1.Comparison of Bone Mineral Density and Lipid Profiles in Pre and Postmenopausal Women.

BACKGROUND: Estrogen deficiency accelerates loss of bone mass and changes lipid profile in the postmenopausal women, so that the osteoporosis and astherosclerosis were developed. But it has not enough studies including the premenopausal women. So we have investigated about the differences of body mass index(BMI), lipid profile and bone mineral density (BMD) with pre- and postmenopausal women. METHODS: We have evaluated 201 premenopausal women and 322 postmenopausal women out of total 651 who had visited Health Screening Center in the hospital of Eul-Ji Medical College from November, 1995 to July, 1996. RESULTS: The mean age of total subjects, premenopausal women, postmenopausal women were 51.9, 43.9, 56.8 years, respectively. The mean period after menopause was 8.1 years. Significant difference was seen in BMI, lipid profile and BMD according to age and menopause(P<0.01). BMI was related to lipid profile(P<0.01), but not to BMD(P>0.1). In postmenopausal women BMI, BMD and lipid profile were related to postmenopausal period (P<0.05). In viewing their correlations BMD had strong adverse correlations with factors such as age, menopause, and postmenopausal period. Lipid profile had weak positive correlations with factors such as age, menopause, BMI(P<0.001). CONCLUSIONS: The lipid profile are related to factors such as age, BMI, menopause, and postmenopausal period. The BMD is related to above factors except BMI. Prospective study is needed to evaluate the influence of estrogen on BMD and lipid metabolism. Thus, it helps to the prevention and treatment of the osteoporosis and hyperlipidemia in the postmenopausal women.

3.Results of Limb Salvage Surgery in Primary Malignant Bone Tumors

Limb salvage operation in primary malignant bone tumors is increasing recently, according to the improvement of diagnostic methods, surgical technique and adjuvant chemotherapy. The purpose of this study is to obtain the interim results of survival rate and the functional grade of the limb in primary malignant bone tumors treated by limb salvage operation. Between 1985 to 1993, 63 primary malignant bone tumors treated by limb salvage operations. Tumor prosthesis arthroplasty was performed in 49 patients, resection arthrodesis in 9 patients, and IM nailing with cement molding in 5 patients. Autoclave autograft was combined in 2 patients. Adjuvant chemotherapy and postoperative radiotherapy were performed in selected patients. The average follow-up period was 50 months(ranged 12 months to 116 months). In the cases of osteosarcoma, estimated survival rate was 61.9% based on Kaplan-Meier survival plot. In the parosteal sarcoma, the survival rate was 87.5% and 80% in chondrosarcoma patients. According to functional grading by Enneking, 66% was excellent, 20% was good, 11% was fair and one patients was poor. Complications occurred in 16 patients:wound infection was developed in 6 patients, local recurrence in 4, peroneal nerve palsy in 2 patients and femoral stem loosening in 2 patients. Fracture in resection arthrodesis and pulmonary metastasis were also occurred. Reoperation was performed in 10 patients at average 31 months after initial operation. Revision tumor persthesis arthroplasty was performed in one patient due to local recurrence and in 2 patients due to femoral stem loosening. Three amputations were done due to recurrence of tumor. IM nailing with cement molding was performed in one patient due to deep infection and repeated arthrodesis was done in a fracture patients. Scheduled custom-made tumor prosthesis arthroplasty was performed within a year in 2 patients treated with IM nailing with cement molding. In conclusion, with the careful preoperative assessment, adjuvant chemotherapy and skillful surgical technique, limb salvage operation would provide the primary malignant bone tumor patients for longer survival and better quality of life.

4.The Effect of Ocular Massage in Cataract Extraction.

The authors have studied on the effect of digital pressure on eyeball under the surface anesthesia instead of other reducing agents of intraocular pressure and retrobulbar anesthesia in order to lower the intraocular pressure as a preoperative procedure of cataract extraction in 26 eyeballs of 24 patients and following results were obtained: 1. The authors had intraocular pressure lowered till averaging 10.8 mmHg (average 13.5 ~ 8.0 mmHg) of original pressure by ocular massage. 2. Massage on eyeball was thought to be a satisfactory preoperative procedure to perform cataract extractIon. 3. In operative complications, loss of vitreous and rupture of capsule were noticed 11.5% and 60% respectively. and it was thought to be caused by technical failure in surgeons. In view of this fact we could not find that the loss of vitreous was caused by intraocular pressure below 13.5 mmHg.

Porokeratosis may be due to an abnormal clone of cells, predisposing affected individusls to development of malignant neoplssm over involved area. A 58-year old female is prsented with basal cell epithelioma associated with PPPD, exhibiting the classical histopathologic criteria of the disease. Total excision for basal cell epithelioma and oral administration of etretinate for the treatment of porokeratosis plantaris, palmaris et disseminata and for the prevention of cancer developrnent resulted in good response.

7.Hand and Wrist Tuberculosis

Hand and wrist tuberculosis in adults are relatively rare disease. The purpose of treatment is to minimize the limitation of motion and to relieve the pain and swelling of involved joints. The treatments consist of curettage of bone, synovectomy and arthrodesis, which were combined with chemotherapeutic agents for tuberculosis. In the period from June 1989 to Oct. 1993, we performed operative treatment in twelve cases of hand and wrist tuberculosis(hand in 3, wrist in 9). We performed curettage and synovectomy in eight cases and arthrodesis in four. The length of follow up was over one year in all cases. Patients were evaluated by clinical examination, radiography and the Robins criteria for clinical results. The results were analysed, as follows, l. Eight patients who had tenosynovitis with moderate joint destruction were treated by curettage and synovectomy, and seven patients(87.5%) of them were evaluated "good" by Robins criteria. 2. Arthrodesis was performed in four patients who had severe joint destruction, and demonstrates "good" results in 75% of cases. 3. One patient who was treated by synovectomy, complaint intermittent pain after three years postoperatively, but no active lesion was visible on the bone scanning. 4. One patient who was performed wrist arthrodesis feels discomfort during lift up heavy products due to insufficient grip power. In conclusion, the synovectomy and curettage in early diagnosed and not so much advanced hand wrist tuberculsis is more preferable, but arthrodesis is inevitable in advanced cases with profound degree of bony destruction and nonviable cartilage in operative findings, for painless and powerful joint.

8.Evaluation of Functional Outcomes and Quality of Life Assessments in Patients with Malignant Tumors of the Musculoskeletal System.

PURPOSE: This study was designed to validate the functional evaluation system after surgery of malignant musculoskeletal tumors by ISOLS (International Symposium On Limb Salvage). MATERIALS AND METHODS: The reliability, content and criterion validities of the ISOLS system were investigated using the Nottingham Health Profile, the Medical Outcome Study 36-item Short-Form Health survey and the EuroQol instrument to measure the quality of life of patients with malignant musculoskeletal tumors. Forty nine patients were evaluated Osteosarcoma around knee joint was most commonly. Prosthetic reconstruction was performed in 55.1%. RESULTS: In content validity analysis, no social and psychological domains was found in ISOLS and the domains for functions of the lower extremities had a strong Cronbach's coefficient (0.88). ISOLS exhibited strong correlation in criterion validity. Convergent validity was good but the discriminative validity was poor. ISOLS has reliability with a strong correlation with other golden standard outcome measures. CONCLUSION: Overall reliability and validity of the ISOLS system appeared to be acceptable. However, this system may need further development of its appropriate domains to evaluate the quality of life in patients with malignant musculoskeletal tumors.

10.Contralateral Vesicoureteral Reflux in Children with Abnormal Unilateral Renal Development.

A total of 48 cases of tuberculous lesion in the lymph nodes (43 cases), lung (3 cases) and soft tissue (2 cases), was subjected to fine needle aspiration cytology(FNAC). The age of the patients ranged from 19 to 77 year-old (average 33.6 years in age) and the male to female ratio was 1: 4. Thirty-four cases (70.8%) demonstrated distinct granulomatous reaction with or without caseation necrosis, nine cases (18.8%) showed no granulomas, but large amount of necrotic debris with numerous polymorphonuclear cells and histiocytes, and five cases (10.4%) revealed acellular material only. The overall AFB positivity in smears was 62.5%. In areas associated with granulomatous reaction and necrosis, AFB positivity was 55.8%, while it was 80.0% in cases with acellular necrotic material. There were 2 cases of parasitic infestation which could not be easily differentiated from tuberculosis based on aspiration smears only.
