A patient presenting with an ear polyp is a common finding
in otorhinolaryngology practice. The common causes
include chronic otitis media and cholesteatoma. We report
an adult female patient with a history of acute leukaemia
presenting with chronic otitis media symptoms and right ear
polyp. She was subsequently diagnosed as relapse of B-cell
acute lymphoblastic leukaemia based on histopathological
examination. The presentation may be similar to an
inflammatory pathology of the middle ear, making it
misleading.

2.Median Survival Time of Endometrial Cancer Patients with Lymphovascular Invasion at the Hospital Universiti Sains Malaysia

Background: Endometrial cancer is the most common gynaecologic malignancy among
females worldwide. The purpose of this study was to determine the median survival time of
endometrial cancer patients at the Hospital Universiti Sains Malaysia (USM).
Methods: A list of 121 endometrial cancer cases registered at Hospital USM between 2000
until 2011 was retrospectively reviewed. The survival time of the endometrial cancer patients
was estimated by Kaplan-Meier survival analysis. Log-rank tests were performed to compare the
survival of the patients based on socio-demographics and clinical presentation.
Results: Only 108 patients, 87.0%, were included who were of Malay ethnicity. Previous
history included menopause in 67.6% of patients and diabetes mellitus in 39.8% of patients;
additionally, 63.4% of patients were nulliparous. Tumour staging was as follows: 24.5% stage
I, 10.8% stage II, 26.5% stage III and 38.2% stage IV. The overall median survival time of the
endometrial cancer patients was 70.20 months (95% confidence interval (CI): 51.79, 88.61). The
significant factors were age, the presence of lymphovascular invasion and treatment received.
Conclusion: The overall survival of endometrial cancer was low. A prospective study
needs to be carried out to discover more effective and accurate tests for the early detection of
endometrial cancer.
3.Metastatic leiomyosarcoma of the thyroid: A rare entity

Cancer metastasis to the thyroid gland from non-thyroid sites is a rare presentation in clinical
practice. The most frequent primary cancers that metastasise to the thyroid are renal cell carcinoma,
followed by colorectal, lung and breast. We report a case of a 64-year-old Malay lady who presented
with anterior neck swelling 4 years after an initial diagnosis of uterine leiomyosarcoma. She had
undergone a hysterectomy procedure four years ago. Fine needle aspiration cytology of the thyroid
mass suggested undifferentiated thyroid carcinoma. After multi-disciplinary discussion, the patient
underwent thyroidectomy and the final histopathological diagnosis was metastatic leiomyosarcoma
of the thyroid. The diagnosis was aided by an immunohistochemistry panel of positive myogenic
markers, negative epithelial markers as well as the previous medical history of uterine leiomyosarcoma.
Metastatic leiomyosarcoma of the thyroid may mimic primary undifferentiated (anaplastic) thyroid
carcinoma (UTC) with a sarcomatoid pattern, medullary thyroid carcinoma (MTC) with spindle
cells morphology and spindle cell tumour with thymus-like differentiation (SETTLE). Hence, a
multidisciplinary approach must be practised by pathologists, surgeons and radiologists to consider
metastatic lesions of the thyroid gland, especially when a previous history of cancer exists or is
suspected.
5.Rapid Subjective Vertigo and Psychological Improvement Post Gans Reposition Manoeuvre in 3 Days Duration

Benign paroxysmal positional vertigo (BPPV) is one of the most common vestibular disorder worldwide. Management of this pathology is not complicated and can be done in a short period of time with a proper diagnosis and
treatment technique. There are several established methods for treatment of this disease includes Epley’s maneuver,
Gans Canal Repositioning maneuver (Gans CRM) and others. This study reported a 38-year-old female presented
with dizziness and vertigo, which is severely altered her daily activity, showed 90% improvement using the psychological effect assessment tool after three days of treatment with Gans CRM.
