1.Pericardial Patch Closure Combined with Coronary Artery Bypass Grafting in a Case of Isolated Extracardiac Unruptured Left Sinus of Valsalva Aneurysm with Stenosis of the Main Trunk of the Left Coronary Artery

A 76-year-old woman was admitted to our hospital because of sudden onset of chest pain and dyspnea. Echocardiography, chest CT, and cardiac catheterization revealed an isolated extracardiac unruptured left sinus of Valsalva aneurysm, with stenosis of the main trunk of the left coronary artery. Neither aortic regurgitation nor aortic annular dilatation was recognized. As an infection associated sinus of Valsalva aneurysm could not be ruled out, we performed patch closure using autologous pericardium and coronary artery bypass grafting to the left coronary system. The postoperative course was uneventful. One year after the operation, CT revealed that aneurysm of the left sinus of Valsalva had disappeared and that the grafts were patent.
3.Inflammatory Exudates Modulate the Function and Apoptosis of Neutrophils

Biochemical factors of neutrophils in exudates from acute surgical wounds have been investigated. However, the fate and functions of neutrophils in the exudates have not been clarified and no prior study had compared exudates in the early and late surgical wound stages. The purpose of this study was to investigate the differing effects of exudates on human neutrophils between the early and late phases of inflammation, and to clarify the factors affecting neutrophil function. Exudates from 30 surgical patients with oral cancer who underwent radical neck dissection were collected on the 1st and 5th day after surgery (EX1, EX5). Neutrophils were obtained from two healthy volunteers. Following incubation of the neutrophils with the exudates we examined superoxide anion production and hAPO-1/Fas expression by the neutrophils, apoptosis of the neutrophils and cytokine concentration in the exudates, and the factors related to neutrophil apoptosis. Superoxide anion production by neutrophils incubated with EX1 was significantly higher than by those incubated with EX5, while hAPO-1/Fas expression by the neutrophils was also significantly greater upon incubation with EX5. The DNA ladder was detected only in human neutrophils incubated with EX5. The level of IL-10 in EX1 was significantly lower than that in EX5. Apoptosis of the neutrophils incubated with EX5 was suppressed by the addition of anti-IL-10 antibody. These results indicate that neutrophils have their functions augmented and apoptosis is inhibited in the early phase of inflammation. Furthermore, IL-10 is involved in apoptosis of neutrophils in the late phase of inflammation.
5.Above-Knee Femoropopliteal Arterial Bypass for Lower Limb Ischemia Associated with Bilateral Persistent Sciatic Arteries

Persistent sciatic artery (PSA) is a rare congenital vascular anomaly with an incidence ranging from 0.025 to 0.06% of the general population. We report a 65-year-old male patient with bilateral PSA suffering from intermittent right-calf claudication. Peripheral pulsations could be palpated well at the right common femoral artery but not at the right popliteal artery. His ankle brachial index (ABI) was 0.79 on the right and 0.99 on the left. Computed tomographic (CT) angiography demonstrated that the bilateral PSAs were branched from the internal iliac arteries and the right PSA was completely occluded, whereas aneurysmal change was not evident. Both superficial femoral arteries were incompletely developed. Blood flow to the peroneal artery, anterior and posterior tibial arteries were maintained in both lower legs via a network of collateral arteries. Arterial bypass from right common femoral artery to the above-knee popliteal artery using knitted Dacron graft was performed and postoperative ABI of the right lower limb improved to 1.06. Ten months after the intervention, the patient was then suffered from intermittent left-calf claudication caused by complete occlusion of left PSA, therefore, above-knee femoropopliteal arterial bypass using knitted Dacron graft was performed. PSA is often asymptomatic until a complication develops and the careful monitoring to prevent the risk of thromboembolic and aneurysmal events are recommended.
6.High basal metabolic rate/fat-free mass ratio in Japanese elite male athletes can be attributed to body composition

The aim of the present study was to evaluate the relation between basal metabolic rate (BMR) and body composition in 29 Japanese elite male athletes (age 25.6±3.7y, height 175.6±7.0cm, body weight 74.7±12.8kg, fat-free mass (FFM) 64.9±9.3kg). The subjects were classified into three groups according to their FFM. There was a significant difference in BMR/FFM between S-group (less than 0.5SD of average FFM) and L-group (more than 0.5SD). The parcentage of RM to body weight in S-group was significantly higher than in L-group. In conclusion, high basal metabolic rate/fat-free mass ratio in Japanese elite male athletes can be attributed to body composition.
7.THE ACTN3 R577X NONSENSE ALLELE IS UNDER-REPRESENTED IN ELITE-LEVEL JAPANESE ENDURANCE RUNNERS

Background: Although previous reports have shown a lower proportion of the ACTN3 XX genotype (R577X nonsense polymorphism) in sprint/power athletes compared with controls, possibly attributed to the importance of skeletal muscle function associated with alpha-actinin-3 deficiency, the findings on association between endurance athlete status and R577X genotype are equivocal. Purpose: The present study was undertaken to examine association of ACTN3 R577X genotype with elite Japanese endurance athlete status. Subjects and Methods: Subjects comprised 79 elite Japanese endurance runners (E) who participated in competition at national level and 96 Japanese controls (C). We divided endurance runners into two groups, i.e., 42 national level runners (E-N) and 37 international level runners (E-I) who had represented Japan in international competition. R577X genotype (rs1815739) was analyzed by direct sequencing. Frequency differences of polymorphisms between athletes and controls were examined by Chi-square tests. Result: The R allele frequency tended to be higher in E group than in C group (P=0.066). When we divided E into two groups, the R allele frequency in E-I group was significantly higher than that in C group (P=0.046); whereas there were no significant differences between E-N and C groups (p=0.316). Then, the three genetic models were tested. In the additive genetic model (RR>RX>XX), there were significantly differences between E-I and C (P=0.038), but not the dominant (RR vs. RX+XX) and the recessive (RR+RX vs. XX) genetic models. Conclusion: R allele of the R577X genotype in the ACTN3 gene was associated with elite Japanese endurance athlete status.
8.Anigioplasty of Isolated Left Coronary Ostial Stenosis-A Case Report.

A case of isolated left coronary artery ostial stenosis treated successfully by the saphenous vein patch plasty is reported. A 49-year-old woman was referred for surgery because of unstable angina with subendcardial infarction on ECG. Coronary angiogram showed isolated severe stenosis of left coronary artery ostium without stenotic lesion in the periphery and right coronary artery. At surgery, the aorta was incised obliquely downward to the left coronary ostium and this incision was further extended 8mm distally in the main trunk. Atheromatous left coronary ostium was enlarged with the saphenous vein patch. Postoperatively, angina disappeared and aortic root angioram revealed a well dilated ostium. At 1 year follow-up, the patient remains asymptomatic.

Recent comprehensive investigations clarified that immune microenvironment surrounding tumor cells are deeplyinvolved in tumor progression, metastasis, and response to treatment. Furthermore, several immunotherapeutic trialshave achieved successful results, and the immunotherapeutic agents are available in clinical practice. To enhancetheir demonstrated efficacy, combination of immunotherapy and ablation has begun to emerge. Local ablationshave considerable advantages as an alternative therapeutic option, especially its minimal invasiveness. In addition,local ablations have shown immune-regulatory effect in preclinical and clinical studies. Although the correspondingmechanisms are still unclear, the local ablations combined with immunotherapy have been suggested in the treatmentof several solid malignancies. This article aims to review the published data on the immune-regulatory effectsof local ablations including stereotactic body radiotherapy, cryoablation, radiofrequency ablation, and high-intensityfocusedultrasound. We also discuss the value of local ablations combined with immunotherapy. Local ablations havethe potential to improve future patient outcomes; however, the effectiveness and safety of local ablations combinedwith immunotherapy should be further investigated.
10.Comparison of Characteristics of Streptococcus dysgalactiae subsp. equisimilis Isolates Causing Repetitive vs Single Infections

No study has described Streptococcus dysgalactiae subsp. equisimilis (SDSE) isolates that cause repetitive infections (recurrence and reinfection). We compared the microbiological characteristics of SDSE causing repetitive infections with those causing single infections. Three patients with invasive infections were identified based on their medical records, and multiple SDSE isolates were collected at intervals over three weeks, using a laboratory repository. Isolates from 12 patients with single-episode infections served as controls. Six isolates were collected from three patients with first and second episodes of infection. All isolates causing either repetitive or single-episode infection were subjected to emm typing, multilocus sequence typing (MLST), pulsed-field gel electrophoresis (PFGE), and random amplified polymorphic DNA (RAPD) analyses. Amplification of five virulence genes (sicG, prtF1, prtF2, lmb, and cbp), biofilm formation (BF), and cell invasion abilities (CIAs) were measured as virulent phenotypes. We observed close genetic similarities in the data obtained by emm typing, MLST, PFGE, and RAPD in four isolates from two patients, suggesting recurrence, whereas two isolates from one patient indicated genetic differences in these data, suggesting re-infection. The presence of the five virulence genes and the BF and CIA measurements appeared not to contribute to repetitive infections, compared with isolates causing single-episode infection. In conclusion, clinicians encountering patients with repetitive infections should be aware of both possibilities: recurrence with closely related strains and reinfection with different strains.
