The Author's are presenting a case of hypoplastic ectopic kidney in pelvic cavity with short ureteral length and ectopic ureteral orifice in vaginal vestibulum. The size of ectopic kidney was unusually small, chest nut size, as dislocated kidney congenitally which made put more effort then usual diagnostic approach to locate it.

4.Direct Repair of Pars Defects in Symptomatic Lumbar Spondylolysis Using Pedicle Screws and Universal Hooks : Preliminary Report.

OBJECTIVE: The authors introduce a technique of direct repair of pars defects in symptomatic lumbar spondylolysis using pedicle screws and universal hooks. METHODS: From March 2001 to September 2001, six patients with symptomatic lumbar spondylolysis and positive pars injection test underwent this procedure. Patients with lumbar instability and discogenic pain were excluded. After posterior midline incision, clearing of fibrous, cartilaginous and sclerotic tissues in and around the defects of pars was done until healthy bone was exposed. Strips of cancellous bone were taken from adjacent laminae and packed into the defects of pars. After insertion of the pedicle screws, universal hooks were placed in the lower margin of the lamina of the involved vertebra. With gentle compression between the head of screw and universal hook, the nut was fixed. Same procedures were done on the contralateral side. RESULTS: Postoperatively, all six patients experienced clinical improvement and there was no complication related with operation. CONCLUSION: This new technique is considered useful for direct repair of the defects in symptomatic lumbar spondylolysis without significant injury in the normal structures.

7.Technique of Pedicle Screw Fixation and Derotation for the Improvement of Rotational Deformity in Scoliosis Surgery:Derotation:Screw Rotation.

STUDY DESIGN: This is a prospective design. SUMMARY AND BACKGROUND DATA: Derotation makes powerful coronal and satisfactory sagittal correction, however, making rotational correction is still controversy. OBJECTIVES: To introduce a new technique to improve the vertebral rotation. METHODS: 1. Fix the pedicle screws of concave side of thoracic spine with nut driver or derotator before derotation maneuver. 2. During the derotation (counter-clockwise), rotate the nut driver/derotator to the opposite direction (clockwise). RESULTS: A King type II AIS girl with the magnitude of right thoracic and left lumbar curve was 54 degrees and 40degress respectively was reviewed. Thoracic apical rotation was checked 32 degrees. When derotation maneuver was done, the Cobbs angle of thoracic verteba was corrected to 14 degrees (74.1%), however the apical angle was aggravated to 34 degrees . When derotation-screw rotation(DSR) was done, thoracic angle was corrected to 3 degrees with 94.4% of curve correction. The apical rotation was improved to 25 degrees. Preoperative 19.6 degrees of RAsac was corrected to 10.4 degrees showing 46.9% of correction. Postooperatively she was balanced inspite of overcorrection. CONCLUSION: Derotation itself did not improve or slightly aggravate the rotation of apical vertebra. Apical rotation was dramatically improved by derotation combined by screw rotation technique. Maximal curve correction was obtained without trunk decompensation. This new technique may replace the advantages of anterior instrumentation in better rotational correction and saving fusion levels.

8.PROGNOSIS FOLLOWING THE LAG-SCREW FIXATION FOR THE FRACTURED MANDIBULAR CONDYLE.

PURPOSE: This is to evaluate the clinical results and radiologic changes of the mandibular condyle fractures following the open reduction and fixation using the Lag-screws introduced by Eckelt and Martin Co. MATERIALS & METHODS: Ten patients who had been treated by the Lag-screw fixation for the unilateral fracture of the mandibular condyle at the high level and followed up for over 6 months(ranged from 24 weeks to 33 weeks). The incisal opening by time elapsed, displacement of the fragments, bone resorption around the Lag-screws, operating time consumed, and untoward complications were evaluated. The data were tested by repeated measure ANOVA and paired t-test. RESULTS: The maximum mouth opening was increased by time as follows ; 20.2+/-2.8mm soon after reduction. 26.3+/-3.9 at the 2nd week, 37.7+/-4.2mm at the 4th week, 44.4+/-4.3mm in PO 2 months(P<0.05). The bone resorption at anterior to lag-screw nut was measured to 1.9+/-1.0mm, while the posterior resorption was 2.6+/-1.9mm on average(P<0.05). Reduction and fixation of the fragments by Lag-screw were done within 80 minutes including the skin closure. The clicking sound of the TMJ(40%), weakness of the marginal branch(60%) were complicated but transient for 4-8weeks. There were no signs of bony displacement, but loosening of screws were observed at the time of removal. CONCLUSION: Open reduction and fixation with condylar Lag-screw(Martin co., Germany) thru the ramus can be a good option to reduce the high level(Kruger's Level III & IV) fracture of the mandibular condyle with anterior or medial displacement. However, this procedure requires 2nd surgery to remove the devices and it may complicate improper reduction for delayed fractures and in case of 's' curved mandibular ramus.

10.Advantages and Disadvantages of Prostho-Keratoplasty by Using of Cardona's Bolt and Nut Model.

The idea of replacement for opaque cornea by using of artificial material such as a transparent glass piece is an old thinking since year of 1777(Pellier de Quengsy). But the active researches on the concept of artificial corneas as an optical property were stimulated since the synthesis of plastic polymers and its application to medical use during the past 30 years. In year of 1964, Dr. Hernando Cardona1) designed his first prototype by using pure methyl methacrylate and/or with autologous unguinal material for an interlamellar supporting plate. By the way, Strampelli (1964) also has designed his osteo-odonto-kerato-prosthesis for same purpose, with another attempt of arthrochondro-keratoplasty in few years later. And Hayano (1969) also has designed a through and through type with methylmethacrylate. Since year of 1980, author has experienced clinically on prostho-keratoplasty by using Cardona's bolt and nut model, which has a flexible perforated retention nut plate in 26 consecutive patients.
