1.Radiologic study of silicosis in Korean

These radiologic studies were carried out on 265 cases of silicosis which were diagnosed clinically atindustrial Accident Hospital affiliated with Catholic Medical Collage, suring the period of 3 years from 1974 to1976. 265 cases of silicosis consisted of 96 cases (35%) of coal miner, 91 cases (34%) of coal choicer and 82cases (31%) of others. The average age was 42.2 years and average working period was 9.2years. Qualitative andquantitative features in the analysis of roentgen findings were based on UICC/Cincinnati and KLO classification.The qualitative features showed 26% of "p" opacity, 46% of "q" opacity 4% of "r" opacity as rounded profusion and5% of "s" opacity, 11% of "t" opacity, 2% of "u" opacity as irregular profusion of small opacities.Large opacitiesshowed 3% of group A, 2% of group B and 1% of group C. Quantitative features revealed 25% of Type 1, 54% of Type2, 16% of Type 3 and 5% of Type 4. The qualitative and quantitative features showed significant differences as ageand working period increased with age and working period. Major complications were pulmonary emphysema,tuberculosis and pleural abnormalities.

2.A Case of the Foutrh Branchial Cleft Cyst Causing Respiratory Difficulty in Neonatal Period.

It is commonly believed that the branchial anomalies are persistent remnants of the embryologic branchial apparatus. These anomalies are originated from branchial cleft and most commonly involved in second branchial deft. Clinically they present palpable mass, discharge from fistula, recurrent infection due to obstruction of Gstula tract. As respiratory symptoms, they may present hoarseness, preceeded by or associated with upper respiratory infection. However, it has not been reported that they caused airway obstruction in newborn infants. Wc experienced a case of the fourth branchial deft cyst in a 2 day old infant who showed respiratory difficulty due to airway obstruction.

3.A Case of Basaloid Follicular Hamartoma Combined with Basal Cell Epithelioma.

Basaloid follicular hamartoma(BFH) is a rare, benign adnexal tumor with a wide clinical appearance spectrum. A characteristic finding is multifocal islands in the papillary dermis and branching cords of basaloid epithelial cells in continuity at various sites with the basal layer of the epidermis and the hair follicles. We present a case of basaloid follicular hamartoma combined with basal cell epithelioma that developed on the tip of the nose in a 75-year-old man. To our knowledge, this is the first case report of basaloid follicular hamartoma combined with basal cell epithelioma in the Korean literature.

4.Discovery of Endocotyle incana and Spelotrema pseudogonotyla (Digenea: Microphallidae) from Scolopacid Migratory Birds in Korea.

Migratory birds have been suspected as playing a central role in transmission of various trematodes, but few surveys have been undertaken in the Republic of Korea. In the present study, we describe new fauna of microphallid flukes in Korea. Two species of microphallids were found in the intestines of 2 migratory bird species, namely the red necked stints (Calidris ruficollis) and the terek sandpiper (Xenus cinereus), in a coastal area of Gunsan-si, Jeollabuk-do. The microphallids recovered from the latter were 451 microm in length and 217 microm in width, and the eggs were very small, 13 microm by 8 microm. They had the characteristic extracecal vitellaria intruding into the median posttesticular area, belonging to Endocotyle incana. Another microphallids were discovered from both migratory birds, 417 microm in length and 249 microm in width. The cirrus was projecting into the genital atrium in form of male papilla, and bounded by a muscular flap, termed pseudogonotyl. They were identified as Spelotrema pseudogonotyla in consideration of the distinctive male papilla and pseudogonotyl. Besides these flukes, Maritrema obstipum and Gynaecotyla squatarolae also were recovered. This paper is in fact the first report on the presence of Spelotrema and Endocotyle in Korea.

6.Computed tomography (CT) in acute head trauma

The retrospective evaluation of 50 cases of head trauma that were subjected to CT leads to the following conclusion; 1. CT survey, should be the first neuroradiological procedures to performe. 2. There is generally adirect relationship between the severity of clinical presentation and the CT findings of the abnormality; responsible for the clinical status, the number and intensity of tissue abnormalities on CT scans increase proportionately with the severity of the clinical signs and symptoms. 3. An accuracy is 100% in diagnosis of intra and extracerebral collections of blood. 4. The causes of acute head tauma were automobile, 64% and falling down,32%.

7.The IL-1 Gene polymorphisms in Korean Children.

The severe form of chronic periodontitis(CP) has been reported to be strongly associated with the presence of allele 2 of composite IL-1B(+3954) and IL-1A(+4845) genetic polymor- phisms(genotype positive). However, other studies have reported conflicting findings. These might have resulted from differences in ethnic background and disease entities. The aim of this study was to determine the distribution of IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN(VNTR) genetic polymorphisms in children as a future Korean population. The study population consisted of 92 children from the Dept. of Pediatric Den- tistry, Chonnam National University Hospital. Genomic DNA was obtained from buccal swab. The IL-1A(+4845), IL-1B(+3954), and IL-1B(-511) genes were genotyped by amplifying the polymorphic region using multiplex polymerase chain reaction(PCR), followed by restriction enzyme digestion and gel electrophoresis. IL-1 RN(VNTR) polymorphism were then evaluat- ed by PCR amplification and fragment size analysis in agarose gel. The allele 2 frequency was 41.3%, 4.3%, 47.8%, and 9.9% for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN respectively. The frequency of genotype with allele 2 carriage for IL-1A(+4845), IL-1B(+3954), IL-1B(-511), and IL-1 RN was 77.1%, 7.6%, 63.0%, and 15.2% respectively. The allele 2 frequency in IL-1B(+3954) was significantly higher in female than in male population(p<0.05). The negative association was shown between the presence of allele 2 in IL-1B(-511) and in IL-1B(+3954), and the carriage rate of IL-1B(+3954) allele 2 tended to lower in IL-1B(-511) allele 2(P=0.056). Only 7.3% of children carried the composite genotype of IL-1A(+4845) and IL-1B(+3954). These results suggest that the polymorphism of IL-1B(+3954) and the positive composite genotype was relatively rare in Korean population.

8.The Effects of Changes in Acid - Base Balance on Neuromuscular Blockade by Org NC 45 .

The sudden onset of respiratory acidosis or alkalosis due to inadequate ventilation during general inhalation anesthesia may influence the action of neuromuscular blocking agents. In virtro animal studies by Funk et al.(1980) suggested that the neuromuscular blocking action of Org NC 45(NC) was minimally depressed above pH 7.68 and significantly potentiated under acidotic conditions(pH 7.05). They proposed that this was the result of an increase in NC metabolism by alkaline hydrolysis in the alkalotic state and greater molecular stability during acidosis. This study was performed to determine the effects of the neuromuscular blocking action of NC during respiratory acidosis and alkalosis. The patients were divided in to 3 groups: 1, ll & lll and experienced normocarbia, hypocarbia and hypercarbia, respectively. Hypocarbia was induced by hyperventilation and hypercarbia by adjustment of a rebreathing valve in the CO2 absorber in the semiclosed system. Simultaneously, arterial blood samples were collected from radial arteries for arterial blood gas analysis including pH and pCO2. Following the administration of succinylcholine(SCC) and the recovery of a 75% twitch height, ED95 of NC was given to the patient and the results were recorded by an evoked electromyograph (NMT, Datex). The results are follows: 1) The number of patients in groups l, ll and lll were 22, 13 and 8, respectively. The patients in each group were evenly distributed with respect to age, body weight and anesthesia. 2) The end-tidal CO2 tension in group l, ll and lll group was 38.86+/-4.62, 20.23+/-2.42 and 52.00+/-4.86mmHg, and the arterial pCO2(pH) was 37.36+/-5.71(7.461+/-0.054), 23.00+/-1.51(7.649+/-0.032) and 53.29+3.35 mmHg(7.314+/-0.026), resptectively. The end-tidal CO2 tension, arterial CO2 tension and pH in group ll and lll were significantly different from those of group l(p<0.05). 3) The onset time of SCC in group ll and lll was shorter than that in group l (p<0.05), but within 1 min in all groups. The duration of SCC in group lll(19.56+/-6.15min) was longer than that in group l (14.74+/-4.56min) (p<0.05). 4) Although there was no significant difference among the groups with respect to onset time and duration, the recovery index in group ll(10.29+/-2.21min) was significantly different from group l and lll(14.76+/-5.26 and 13.50+/-13.67 min, respectively) (p<0.05). After administration of NC in 5 min intervals, twitch tension was measured and the results were inserted into a regression equation which emphasized the delayed recovery in group lll(r=0.87). In conclusion, the recovery index in alkalosis shortened and the initial twitch tension in acidosis following NC administration was delayed compared to that in normocapnis and alkalosis. Patients with alkalosis may require more frequent doses of NC and continuous monitoring following repeated or continuous infusion in acidosis.

9.Recovery of Oswaldotrema nacinovici from Whimbrels (Aves) in Korea.

Adult specimens of Oswaldotrema nacinovici (Digenea: Philophthalmidae) have been first identified in Korea from 2 migratory birds (whimbrels, Numenius phaeopus) which were found dead at a western seashore area near Gunsan. The worms were recovered in the intestine of these birds. The worms were morphologically characterized by a large ventral sucker in comparison to the oral sucker, an external seminal vesicle extending beyond the posterior margin of the ventral sucker, and conspicuous uterine seminal receptacle. It was noteworthy that metacercariae-like bodies were contained within the inflated regions of 2 ceca. Other intestinal trematode species found in whimbrels included Spelotrema pygmeum, Gynaecotyla squatarolae, Maritrema obstipum, and Himasthla megacotyle. Zoonotic potential of these trematode species should be taken into considerations.

10.Sequential Changes of the Breast after Partial Mastectomy with Irradiation in Breast Cancer: Mamrnographic and Ultrasonographic Findings.

PURPOSE: The purpose of the study is to determine the mammographic and ultrasonographic features of the breasts with partial mastectomy and irradiation. MATERIALS AND METHODS: The authors reviewed the serial studies of 23 patients who had partial mastectomy and irradiation. Mammogram and ultrasonogram were perfomed every 6 months after surgery in all patients. Sixteen of 23 patients took mammogram and ultrasonogram 1 month after surgery additionally. We evaluated skin thickening, edema, new calclfication, and postoperative scar. RESULTS: Skin thickening was observed in all patients at initial study after surgery and were most pronounced 6 months after surgery. In the most of patients, increased breast density suggesting edema was seen at the initial study after surgery. Skin thickening and edema were most pronounced 6 months who had retumed to normal state 18 months after surgery in 3 of 5 patients who had serial studies until 18 months after surgery. Scars were noted in 20 of 23 patients and 9 of 20 patients had scars 6 months after surgery. The postoperative changes including skin thickening, edema, and scar were most pronounced at 6 months after surgery and had retumed to normal at 18 months after surgery. CONCLUSION: We conclude that postoperative imaging should be obtained 6 months after surgery, followed by every 6 month intervals, which can be effective in differentiating postoperative scar from recurrent carcinoma and can avoid invasive studies.
