1.The Influences of Vascular Endothlelial Growth Factor and Endothelin-1 on Speramtogenesis in Testis.

OBJECTIVE: The effects on spermatogenesis by expression of vascular endothelial growth factor (VEGF) and endothelin-1 (ET-1) were investigated. MATERIALS AND METHODS: Testicular specimens were obtained from 40 infertile males due to primary testicular failure and from 10 fertile males with other urologic problems. The specimens of infertile males were devided into 4 groups according to histologic findings; Sertoli cell only syndrome (A), maturation arrest (B), hypospermatogenesis (C) and sloughing and disorganization (D). VEGF and ET-1 expression were detected with immunohistochemical stain. RESULTS: VEGF expression on Leydig cell was detected in all cases. But, VEGF expression rates on germ cell were significantly higher in infertile group B, C, D compared to that of the control group (p<0.05). ET-1 expression rates on Leydig cell was significantly lower in all infertile group compared to that of the control group (p<0.05). But, ET-1 expression rates on Sertoli cell was significantly higher in all infertile group compared to that of the control group (p>0.05). In germ cell of infertile group, LH, FSH and prolactin were significantly decreased, and estradiol is increased in positive stain group on ET-1 immunohistochemical stain (p<0.05). VEGF and ET-1 expression were not correlated mean seminiferous tubule diameter (p>0.05). CONCLUSIONS: Abnormal spermatogenesis would be reflected in VEGF expression in germ cell.

3.Surgical Treatment of Malunited and Nonunited Colles' Fracture: Report of 6 Cases

Even though closed treatment of Colles' fracture leads to satisfactory clinical results, many patients who have had such a fracture are found to have permanent disability and poor function of hand and wrist. Malunion occurs frequently following Colles' fracture and there is considerable disability, pain and deformity associated with this lesion. Such deformity of wrist can usually be prevented by proper treatment, but if deformity developes, better function and a normal appearing wrist can be restored by a corrective osteotomy. The authors experienced 6 cases of malunited Colles' fracture which had been treated by corrective osteotomy, Darrach resection, and osteoclasis, etc.

4.Immunohistochemical Expression of Integrins and bcl - 2 in Melanocytic Nevi and malignant Melanomas.

BACKGROUND: The integrins comprise a family of transmembrane glycoproteins which mediate cell-cell and cell-matr ix intera ctions, and the bcl-2 oncogene is involved in tumorigenesis by blocking apoptosis. Typical acquired melanocytic nevi appear to follow a life cycle characterized by active growth, maturation, and progressive involution. In contrast, congenital rnelanocytic nevi have a different course and prcgnosis with potential for development of melanoma, and metastatic spread is very common in malignant melanoma. Objective . We investigated the expression of integrins (a2,a3,a) and bcl-2 in congenital and acquired rnelanocytic nevi, and malignant, melanomas by immunohistochemical technique. Methods . Formalin-fixed and paraffin-embedded tissues from 7 compound nevi, 7 intradermal nevi, 5 congenital melanocytic nevi, and 5 malignant melanomas were immunolabeled with monoclonal antibodies directed against a2, a3,and a integrin, and bcl-2 protein. RESULTS: All acquired and congenital melanocytic nevi showed a uniform staining for a2, a3, a and bcl-2. In 3 compound nevi which the nevus cells lie mainly within the upper dermis, the nevus cells in the papillary dermis showed weak to negative a expression. Within neurotized are as of intradermal nevus, bcl-2 and a expression became weaker and totally absent. All 5 malig- nant melanomas exhibited strong a2, a3, a, and bcl-2 expression. However, the tumor cells in the lower advancing border failed to express a integrin in 3 of 5 malignant, melanomas. In most malignant melanomas, there was variable intensity of staining for bcl-2 in different areas of the tumor. Conclusion . These findings suggest that the integrins and bcl-2 may be involved in active growth, maturation, and progressive involution of the melanocytic nevi, and seem to be related to metastasis and tumorigenesis of the malignant melanoma.

5.Clinical Significance of the Routine "Dipstick" Urinalyses in Pediatric Inpatients.

To find clinical significance of routine “dipstick” urinalysis on admission, we analyzed the results of the “dipstick” urinalyses performed in 844 patients admitted to the pediatric department of Yeungnam University hospital from May 1, 1983 to October 31, 1984. Ketonuria, proteinuria, hematuria and glucosuria were found in 9.5%, 4.9%, 2.4% and 1.1% of the patients respectively. There were no significant differences by sex, age and presence of fever. However, proteinuria and ketonuria were found more frequently in the patients with dehydration (p<0.05). All of the patients with proteinuria and ketonuria showed negative results on follow up. However, among 13 patients with hematura, 9 patients showed negative results and 4 patients had persistent hematuria on follow up. These 4 patients were found to have chronic pyelonephritis (2), asymptomatic urinary tract infection (1) and benign recurrent hematuria (1). This routine “dipstick” urinalysis on admission seems to be simple test which is useful in detecting unrecognized kidney and other urinary tract disease.

7.Suicidal Death by Nicotine Poisoning.

Nicotine is a water-soluble alkaloid extracted from tobacco plants, and most frequently encountered in snuff, chewing tobacco, cigarettes, cigars and pipe tobacco or in a limited number of pesticides. Nicotine overdose or poisoning might be serious or fatal and lead to death; caused by cardiovascular arrest, respiratory muscle paralysis, and/or central respiratory failure due to its toxic effect. Suicide by nicotine ingestion has been rarely reported due to emetic response. We present a case of suicidal nicotine poisoning. A 56-year-old man was found dead and the postmortem examination revealed no injury or disease. We confirmed a high concentration of nicotine in the toxicological test. Through scene investigation, we determined this case as a suicidal nicotine poisoning.
