1.Developmental surface ultrastructure of Macroorchis spinulosus in albino rats.

Developmental surface ultrastructure of Macroorchis spinulosus was studied by scanning electron microscopy. One-day-old juvenile fluke was leaf-shaped and bent ventrally. Body surface was covered densely with peg-like spines and with cobblestone-like cytoplasmic processes. Ciliated sensory papillae were concentrated around oral sucker. Several unciliated sensory papillae occurred equidistantly on oral sucker and acetabulum. The ciliated papillae appeared in two longitudinal lines symmetric bilaterally on dorsal surface. On adult flukes, tegumantal spines became wider in middle of the body surface. The cytoplasmic processes differentiated into more fine velvety form. It is likely that the differentiated fine cytoplasmic processes are an increased absorptive surface to adult M. spinulosus. It is suggested that single pointed tegumental spines on anterior half of the body may be supportive for this fluke to migration.

2.CS-3000 Plus Cell Separator; A-35 Collection Chamber vs PLT-30TM Collection Chamber.

BACKGROUND: Recently, it has been established that plateletpheresis needs more efficiency and shorter processing time. Fenwall laboratories developed a new collection chamber for CS-3000 Plus, PLT-30TM collection chamber, which can reduce the processing time with efficient collection. We evaluated the PLT-30TM collection chamber by comparing it with A-35 collection chamber that has been used as a standard collection chamber of CS-3000 Plus us. METHODS: Thirty platelet collection procedures were performed using the CS 3000 Plus with A-35 collection chamber and PLT-30TM collection chamber. The changes of the hematologic parameters between pre- and post-donation in donors and the total platelets yields and the contaminated WBCs in the plateletpheresis products were evaluated. In processing, the yield predictor calibration was adjusted to 1.00 and 1.13 in A-35 and PLT-30TM respectively. Yield predictors of pheresis were the same as 3.5x1011 in both and end point volumes were calculated from the CS-3000 Plus. Processing volume and processing times were compared between A-35 and PLT-30TM groups. RESULTS: With PLT-30TM collection chamber, 3.38+/-0.72x1011/L platelets were harvested, whereas 3.20+/- 0.73x1011/L were collected with A-35 collection chamber, which was not significantly different. But processing time with the PLT-30TM collection chamber was more reduced than that with the A-35 collection chamber by about 20 minutes (PLT-30TM : 88.6+/-8.4 min, A-35 : 106.7+/-11.7min). Collection efficiency of PLT-30TM chamber was 50.7+/-12.5% and that of A-35 chamber was 44.4 + 8.8%. The leukocyte contamination of the platelet concentrates were not statistically different(PLT-30TM: 0.0-3.6x106, A-35 : 0.1-4.1x106). CONCLUSIONS: PLT-30TM collection chamber has the advantages of shortening the donation time and decreasing the processing volume with better collection efficiency and flexibility of platelet concentrate volume.

3.Postcardiotomy Mechanical Circulatory Support in Congenital Heart Diseases.

BACKGROUND: To review the experience that used both ventricular assist device(VAD) and extracorporeal membrane oxygenation(ECMO) for children with congential heart disease requiring postcardiotomy mechanical circulatory support. MATERIAL AND METHOD: Between March 1993 and May 1995, we applied mechanical assist device using centrifugal pump to the 16 patients who failed to be weaned from cardiopulmonary bypass(n=15) or had been in cardiogenic shock in intensive care unit(n=1). The diagnosis were all congenital heart diseases and the ages of patients ranged from 20 days to 10 years (mean age=2.5+/-3.5 years). RESULT: The methods of mechanical circulatory support were LVAD(n=13), BVAD (n=1), and ECMO(n=2). The mean assist times were 54.0+/-23.7 hours. Post-assist complications were in orders: bleeding, acute renal failure, ventricular failure, respiratory failure, infection, and neurologic complication. It was possible for 9 patients(56.3%) to be weaned from assist device and 5 patients(31.3%) were discharged from hospital. There was no statistical significant between hospital discharged group and undischarged group by age, body weight, cardiopulmonary bypass time, and assist time. CONCLUSIONS: The ventricular assist device is an effective modality in salvaging the patient who failed to be weaned from cardiopulmonary bypass, but multiple factors must be considered for improving the results of mechanical circulatory support ; such as patient selection, optimal time of starting the assist device, and prevention and management of the complications.

4.Intraarticular Arthroscopic Finding in Grade III MCL Rupture

The medial collateral ligament is the most commonly injured ligamentous structure of the knee. The management of the medial collateral ligament injuries were divided into surgical and conservative treatment. More recently, many investigators reported that isolated medial collateral ligament injuries did well under non-operative treatment. So, arthroscopic examination of the knee to rule out other intraarticular pathologic conditions such as cruciate ligament injuries, meniscal tear and osteochondral lesion is essential. From October 1993 to April 1995, we treated 45 cases of Grade III medial collateral ligament injuries and arthroscopic finding for cruciate ligament, meniscus, bony structure were analized in these cases. The result were as follows; 1. Anterior cruciate ligament injury:37.5% in +1 degree, 42.8% in +2 degree, 86.7% in +3 degree 2. Posterior cruciate ligament injury:12.5% in +1 degree, 13.4% in +2 degree, 33.3% in +3 degree 3. Meniscal injury:12.5% in +1 degree, 21.3% in +2 degree, 33.4% in +3 degree 4. Medial capsular ligament injury was more frequent and severe in the meniscofemoral ligament than in the meniscotibial ligament, but that finding was not correlated with degree of medial instability.

6.Treatment of Acute Retinal Necrosis with Intravenous Acyclovir.

The Acute Retinal Necrosis(ARN) is characterized by necrotizing retinitis, uveit is and retinal vasculitis occurring in otherwise healthy patients. The cause of this disorder is thought to be herpes group virus. The ARN typically presents with the incidious onset of diffuse uveitis, followed by acute retinal whitening, vasculitis, leading to severe destruction of the retinal architecture, frequent retinal detachment and consequent loss of visual function. We experienced 4 cases of ARN in otherwise healthy patients. We treated 4 cases of ARN with intravenous acyclovir(1500mg/M2/day) and oral prednisone. Among 4 cases, one case retained relatively good visual acuity(20/70), 3 cases remained light perception and 2 cases developed retinal detachment. No patient developed new retinal lesion in the fellow eye and there was no evident ocular or systemic side effect from this tyerapy. Intravenous acyclovir and oral prednisone for the treatment of early stage of ARN is not dramatic in effectiveness but seemed to be the treatment of choice at present time.

7.One Case of Menetrier's Disease.

Menetrier's disease is a rare disease characterized by the presence of large rugal folds involving part or all of the stomach. Patients with hypertrophic gastropathy often have distressing abdominal symptoms, weight loss and edema due to gastric protein loss. The 48-year-old male patient was admitted to the Hangang Sacred Heart Hospital with cheif complaints of indigestion and epigastric pain. The diagnosis of Menetriers disease is established by radiologic, endoscopic, and pathologic examination. He was treated with soft diet, antacid, H2- receptor antagonist, and IV albumin. We report a case of Menetriers disease with brief review of literatures.

8.A longitudinal study on the soft tissue facial profile change: Study report from 9 to 13 years of age.

This investigation was undertaken to know how soft tissue facial profile could be changed with age. The 3 serial lateral cephalometric roentgenograms of the twenty nine boys and twenty six girls between 9 and 13 years of age were studied and the findings seemed to warrant the following conclusions. 1. The author made the tables of means, standard deviations in each item, sex, age. 2. Soft tissue facial angle, soft tissue facial convexity including the nose tended to increase, but others tended to remain relatively stable. 3. Facial soft tissue thickness increased with age and the growth of facial soft tissue in the middle region (point A, LS, LI) was greater than others in the facial region. 4. In the soft tissue vertical proportions, G1'-Sn/Sn-Me' was 1.1, Sn-St/St-Me' was 0.51:1, Sn-LI/LI-Me' was 0.82:1 and those were stable with age.

9.Prevalence and Progression of Stage 0 Macular Hole in Fellow Eyes of Patients with Idiopathic Full-thickness Macular Hole

Purpose:
To assess the prevalence and progression of a stage 0 macular hole in the fellow eye of patients with an idiopathic full-thickness macular hole.
Methods:
The fellow eyes of 189 patients who underwent idiopathic full-thickness macular hole surgery were examined by biomicroscopy and spectral domain-optical coherence tomography (SD-OCT). A subset of 21 fellow eyes with a stage 0 macular hole was observed. Changes in the macular hole were evaluated by biomicroscopy and SD-OCT for an average of 29 months.
Results:
Among the 21 eyes, 15 showed no change in perifoveal vitreous detachment (71.4%). Two eyes (9.5%) developed complete vitreofoveal separation, and one of the two developed a separation after progression to stage 1A. Among 21 eyes, 5 (23.8%) developed above stage 1A, and one of the five progressed to stage 1B after five years, which was successfully treated with vitrectomy and gas tamponade.
Conclusions
Perifoveal vitreous detachment in the fellow eye on SD-OCT, defined as a stage 0 macular hole, occurred at an earlier phase than stage 1A macular holes and may progress to an advanced stage. Therefore, patients who undergo macular hole surgery and have a stage 0 macular hole or perifoveal vitreous detachment in the fellow eye should be followed closely.
