2.Cancer Patients' Use of Self-care Behaviors in Managing Side Effects Related to Chemotherpy.

The purpose of the study is to research the side effects of chemotherapy which are experienced by cancer patients, theirself-care behaviors to manage the side effect symptoms, and to for provide the fundamental knowledge basis for nursing intervention and self-care education. The subjects were 15 cancer patients receiving chemotherapy in 2 university hospitals. The side effects and self-care behaviors were categorized into 7 themes. 1) First theme Patients experienced nausea, vomiting, alteration in appetite, constipation, diarrhea, and indigestion. Patients used emetics, ate sweets, fruits, drank alcohol and cold veberages to deal with nausea and vomiting. They took their favorites, and yogurt and medicine, and an easily digestible diet to decrease the side effects on digestive function. 2) Second theme Patients experienced oral-dryness & stomatitis. Patients consumed water, candy a warm food and various beverages. 3) Third theme, To cope with changed sense of taste, patient ate their favorite foods, and to help offset alteration in tactile sense they used massage. Concerning changed sensitivity to decreased temperature, ultraviolet treatment and various means of keeping warm were used. To deal with the changed sense of smell and hearing, they avoided noise and bad odors as much as possible. 4) Fourth theme Patients experienced discoloured skin and alopecia. Patients wore appropriate clothes to hide it. To deal with alopecia, they used hats, head kerchief, and positive thinking. 5) Fifth theme Patients experienced weight loss, URI symptoms, fatigue, pain, insomnia and they took various food health products, medicines, and naps. 6) Sixth theme Patients experienced musculoskeletal changes and decreased amounts and range of activities. They did self-care behaviors such as taking baths, exercising etc. 7) Seventh theme Patients felt varing level of anxiety and for this they had fellowship with support companies and used religion, self-control, and positive thinking. From the above research, it can be concluded that : Patients used self-care behaviors which were not proved in effectiveness and education for the prevention and management of the related side effects of chemotherapy was not effective, either.

3.Coping with Experiences in Multiple Chronic Diseases in the Rural Elderly.

PURPOSE: The purpose of this study was to describe how the rural elderly cope with experiences in multiple chronic diseases. METHOD: Data were collected through participant observation and in-depth interview using ethnography. The participants were 9 women and 2 men who aged over 65, were living in rural community, and had experienced two or more chronic diseases. RESULTS: According to the results of this study, those who had experienced multiple chronic diseases went through the stages of 'recognizing of revealed symptoms', 'discovering of disease', 'overcoming', 'neglecting', 'discovering another disease', 'being frustrated' and 'living with suffering'. CONCLUSION: The results of this study are expected to be utilized as basic materials to develop a nursing intervention program for effective management of chronic diseases.

4.Effects of Postpartum Exercise on Mental Health.

PURPOSE: This study was to examine the effect of postpartum exercise on mental health. Mental health comprises a 9 symptom dimension of Somatization, Obsessive-Compulsive, Interpersonal Sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid Ideation, and Psychoticism. METHOD: A non-equivalent pre-test, post-test control group study was conducted. Fifty-two puerperal women who were admitted to a postpartum ward of a mother-baby clinic in Pusan were recruited; 26 women were assigned to the experimental group and 26 women to the control group. Postpartum exercise was applied to the experimental group from postpartum day 3 for a period of 8 weeks while no exercise program was applied to the control group. Mental health with SCL-90-R was measured before and after exercise. Data was analyzed using mean, x2-test, t-test, and ANOVA by SPSS 10.0. RESULT: The mental health status in the exercise group did not show a statistically significant decrease when compared to that of the control group. CONCLUSION: There is no effect of postpartum exercise on women's mental health during the first 8 weeks of the postpartum period. Further studies are needed for randomized clinical trials with a larger sample size.

6.A Study on Women's Fatigue and Sleeping Disturbance.

PURPOSE: This study was to identify the relationship between fatigue and sleep disturbance in adult women so as to provide basic data for developing a women's health promotion program and nursing intervention. METHOD: A convenience sample of 184 women who visited a Well-baby clinic at P city Maternal child center during October in 2004 was obtained. Data was collected by self-administered questionnaires. RESULTS: The Subjects' mean age was 36.87 years and 70% were employed and had more education than high school. The mean level of fatigue was 56.65, mean sleeping hours were 6.54, and time until falling asleep was 17.58 minutes. Scores of fatigue differed by subject's age, level of education, and menstruation status. In addition, sleeping hours differed by age, occupation status, and income level. There were significant low levels of correlation between fatigue and sleeping hours (r=-.16) and getting up too early (r=-.15). CONCLUSION: This study concludes that fatigue and sleeping patterns have a significant relationship.

7.A Study on Nutritional Status and Clinical Practice of Neurosurgical Tube-fed Patients.

This study was performed to investigate the nutritional status of neurosurgical tube-fed patients. The objective of this study was to improve the nutrition management of tube-fed patients. The current practices of tube feeding and enteral nutrition formula as for 95 patients in the hospital were examined by reviewing patients charts, and interviewing patients, nurses and their family members. The results are summarized as follows ; 57.9% of patients that received the formula showed a decrease in mental status. Among the subjects, 55.7% had nervous system diseases due to old age. Most of the tube-fed patients were hospitalized in the emergency room and while hospitalized, the status of their respiratory organ was abnormal. The method of tube-feeding was by Bolus injection and the type of the tube was a 16Fr size silicone tube. The amount of the injection per day while tube-feeding was on average 1424 kcal for men and 1322 kcal for women and the calories per day ranged from 1000-1500 kcal(50.5%). The injection volume averaged 332.7ml and 45% of patients received more than 300ml per injection. The tube feeding intake rate was 127.9ml/min with 50% of subjects in the range of 50-100ml/min and 10% at 20ml/min. The longer the hospitalization, the older age, coma status, and the higher tube feeding rate, the more decreased were the biochemical parameters.

8.Anatomical variants of paranasal sinus affecting the ostiomeatal unit.

It is well known that anatomic variations affecting the ostiomeatal unit (OMU) become one of the causes of mucosal abnormalities of paranasal sinuses (PNS). Findings of coronal plane CT scans of PNS were analyzed in 95 patients with sinusitis or sinusitis-like complaints. Anatomical variations were seen in 88 cases of 60 patients. Mucosal abnormalities were seen in 27 of 47 cases with concha bullosa, 23 of 37 cases with enlarged ethmoid bulla, 8 of 33 cases with Agger nasi, 20 of 26 cases with nasoseptal deviation, 7 of 8 cases with Haller cell, 3 of 6 cases with duoble middle turbinate, 4 of 6 cases with medially bent uncinate process, 1 cases with laterally bent uncinate process, and 33 of 40 cases with mixed variations. Mucosal abnormalities in cases with antomical variations were detected in 61 cases, including maxillary sinus(51 cases), anterior ethmoid sinus (26 cases), frontal sinus (4 cases), and mixed (32 cases). Mucosal abnormalities without anatomical variations were detected in 13 cases involving maxillary sinuses (7 cases), anterior ethmoid sinuses (5 cases), and frontal sinus(1 cases). In conclusion, as coronal plane CT scan has the capability of dolineating anatomical variations and mucosal abnormalities of PNS, it is considered useful in evaluating the treatment and prognosis of chronic or recurrent sinusitis.

10.Evaluation of the Completeness and Validity of the Registration in the Implementation Study of Seoul Cancer Registry(ISSCR).

The incidence data(1991.7.1~1992.6.30)from the Implementation study of seoul cancer Registry(lSSCR) were evaluated in terms of its completeness and validity. Two indicators for the completeness, Mortality/Incidence ratio(M/I ratio) and Age-specific Incidence Curve, showed fairy good registration throughout the age-sex specific strata, except the strata aged over 75 years old. The strata had very high M/I ratio(over 100%) and decreasing pattern of incidence, which suggested incomplete registration of cancer in this group. The active surveillance by a ISSCR staff improved the registration rate especially among elderlies. From the site specific M/I ratio, we found that liver cancer had oddly high M/I ratio. Since this high M/I ratio of liver cancer appears consistently in other reliable cancer registries, it is more like to be due to the highfatality of it rather than incomplete registration. The validity of the incidence data was assessed by three indicators; Histological verification (HV%), Primary Site Unknown (PSU%), and Age Unknown(Age UNK%). The average HV% were 77% for men and 85% for women, which were slightly lower than those of other reliable cancer registries This low HV% might be due to the considerable size of relative frequency of liver cancer in Korea, regarding the fact that the diagnosis of liver cancer is made mostly by non-biopsical radiologic methods (CT, Ultrasono, Angiography, MRl etc.). The level or PSU% and Age UNK% were in acceptable range, but not low enough, especially in terms of Age UNK%. Although ISSCR data had acceptable quality in general, it is needed to have more hospitals participate in the registry surveillance, to make registry data merged with death certificate data regularly, and educate the registration staffs to be more competent and dedicated.
