1.A Case of Tuberculosis Cutis Orificialis following Pulmonary and Intestinal Tuberculosis.

We observed a case of tuberculosis cutis orificialis, which occurred in 50 year-old man. He had been suffered from single markedly inflamed, erythematous painful ulcer, covered by dirty brown pseudomembranous material, around anus for 3 months. The laboratory findings (CBC, routine urinalysis, stool ova and parasite with occult blood) are within normal limits except elevated ESR (52mm/hr.). Staphyloeoccus aureus and proteus are observed on wound culture, and many AFB are seen on scrapings of wound but not on sputum for 3 times. Unfortunately culture was not done. Tuberculin skin test revealed strong reaction (36mm/hr.). Chest PA and small bowel series showed far advanced pulmonary tuberculosis and intestinal tuberculosis on ileocecal area. But the findings of upper gastro-intestinal series and barium enema are within normal limits. Histopathologic findings of skin lesion showed tuberculous and tuberculoid granuloma in the deep dermis and subcutaneous tissue. Treatment with INH, myambutol and rifampin resulted in some improvment on the chest and considerable improvement on the skin and intestine within 6 months.

4.Clinical research of Shoulder Injuries Developed in Volleybal Players Spking at a Posterior Angle

There is no sport in which the shoulder does not play an integral role. There are many patients with shoulder joint problems in volleyball, baseball, tennis and golf. In volleyball, a quick strong spike is necessary. Therefor, the treatment of shoulder injuries as well as the protection of the shoulder is a great concern of the player. The authors have done and analysis of the playing history and the clinical studies of the volleyball players who have been recorded as patients of the department of orthopedic surgery, Korea National Police hospital, from January 1977 to December 1980. Results of the study are as follows: l. Of the ages between 17 and 25, the average age was 19.9. 2. Out of 228 case studies 96 complained of shoulder pain or 42.1%. 3. The 84, or 87.6% of the players used the posterior angle spike. The anterior angle spikers and middle angle spikers were 6 players or 6.2% each. 4. Of 60 posterior angle spikers attempted to change spiking angle, 46 or 76% were able to alieviable the shoulder pain. 5. In the posterior angle spikers the lever arm is at its shortest extent so the cases of pain were analyzed as due to repeated impingement of supraspinatus tendon.

6.The Effect of Ocular Massage in Cataract Extraction.

The authors have studied on the effect of digital pressure on eyeball under the surface anesthesia instead of other reducing agents of intraocular pressure and retrobulbar anesthesia in order to lower the intraocular pressure as a preoperative procedure of cataract extraction in 26 eyeballs of 24 patients and following results were obtained: 1. The authors had intraocular pressure lowered till averaging 10.8 mmHg (average 13.5 ~ 8.0 mmHg) of original pressure by ocular massage. 2. Massage on eyeball was thought to be a satisfactory preoperative procedure to perform cataract extractIon. 3. In operative complications, loss of vitreous and rupture of capsule were noticed 11.5% and 60% respectively. and it was thought to be caused by technical failure in surgeons. In view of this fact we could not find that the loss of vitreous was caused by intraocular pressure below 13.5 mmHg.

8.Postoperativ Seizure Outcome in Patients with Cerebral arteriovenous Malformations.

This study was intended to investigate the pre-and postoperative profile of seizures in the patients with cerebral arteriovenous malformatins(AVM's), and to evaluate various preoperative factors to predict the postoperative occurrence of seizures. The patients consisted of 46 consecutive cases with supratentorial AVM's operated on from May 1987 to May 1993. Their mean follow-up duration was 40 months. The pre- and postoperative seizure profiles were similar to each other. The overall incidence of patients experiencing seizure were 50% preoperatively and 52% postoperatively, and the incidence of intractable seizure was 15%, which was the same in both the pre-and postoperatively period. The presence of preoperative seizure and large size of AVM were significant indicators of high incidence of postoperative seizures(P<0.05), while other factors such as preoperative intracerebral hemorrhage or embolization showed no significant influence on the occurrence of postoperative seizures. About 70% of the patients with preoperative seizures also had postoperative seizures, while those without preoperative seizures developed new ones only in 35% of the cases postoperatively. The mean size of the AVM's in the group, with postoperative seizures was larger than that of the group without postoperative seizures(4.56 vs. 3.02 cm). Regarding postoperative intractable seizures, 57% of the group with preoperative intractable seizures remained intractable postoperatively and groups with preoperatively controlled and groups without seizures demonstrated postoperative intractablity only in 13% and 4% respectively. In conclusion, surgery of AVM's did not change the overall incidence of seizures, both controlled and intractable, and patients with preoperative and large sized AVM's and significantly higher possibility of postoperative seizures. Patients with preoperative intractable seizures resulted in control of seizures in 43% after removal of AVM, while 57% remained intractable. Extirpation of epileptic focl, after localization with preoperative study, seems to be needed to improve the outcome of the postoperative seizures.

9.A case of leiomyoma on urethrovaginal septum.

Leiomyoma is rare. Most of patients present with urinary tract infection, hematuria, dyspareunia, or complaint of mass. A case of leiomyoma on urethrovaginal septum in a 36 years old female was presented. The presenting symptom was a protruding mass at the anterior vaginal wall The mass was removed surgically and confirmed as a leiomyoma pathologically.
