PURPOSE: This study aims to examine the levels of the perception and work performance of patient safety based on the healthcare accreditation criteria among long-term care hospital nurses. METHODS: A cross-sectional study was performed using questionnaires. Out of 205 criteria, 39 items relevant to patient safety were selectively adapted for this study. Data were analyzed with descriptive statistics, t-test, ANOVA, and Pearson correlation coefficient. RESULTS: The mean scores of perception and work performance were 4.36 and 4.24 out of 5, respectively, and the difference between them was significantly different (t=5.78, P<.001). The two variables were both significantly higher among those nurses who were older, married, head nurses, had many nursing experiences, and aware of Healthcare Accreditation than the other nurses. Positive correlations were observed between perception and work performance in all three sub-systems. The relations between these two in the patient care system was the most highly correlated (r=.894, P<.001). The lowest scores of perception and work performances were fire-related criteria (i.e., reporting). CONCLUSION: Overall, subject's perception proves to be higher than their work performance. It is necessary to develop some viable environment and training programs to enhance their work performance up to the level of their perception of patient safety.

PURPOSE: This study was done to examine fatigue in elderly people with chronic pain and identify factors influencing fatigue. METHODS: Participants in the study were 296 elders suffering from pain for more than 6 months. The data were collected by personal interviews using questionnaires and were analyzed using Pearson correlation coefficients, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 15.0 version. RESULTS: The mean score for fatigue was 30.5, and fatigue showed a significant positive correlation with depression (r=.57, p<.001), perceived stress (r=.12, p=.035) and pain disability (r=.52, p<.001), but a negative correlation with social support (r=-.21, p<.001) and ADL (r=-.22, p<.001). Depression was found to be a significant predictor of fatigue. CONCLUSION: The results of this study show that psychological factors like depression have more influence on fatigue in elders than physical activities or daily capability. Therefore, it is important to consider psychological factors which influence fatigue as well as the fatigue itself in order to efficiently control fatigue.

PURPOSE: This study was done to identify factors (pain stress, perceived stress, pain disability, fatigue, depression) strategies used by elders to cope with pain based on their type of pain belief. METHOD: Data were collected from 314 elders in community settings in Seoul from September to December of 2007. Cluster analysis, t-test, and ANOVA were used to analyze data. Result: The types of pain belief were classified as the following groups: Self-blame, Enduring & Mysterious, and Short-term & Understandable. Perceived stress (t=2.36, p=.02), social support (t=2.24, p=.03), extent of pain relief (t=2.39, p=.02), and duration of pain relief (t=2.09, p=.04) were important factors for active and passive coping in the Self-blame group. Pain stress (t=2.39, p=.01) and depression (t=-3.99, p=.00) were significantly related to the active coping in the Enduring & Mysterious group. Perceived stress (t=2.55, p=.01) was an important factor in the passive coping in the Short-term & Understandable group. CONCLUSION: Considering different types of pain belief in elders and factors that are significantly related to different coping strategies, future nursing interventions should be population specific to encourage active coping strategies and to decrease passive coping strategies.

Objective: . To investigate the impact of the amendment of the Korean National Health Insurance (KNHI) reimbursement criteria for anti-tumor necrosis factor-α (TNF-α) agents based on from conventional clinical and laboratory measurements to disease activity score of 28 joints (DAS28) on treatment pattern, clinical response, and persistence rate in patients with rheumatoid arthritis (RA). Methods: . This multicenter retrospective cohort study evaluated 148 RA patients eligible for the initiation of anti- TNF-α agents as the first-line biologics by either the past (n=95) or current (n=53) KNHI reimbursement criteria. Persistence was defined as the duration between the initiation and discontinuation of anti-TNFα agents. Results: . In total, 106 (71.6%), 35 (23.6%), and 7 (4.7%) RA patients started treatment with adalimumab, etanercept, and infliximab, respectively. RA patients who received anti-TNF-α agents under the current reimbursement criteria had a significantly lower mean DAS28-erythrocyte sedimentation rate (ESR) (6.02 vs. 6.95, p＜0.001) and daily prednisolone-equivalent glucocorticoid dose (4.51 vs. 6.17 mg, p＜0.001) than those who received anti-TNF-α agents under the past reimbursement criteria. No significant differences in the 1-year remission rate defined by DAS28-ESR＜2.6 (17.9% vs. 30.2%, p=0.085) and the persistence rate (p=0.703) between the past and current reimbursement criteria was observed. Conclusion . Our data suggest that less active RA patients can receive reimbursement for anti-TNF-α agents under the current criteria, and the amendment of the KNHI reimbursement criteria may improve access to anti-TNF-α agents without affecting the treatment response and persistence rate.
