6.Left Ventricular Radial Strain in Children with Dilated Cardiomyopathy: Analyzed with Two Dimensional Speckle Tracking Imaging Method.

PURPOSE: The purpose of this study is to analyze left ventricular (LV) radial strain (S) and strain rate (SR) in children with dilated cardiomyopathy (DCM) and to evaluate whether the impairment of global radial wall motion correlates with global ventricular dysfunction in DCM. METHODS: In 10 DCM patients (range 0.6-15 years, median age 6.5 years, 3 females) and 17 age and sex matched normal controls (range 0.5-14 years, median age 5 years, 5 females) conventional echocardiography and real time tissue Doppler imaging analysis were performed. Using an automatic two dimensional speckle tracking imaging (STI) method radial S and SR were calculated for each LV segment in 18 segment model. To evaluate the correlation between the impairment of radial motion and LV global function, the average values of radial S and SR of each LV level were obtained, and defined as global (G) S or SR. RESULTS: LV radial S and SR were markedly heterogeneous and decreased in almost all segments of LV in DCM. Impaired radial S and SR were associated with global LV dysfunction. CONCLUSION: In DCM, decreased and disorganized radial S and SR contribute to LV dysfunction. Analysis of radial S and SR with STI method can give further information on LV function in DCM.

7.clinical Evaluation for the Progrosis after the Fontan Operation.

A procedure descried by Fontan and Baudet in 1971 successfully bypassed the right ventricle in Tricuspid Atresia patients, after then many modification of the Fontan operation had been described and applied to many cyanotic complex heart patients. Forty patients with a variety of cardiac malformation underwent the Fontan operation at Seoul National University hospital (September 1978 to June 1986). The age at operation ranged 2 months to 18 years. Each number of cases according to basic cardiac anomaly was as follows; 17 in Tricuspid Atresia, 17 in Univertricular Heart, 2 in Double outlet of Right Ventricle, 2 in Transposition of the Great Arteries and 2 in Criss-cross heart. Total mortality rate after the Fontan operation was 50%. There was only one late death (>30 days). Mortality rate under 4 years of age (67%) was higher than that between 4 and 18 years of age (40%). we observed a significantly higher mortality for patients who, in the immediate postoperative period, had central venous pressure greater than 25cm H2O. 45% among survivals did not require further medication. Although mortality rate after the Fontan operation is much higher than that in the foreign literature, operative mortality will decline with the increased expirence of surgeon and the effective patients selection.

8.Primary Central Nervous System Lymphoma Diagnosed by Biopsy of Skin and Sural Nerve.

Primary central nervous system lymphoma (PCNSL) is a rare tumor that accounts for less than 2% of all cerebral neoplasms. A 72-year-old man had been suffering from recurrent cranial nerve palsies for several years. His condition improved with steroid treatment, but then eventually worsened; he developed encephalopathic symptoms, quadriparesis, axonal neuropathy, and skin lesions. He was eventually diagnosed with PCNSL following an examination of skin and sural nerve biopsy samples. Biopsy of skin lesions or nerves should be considered for the accurate and early diagnosis of PCNSL.

9.The current status and development of a skill examination for the Korean speciality certification examination.

The Specialty Certification Examination is an important part of the assessment of medical education. However, the step 2 skill examinations of 26 medical specialties in Korea are insufficient with respect to achieving the objective of practical examinations that evaluate clinical skill and competence. Among the current step 2 skill examination methods, picture testing using slides or reading of pathology slides/radiologic images is more suitable for testing cognition and knowledge than for testing performance. The oral examination has low reliability because of its relatively short testing period and absence of scoring criteria. In addition, the Specialty Certification Examination is a high-stakes test and the performance during the training course is not reflected in the skill examination. We have reviewed the various skill examinations including clinical practice examinations, objective structured clinical examinations of the United States and Canada, and work-based assessments of the United Kingdom. Based on the review, we suggest some plans for improving the Korean Specialty Certification Examination.

10.Occupational Risk Factors Associated with Musculoskeletal Symptoms in Public Workers.

OBJECTIVES: The purpose of this study was to examine the occupational risk factors associated with musculoskeletal symptoms in public workers. METHODS: In November 2010, 544 participants anwered a survey that included questions on the participants' general characteristics, job-related factors, health-related behaviors, and psychosocial stress. We measured the relationship of risk factors to musculoskeletal symptoms using a logistic regression analysis. RESULTS: After adjusting for general characteristics, factors associated with occupational musculoskeletal symptoms were gender (OR=2.858, 95% CI=1.701~4.801), unnatural posture (OR=2.386, 95% CI=1.308~4.354), psychosocial stress (OR=4.051, 95% CI=1.336~12.282) and sleep quality (OR=1.672, 95% CI=1.052~2.660). CONCLUSIONS: These findings suggest that psychosocial stress levels and physical environment (especially in professions requiring an unnatural posture) are related to musculoskeletal symptoms in public workers.
