3.The Study on the Control of Renin Secretion.

It is still not well documented what the basic roles for the regulation of renin secretion from the kidney take place. Since the early study on the renal ischemia for the production of hypertension was introduced, the renin-angiotensin system has been regarded as the possible pathogenetic mechanism for the renovascular hypertension. The renin-angiotensin system, however, could be activated by various stimuli, such as, the changes of intrarenal perfusion pressure, the load or concentriation of sodium at the sites of macular densa, the changes of the sympathetic nervous activity and the changes of potassium balance. To investigate the renin-angiotensin system and the influence of sympathetic nervous system on the regulation of renin secretion, the renovascular hypertension was induced in the dogs by constriction of unilateral renal artery, and the plasma renin activity was measured. The sodium load at the sites of macula densa was attained by furosemide, and then the activity of sympathetic nervous system was depressed by reserpine. The plasma renin activity was assayed by the method of Helmer and Cohn. By this bio-assay method, the plasma renin activity equivalent to 1 nanogram angiotensin-II can be measurable and the prepared plasma was found to have still vasopressor activity. The results observed in this experimental work are summarized as follows. 1. The blood pressure reached maximum on the 3rd postoperative day, and declined gradually to the level of preoperative day on the day of from the 6th to 12th day following constriction of renal artery. 3. The plasma renin activity was found to be well correlated with the increase of blood pressure, and then declined to its preoperative level with the reduction of blood pressure. It appears, therefore, that the renin-angiotensin system plays an important role in the pathogenesis of renovascular hypertension. 4. The urine flow rate in normal and reserpinized dogs was the same before administration of furosemide, but its rate was significantly increased in both groups after furosemide. The urine flow rate of reserpinized dogs, however, was significantly lower than that of the normal dogs during the first 5 minutes. 5. The urinary sodium excretion in normal and reserpinized dogs was significantly increased in both groups after administration of furosemide and there were no difference between the two groups. 6. The plasma renin activity of renal venous blood was significantly higher than that of femoral arterial blood in both groups before and after administration of furosemide. 7. The plasma renin activity of reserpinized dogs was significantly lower than that of normal dogs before administration of furosemide. After furosemide, however, the plasma renin activity was significantly increased in 30 minutes in both groups. This increase of plasma renin activity was less prominent in resepinized dogs than in normal. This results would suggest that the intrarenal mechanism regulating renin secretion also requires an intact sympathetic nervous system.

4.Drug Eruption Probably Caused by Penicillin in Milk.

Milk allergy can be caused by milk protein components or extrinsic allergens such a.s penicillin. The author have experienced a young female patient with drug eruption like skin lesions. Her skin lesions recurred after reingestion of milk. Intradermal test with rnilk as is and provocation test with oral penicillin are considered to be positive. Therefore her skin eruption seemed to be caused by penicillin in milk.

5.Off-Pump Coronary Artery Bypass Grafting.

Coronary artery revascularization with arterial graft is believed to have superiority to that with vein graft with a long-term survival rate with its superb of patency rate. Furthermore, the bilateral internal thoracic artery grafting is an independent predictor of lower rates of angina recurrence, late myocardial infarction, and late cardiac events including reoperation or reintervention. Off-pump coronary artery bypass grafting (OPCAB), which means surgery under the beating heart without use of cardiopulmonary bypass (CPB), is a new attractive surgical strategy for coronary artery disease, because of its benefits for the patients with porcelain ascending aorta to prevent of cerebral emboli and comorbidities such as renal failure, cerebral event, right ventricle dysfunction, and pulmonary disease. The minimally invasive coronary bypass grafting, which means lesser incision, surgery without CPB, and early recovery, can be the surgical method of choice for single vessel disease or limited double vessel disease, with its advantages of early return to work and cosmetics. The OPCAB has been increasing steadily in Korea since 1996, comprising almost 900 cases among the 2,600 cases of bypass surgery performed in the whole country in 2001. The overall mortality rate of bypass surgery in Korea was near 2.3%, comparable to that in western countries. Conclusively, the surgical revascularization for coronary artery disease has the advantages of reducing the cost and lowering the cardiac event, such as reoperation or reintervention.

8.Observation on Neck Form Changes Related to Aging in Men (Part ll) - by the Neck Surface Changes Using the Tape Method.

As a mean to get a better understanding of the kind of form changes to which is subject the neck of the men in their aging process, replicas of neck surfaces were made on 9 subjects (3 in their twenties, 3 in their forties and 3 in their sixties) using the tape method. Three positions of the neck were taken in account : 1) normal position 2) right turn position 3) bent forward position. Measurements of the developed tape replicas have given us the following results : 1. Shapes of the different developed replicas ; Developed replicas of the elder subject show a more curved shape than the replica of the middle age subject, the younger subject having its neck base line the least curved. 2. Surface Area Changes ; In normal position, the neck replica of the elder group covers a smaller surface (average 163.7cm2) than the corresponding one of the younger group (average 193.5cm3). The bent forward position, increases the back surface of the neck in a percentage ranging from 19% to 26% without any major differences between the 3 age groups. The front neck surface being decreased at the same time we verified that the total surface compared to the normal position, had not increased.
