The authors report a extemely rare case of primary cerebellar nocardial abscess. Cerebral nocardiosis is arare disease and often fatal, and commonly spreaded from nocardiosis of lung or skin. Total excision of abscess and medication with sulfonamide is the treatment of choice. The patient was treated by continuous external dirainage with intermittent irrigation of abscess cavity by sulfonamide and given with sulfonamide for 10 months.

Nocardiosis is an opportunistic infection occurring in immunocompromised patients. Worldwidely, about four cases of nocardiosis of the breast have been previously reported, but none of these were in Korea. We describe a case of breast nocardiosis associated with pulmonary infection in a patient with Cushing's disease. In our patient, multiple, well-defined, conglomerated, tubular-shaped, isodense mass was revealed at mammography, and an irregular-shaped, ill-defined, heterogeneous echoic mass with movable debris at ultrasonography.

Nocardia is an opportunistic pathogen that can cause disseminated disease in serious immunosuppressive patients with organ transplantation, advanced HIV infection, malignancy or long-term corticosteroid use. Cerebral nocardiosis constitutes the most severe form of Nocardial infection. Early detection and treatment of cerebral abscess of Nocardia is important because the mortality is three times higher than that of other bacterial cerebral abscesses. We report a case of N. farcinica brain abscess in a focal segmental glomerulosclerosis (FSGS) patient after steroid treatment.

Even though nocardiosis is one of opportunistic infections, and can occur in Cushing's syndrome, it rarely occurs in patients with Cushing's disease. Herein, a case with Cushing's disease in whom nocardiosis had manifested as a pulmonary lesion, which after percutaneous needle aspiration, empyema and a breast abscess were also noted.

We report a case of a 42-year-old woman who suffered from recurrent nodular skin lesions on her left foot. Sporotrichoid fungal infection was suspected and two linear nodular skin lesions that had occurred after trauma in a public pool were treated with itraconazole for 4 months. These nodular skin lesions were completely flattened. However, four months after complete flattening, a new lesion developed in the scar of a previous nodule. N. asteroides, which is extremely rare for sporotrichoid cutaneous nocardiosis, was cultured from the relapsed lesion. We treated this case with trimethoprim-sulfamethoxazole for 6 months under an emperical basis and this resulted in complete healing, and sensitivity of N. asteroide to trimethoprim-sulfamethoxazole was confirmed later. There has been no recurrence for 3 years. To our knowledge, our case is the first report in the english literature of primary sporotrichoid cutaneous nocardiosis caused by N. asteroides in terms of anatomic location below the knee and host immunocompetency.

Nocardia is an aerobic gram-positive, weak acid-fast, branching, filamentous bacteria causing various clinical infections such as pulmonary nocardiosis, systemic nocardiosis, extrapulmonary nocardiosis, cutaneous nocardiosis and nocardial mycetoma. They are most commonly caused by N. asteroides complex. Humans become infected by inhaling contaminated air-borne dust particles or by traumatic implantation of the bacterium into the subcutaneous tissues. We isolated Nocardia asteroides complex from the hemo-vac of trauma patient and CSFs of two hydrocephalus patients. Nocardia asteroides complex has been isolated less commonly in Korea than in other countries. We think that it is important to extend an incubation time of culture plate when we find the gram positive, branched, beaded filaments and coccoid cells.
