1.The Effect of Social Support on Compliance with Sick-Role Behaviors in Hypertensive Clients.

This study was done through Quasi-experimental research in order to determine the effect of social support on compliance with sick role behaviors and blood pressure in hypertensive clients. This study was done through Quasi-experimental research in order to determine the effect of social support on compliance with sick role behaviors and blood pressure in hypertensive clients. The subjects consisted of 88 hypertensive clients who were registered at Cardio-Vascular OPD in Chonnam University Hospital. They were devided to 43 people of experimental group and 45 of control group by the random sampling. The results of the study may be summarized as follows. 1. Compliance with sick role behaviors of the hypertensive clients was significantly increased in experimental group who received the social support from the nurse than control group who did not receive it(t=16.307, p<.0001). 2. The difference mean of blood pressure before and after intervention of the social support did not show the significant differences between experimental and control group(p<.05). 3. Four of the six people stopped smoking in experimental group after intervention of the social support, but there was not any of the five in control group. Fisher's Exact test told the significant differences between experimental and control group(x2=4.385, p<.05). Finally, the social support was effective on compliance with sick role behaviors and a stop smoking in the hypertensive clients.

2.A Case of Incontinentia Pigmenti.

Incontinentia pigmenti is an uncommon genodermatosis. It affects female infants predominantly, described first by Bardach in l925, with the diagnosis of systematized nevus and Bloch in 1926, and Sulzberger in 1928 rnore compIetely. Skin lesions are characterized by 3 stages such as vesicobullous, verucous and finally pigmentary lesions and leave brownish pigmentation on the extremities and trunk. Hesides skin lesions some ectodermaI and mesodermal organs are affected and show developmental abnormalities. We experienced a case of incontinentia pigmenti in a 45-day-old female infant and present it with the review of literature. She showed extensive vesicobullopustular eruption with linear and reticular pigmentation on the extremities and trunk. Clinical and histopathologic findings of these lesions are compatible with Bloch-Sulzberger type of incontinentia pigmenti.

3.Malignant Lymphoma of Mucosa-associated Lymphoid Tissue Arising in the Conjunctiva.

We report two cases of malignant lymphoma of mucosa-associated lymphoid tissue arising in the conjunctiva. Case 1, an 18-year-old girl, showed her left lower conjunctival swelling for four months and Case 2, a 29-year-old man, showed similar lesions on both his eyes for one year. Histologically, both cases revealed monomorphous dense lymphoid infiltrates invading the overlying conjunctival epithelium with diffuse patterns. The infiltrates were composed of small lymphocytes admixed with centrocyte-like cells within the conjunctival epithelium and stroma in association with B-cell lineage. No relapse was noted during the period of about 1½ years follow-up in case 1 treated by local excision only and in the follow-up period of 2 months in case 2 treated by local excision and subsequent chemotherapy.

4.The Treatment of Tibial Shaft Fractures by Ender's Nailing

Between July 1982 and July 1985, we treated thirty-one fractures of the tibial shaft by intramedullar ary Ender's nailing at the Department of orthopedic Surgery of Pusan Paik Hospital. Reductions of the fractures were achieved by closed method in 25 cases and open method in 6 cases. The results were as follows. 1. The average age was 34. 5 years and the most common cause of injury was traffic accident(67.7%) 2. Nineteen cases (61%) were closed fracture and 12 cases (39%) open. Seventeen cases (55%) were simple fracture and 14 cases (45%) comminuted. The most common level of the fracture was middle third of the tibial shaft. 3. Twenty cases (64.5%) had associated injuries and average interval from injury to operation was 11. 8 days. 4. The average healing time was 16.7 weeks and the time prolonged in comminuted fractures than in simple fractures. 5. Complications were one case of valgus deformity (11°), one case of delayed union with pseudoane urysm of anterior tibial artery on fracture site and one case of infected nonunion in Grade III open comminuted fracture. 6. Ender nails are relative ease of nailing, allow early weight bearing and decrease complication. Ender nailing is considered to be a good method in treating tibial shaft fracture especially for management of multiple injured patient and when other procedures have failed.

5.Reconstruction of Posterior Cruciate Ligament Using Patella Tendon Graft

It is generally accepted that posterior cruciate ligament is the prime stabilizer and perhaps the most importent ligament in the knee. We agreed that primary repair or reconstruction of ruptured posterior cruciate ligament is essential. Recently Clancy reported good results to correct knee instability due to rupture of the posterior cruciate ligament with a procedure that employes a free graft of one-third of the patella tendon with its tibial and patella attachments. The free patella tendon graft has been shown to revascularize readly, develop resonably high tensile strengh, allow bone to bone healing, have excellent static stability and not to deprive the knee of a significant stabilizer. So, we analized the 10 cases of the posterior cruciate ligament injuries who were admitted and treated reconstruction using free patella tendon graft in Busan Paik Hospital between May, 1984 to Dec, 1986 and obtained the following results. 1. In 8 cases, obtained satisfactory results. In 2 cases, obtained unsatisfactory results due to combined other compartment injuries, but improved posterior stability than preoperative status. 2. Reconstruction of posterior crucitae ligament using free patella tendon graft was excellent procedure for regaining of static stability of the knee.

6.The relationship between health habit and stress amount in life events.

BACKGROUND: Chronic stress is associated with physical illness such as coronary artery disease and hypertension. Daily life habits being able to affect physical health and are associated with mental stress. We studied the relationship between physical illness and stress amount so far. However, the investigation about the stress and the health habits to affect disease directly was not studied enough. Eventually we tested this investigation to know the relationship between stress amount and health habits. METHODS: We measured stress amount and health habits for a month(from May 1 to May 31, 1997) through the people who had already received health screening and were working as researchers. We used 147 results out of 212. Health habits were investigated about 6 articles out of "Breslow's 7 Health habits" and stress amount was measured by evaluation scale made by Lee, Pyoung-sook. RESULTS: There were no differences in amounts of stress of subjects according to age, years in work, hours of sleeping, having breakfast or not, the frequency of exercise and body mass index(BMI). But there were significant differences in amounts of stress of subjects according to drinking amount, smoking or not, and health habit index(P<0.01). CONCLUSIONS: Drinking and smoking appeared to be health habits related to the amounts of stress. Prospective studies are needed ? find a causal relationship between health habits and stress amount.

9.Two Cases of Generalized Granuloma Annulare Controlled by Hydroxychloroquine.

We report the clinical evolution of two male patients with generalized granulama annulare(GGA) who were controlled with hydroxychloroquine. In the first case, a 3-year-old boy was treated with systemic steroid, and in the second case, a 68-year-old man was treated with prednisolone, niacin, and retinoic acid. But, remission and recurrence of the skin lesions were repeated. A good clinieal response was achieved in both cases after administration of hydroxychloroquine without side effects or any signs of recurrence. Hydroxychloroquine may be helpful to treat the GGA that have side effects to the systemic corticosteroid or resistence to other therapeutic modalities.

10.A survey on Patients' Compliance with Follow-up Coronary Angiogram after Coronary Intervention.

Coronary intervention is now a well established method for the treatment of coronary artery disease. Coronary restenosis is one of the major limitations after coronary intervention. So medical teams advise the patients to get the follow-up coronary angiogram in 6 months after coronary intervention to know if the coronary artery stenosis recurs or not. This study was done in order to know how many patients complied with the advice, and to identify the relative factors to the compliance with getting the follow-up coronary angiogram. The subjects were 101 patients (male: 58 female: 22, mean age: 61+/-15), who received coronary interventions from Jan. 1st to Mar. 31st 1997, and their data were collected from them by questionnaires one year after intervention. The questionnaires consisted of family support scale, self efficacy scale and compliance with sick role behavior scale. The result may be summarized as follows. 1. The number of patients who complied with getting the follow-up coronary angiogram were 37 people(36.6%) and did not comply with it were 64 people(63.4%). All scores of family support(t=5.56, p<.0001), self efficacy (t=4.13, p<.0001) and compliance with sick role behavior(t=5.66, p<.0001) were significantly higher in the patients who got the follow-up coronary angiogram than in those who did not get it. But there was not any relative factor in demographic variables (p>.05). 2. The major motivations for getting follow-up coronary angiogram were recurrence of subjective symptom(40.5%), the advice of medical team(32.4%), and fear of recurrence (27.1%). The restenosis rate in patients who got the follow-up coronary angiogram was 37.8%. 3.The restenosis rate was higher in the patients who had subjective symptoms than in those who did not have any subjective symptom. So subjective symptom and restenosis rate showed a high positive correlation(r=39.9, p<.001). However, 27.2% of the patients who did not have any subjective symptom showed coronary restenosis. 4. The reasons why they did not get the follow-up coronary angiogram were economic burden(37.5%), improved symptom(34.4%), busy life schedule(10.9%), fear of invasive procedure(9.4%), negative reaction of family member(3.1%), no helper for patient(3.1%) and worry about medical team's mistake (1.6%). The relative fators on compliance with getting the follow-up coronary angiogram after coronary intervention were family support, self-efficacy and Compliance with sick role behavior. And the most important reason why the patients did not get the follow-up coronary angiogram after coronary intervention was an economic burden.
