosis is 10-13 mm in 3 cases of osteitis condensans ilii onboth CT &KUB, 7.5-9mm in 4 cases of osteitis condensans ilii on CT only. 4. The incidence of osteitis condensansilii is increased significantly when the width of iliac sclerosis is less than 7.5mm as diagnostic criteria. 5.Relatively high correlation between osteitis condensans ilii and delivary in our study (66.6%)
