1.Relationship between Demodex sp. Infestation and Acne Problem among Adults in Klang Valley, Malaysia

Demodex sp. is a skin ectoparasite known as follicle mite. The objectives of this study were to determine the prevalence of Demodex sp. infestation among Malaysian adults with age ranges from 20 to 29 years old in Lembah Klang and to determine the relationship between the infestation status and gender and acne problem on face. This is the first study that has been conducted in Malaysia to determine the prevalence of Demodex sp. infestation. A total of 350 people participated in this study. Samples were selected by stratified random sampling. Skin scraping was carried out on
forehead, nose, cheeks and chin. All specimens were mounted in Hoyer’s medium and studied under light microscope. Questionnaires were distributed to the subjects to identify acne problem on their skin conditions. The overall prevalence of Demodex sp. infestation was 18.9% with higher prevalence in males (25.7%) compared to females (12.0%). There
was no significant difference between acne problem and Demodex sp. infestation. In conclusion, infestation of Demodex sp. was detected in a fifth of the subjects, with men had more problems.
2.Multiple gastrointestinal melanoma causing small bowel intussusception

A 61-year-old gentleman presented with small bowel intussusception from small bowel melanoma intussusceptum. He complains of intermittent abdominal distension but no history of intestinal obstruction. Apart from this, he was also symptomatic anemia which required repeated transfusion for the past few months. The contrast-enhanced computed tomography of the abdomen shows an omental mass with small bowel intussusception. He then underwent an exploratory laparotomy with segmental resection of the affected segment. Histopathological examination confirmed primary gastrointestinal melanoma. Multiple small bowel malignant melanoma is a rare disease. It remains a controversial diagnosis as it may be a primary or metastasis from an unidentified or regressed primary cutaneous melanoma. Prompt surgical intervention enables us to obtain tissue diagnosis, prevent complete intestinal obstruction and strategize the goals of treatment for the patient.
3.CLINICAL CHARACTERISTICS AND OUTCOMES OF KIMURA DISEASE IN MALAYSIA: A CASE SERIES AND A LITERATURE REVIEW

Kimura disease is a rare benign chronic inflammatory disorder of the soft tissue with predominant occurrences in young Asian male adults. However, there is limited information of Kimura disease in Southeast Asia. Hence, the clinical characteristics and outcomes of Kimura disease in Malaysia was investigated. Published Kimura disease cases from Malaysia were comprehensively searched in PubMed and Google Scholar up to December 2020 using the keywords “Kimura Disease” and “Malaysia”. Twenty-three papers were identified for review and case series of seven Kimura disease patients from a hospital in Malaysia were descriptively analyzed. A total of 60 cases were obtained from both sources. Eighty-seven percent were men with a male:female ratio of 6.5:1 and majority of the cases were Malays (77.1%). Median age of onset was 22.0 (IQR 12.5-31.5) years, while the median duration before diagnosis of Kimura disease was 2.0 (IQR 0.8-5.0) years. Head and neck region (95%) were the most frequently involved anatomical site. Peripheral eosinophilia was detected in 88.9% of the cases at presentation and renal involvement was observed in four (22.2%) patients. Surgery (57.5%) was the commonest first choice of treatment, followed by a combination of surgery and steroid (29.8%), steroid alone (10.6%), and a combination of steroid and immunosuppressive agent (2.1%). Local recurrences were observed in 28.6% of the cases. In conclusion, a much younger age of disease onset was found among Malaysians. A high recurrence rate of one in every four patients was observed, indicating the need for further evaluations of treatment strategies.
