1.Concept Analysis of Comfort.

The concept of comfort has been regarded as a core concept of nursing and it has been frequently used as one of main goals of nursing care. Promoting comfort has been a very important objective in clinical nursing practice. There have been some ambiguities in defining the concept of 'comfort" among nursing scholars. Therefore it was necessary to analyse the concept of comfort for clearer definition to be able to share among scholars. This study is an attempt to analyse concept of comfort and to do it according to a series of concept development processes described by Walker and Avant. Based on the results of the study, the attributes of scope, precedence and consequence of comfort were identified : 1. Attributes 1) stability 2) directionality 3) equilibrium 4) firmness 2. Scope 1) physical 2) social 3) emotional 4) spiritua~ 3. Precedence and Consequence 1) precedence (1) instability (2) unsatisfied needs 2) consequence (1) physical & psychological relaxation (2) enhancement of self-worth (3) enhancement of self-efficacy (4) satisfied needs.

2.A Study of the Effectiveness of Token Economy on Children's Behaviors for Oral Medication.

Oral medication is one of the most frequent treatment in clinical care, and frequently refused by children. Children's refusal spends unuseful time and require nurses' patience. So this study was attempted to develop token economy program which promote children's medication, and to evaluate its' effect to construct the experimental ground. This study was a quasi experimental study under the nonequivalent control group non-synchronized design. The subjects of this study were 48 children who were aged 3-12 years and admitted pediatric ward because of their respiratory disease. Token economy program was consisted of cartoon record paper with stickers which expresses the better medication is taken the more germs die, and gift was given for back up reinforcer. To evaluate this program, time spent for oral medication was measured six time to both experimental group and control group, and measured the perceived effectiveness of this program by nurses. To determine the effect of the program, the data were analyzed by the SAS 6.12 program with t-test and mean, standard deviation. The results of this study were as follows: 1. In experimental group, time spent for oral medication was significantly shorter than in control group(t=5.24, p=0.0001). Therefore, the hypothesis of this study was strongly accepted. 2. Nurses perceived this program as an effective one(mean=3.125 ~ 3.792). The effectiveness occurred before, during and after medication as well as in parental responses, especially in using verbal dissatisfaction after medication. In conclusion, it was found that the token economy program for admitted children was effective in inducing the children's behavior of oral medication. There was no effectiveness in very highly anxious children. Whereas, it is recommended that this program should not be used for such children until they settle down. If the token economy program was made in various situation with creative thinking, it will be very useful nursing measurement, especially in caring for children. To improve the quality of nursing care, the various programs, which can give joyfulness to stressed patients should be developed.

3.Gender Differences in Behavioral Characteristics and Their Awareness of Obese Children.

The purpose of this study was to investigate gender differences in behavioral characteristics and their awareness of obese school children. The results were as follows : 1. In comparison with eating habits and food preference, the girls had more irregular breakfast and were likely to have vegetables/fruits than the boys. 2. In characteristics related to exercise and activities, boys did more exercise than girls. Particularly, in lunch free time, boys participated in playing various active excercise with friends, but girls participated in more non-active behaviors(such as, chattering, reading, or playing jack-stones). 3. In comparison with their awareness related to obese persons, about a half of boys had positive opinions(e, grong, healthy.), but about only 20% of girls had positive opinions.

4.A Study on Influencing Factors in Health-promoting Lifestyle of Adolescents.

The purpose of this study were to define the degree of performance in health promoting lifestyle and to identify the variable related to performance inn health promoting lifestyle of adolescents. The subjects of this study were 469 adolescents in the 4 high schools. The sample data were collected using a purposive sampling method from July 1 to July 27. The collected data were analysed by using descriptive statistics, Pearson's correlation Coefficients, and stepwise multiple regression with SPSS PC+ program. The results of this research were as follows. First, The means of performance in the health promoting lifestyle of adolescents revealed total 2.478 ; harmonious relationships 3.045, regular diet 2.236, professional health management 1.322, sanitary life 2.910, self regulation 2.558, emotional support 2.696, healthy diet 2.048, rest and sleep 2.651, exercise and activity 2.491, self actualization 2.466, diet control 2.408. The factor with the highest degree of performance was the harmonious relationships, whereas the one with the lowest degrees was the professional health management. Second, the relationship between the degree of performance in health promoting lifestyle and its related variable were as follows. (1) Performance in the health promoting lifestyle was significantly correlated with self esteem, self efficacy, health conception, perceived health status, mother's health promoting lifestyle, mother's health conception. (2) The most important factor that affects performance in the health promoting lifestyle of adolescents was self esteem. The combination of self esteem, health conception, mother's health promoting lifestyle, self efficacy, perceived health status accounted for 45.2% of the variance in health promoting lifestyle of adolescents.

5.Can the C-14 Urea Breath Test Reflect the Extent and Degree of Ongoing Helicobacter pylori Infection?.

PURPOSE: The C-14 urea breath test (C-14 UBT) is the most specific noninvasive method to detect Helicobacter (H) pylori infection. We investigated if the C-14 UBT can reflect the presence and degree of H. pylori detected by gastroduodenoscopic biopsies (GBx). MATERIALS AND METHODS: One hundred fifty patients (M:F=83:67, age 48.6+/-11.2 yrs) underwent C-14 UBT, rapid urease test (CLO test) and GBx on the same day. For the C-14 UBT, a single breath sample was collected at 10 minutes after ingestion of C-14 urea (137 KBq) capsule and counting was done in a liquid scintillation counter for 1 minute, and the results were classified as positive ( 200 dpm), intermediate (50~199 dpm) or negative (<50 dpm). The results of CLO tests were classified as positive or negative according to color change. The results of GBx on giemsa stain were graded 0 (normal) to 4 (diffuse) according to the distribution of H. pylori by the Wyatt method. We compared C-14 UBT results with GBx grade as a gold standard. RESULTS: In the assessment of the presence of H. pylori infection, the C-14 UBT global performance yielded sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy of 92.5%, 88.4%, 97.1%, 88.4% and 91.3%, respectively. However, the CLO test had sensitivity, specificity, PPV, NPV and accuracy of 83.2%, 81.4%, 91.8%, 81.4% and 82.7%, respectively. The quantitative values of the C-14 UBT were 45+/-27 dpm in grade 0, 707+/-584 dpm in grade 1, 1558+/-584 dpm in grade 2, 1851+/-604 dpm in grade 3, and 2719+/-892 dpm in grade 4. A significant correlation (r=0.848, p<0.01) was found between C-14 UBT and the grade of distribution of H. pylori infection on GBx with giemsa stain. CONCLUSION: We conclude that the C-14 UBT is a highly accurate, simple and noninvasive method for the diagnosis of ongoing H. pylori infection and reflects the degree of bacterial distribution.

8.Nutritional and Health Status of Korean Elderly from Low-income, Urban Area and Improving Effect of Meal Service on Nutritional and Health Status - V. The Effect of Meal Service for One Year on Nutritional and Health Status.

This study was performed to assess the effect of one year's of meal service for home-staying urban elderly with low incole on their nutritional status. One hundred and eighty three subjects, who had already completed the first nutritional survey, were assigned to two group : meal served(served) and non-meal served(non-served). A meal containing approximately on half of the RDA for energy, protein, calcium and iron was served as lunch everyday to served group. After on year of meal service, follow-up-nutritional survey was done and changes of parameters were analyzed with paired t-test. Served female showed signficantly increased intake of riboflavin and calcium, while non-served female showed significantly decreased intake of calcium. Serum total protein, serum albumin and serum cholesterol were significantly increased in female regardless of meal service. Served remale was observed with significantly elevated LDL-cholesterol, whereas non-served female showed singnificantly lowered HDL-cholesterol. Significantly decreased serum iron, serum transferrin saturaion and significantly increased TIBC were observed for female regardless of meal service. But the proportion of anemic elderly according to Hb or serum iron was decreased more in served group. Female showed significantly increased serum zinc and copper regardless of meal service, whereas only served male showed significantly increased serum copper.

9.Lived Experience of Considering Tomorrow among North Korean Refugees.

PURPOSE: The present study was done to discover the structure of universal actual experiences 'Considering tomorrow' of health and quality of life among North Korean refugees in terms of the socio-cultural context of South Korea. METHOD: The research question was 'what is the structure of the actual experience of 'Considering tomorrow?', which was examined based on the Parse's human becoming research method. Five North Korean adult refugees were recruited from a National Reconciliation Committee in Seoul/Incheon. The data was gathered from dialogues and collected from February, 2006 to November, 2006. RESULTS: The structures found in this study were: 'hope for future life by taking responsibility and having harmony with South Koreans, by forming an integrated identification; having a chance for positive engagement, by attaining human freedom and hope; feeling respected, by assimilating self to the new world; getting freedom back, by facing a new challenge and preparing self for a new social role; overcoming cultural differences with fortified hardiness for survival, by making a decision for a life course with individual growth. In addition, conceptual integration was that 'Considering tomorrow is transforming the enabling-limiting values'. CONCLUSION: Health professionals need to know North Korean refugees' psychological difficulties, expectations of treatment, help seeking behavior, and expectations from mainstream culture. Additionally, understanding North Korean refugees' needs for reality, health education and a multi-disciplinary team approach are necessary to improve their health.

10.A Study on the Development of a Nutrition Education Program and Measurement of It's Effects.

The purpose of this study was to develop a nutrition education program for preschoolers, and to measure it's effects. This program consisted of text. pictures (cartoons), games and topics of discussion. This study was an experimental study undertaken by one pretest-posttest design group. The subjects were 17 preschoolers who were aged 5 and attending an educare center in Seoul. These were the effects of this program: The hypothesis of this study was that "the preschooler's score will be improved after education" was supported (t=5.177, p=.000). Several correlates were examined. There weren't significant differences between pretest and posttest in the importance of balanced nutrition; in the result of under-nutrition; that the black food-group precipitated dental carries and obesity; or the reaction of foods after meals. It is recommended that the nutrition education program not only contain content about the prevent of obesity, but also about the knowledge of various foods and their effects on the human body.
