1.A Case of New Vessels on the Optic Disc(NVD) Associated with Idiopathic Thrombocytopenic Purpura(ITP).

The most common ocular finding of the idiopathic thrombocytopenic purpura(ITP) is superficial retinal hemorrhages in both eyes. But the descriptions of new vessel on the optic disc(NVD) associated with ITP are scarcely found in literatures. A 17-year-old girl who managed for ITP complained of visual distrubance of the right eye and floaters of the left eye. On fundoscopy the right eye was failed to examine due to thick vitreous hemorrhage and the left eye showed NVD and preretinal hemorrhage. After panretinal photocoagulation of the left eye, NVD was regressed.

4.Glomus Tumor: 4 cases Report

Glomus tumor is well recognized and documented lesion which usually presents as painful dermal nodules in fingers. The tumor results from hypertrophy of a glomus, which is a normal structure of the skin, a coiled arterioyenous shunt regulating body temperature. Glomus tumor may occur at any age but is most frequent in the middle age of life. Pain, tenderness and cold sensitivity are the classic triad of symptoms. Complete surgical exision is the treatment of choice for glomus tumors. Successful removal of tumor leads to complete relief of pain and return to normal function. We report 4 cases of subungual glomus tumor successfully treated by complete exision with review of literatures.

5.The bone formation around anodic oxidized titanium implants in the tinbiae of ovarectomized rats.

Anodic spark deposition method(ASD) surface treated titanium implant possesses a considerable osteoconductive potential that promoting a high level of implant osseointegration in normal bone. The purpose of this study was to observe the ASD implant's osseointegration in the osteoporosis-induced animal model. Twenty four rats, 10 weeks of age, were ovarectomized and 5 weeks later divided into two groups : ASD implant group and control implant group. Titanium screw implants (diameter; 2.0 mm, length, 3.5 mm; pitch-height, 0.4 mm) were designed for this study. Experimental implants were ASD treated and no treatment on control implants. ASD implants and control implants were placed in to left tibiae of rats. The rats were sacrificed at different time interval(1, 2, 4 and 8 weeks after implantation) for histopathologic observation and immunohisto -chemistrical observation, with collagen type I, fibronectin, integrin alpha2beta1 and integrin alpha5beta1 antibodies. The results obtained from this study were as follow: 1. Histopathologic findings, overall tissue response and the pattern of bone formation in both groups were similar. In ASD group, more newly formed bone was seen at 1 week and 2weeks than control group. 2. The levels of type I collagen and fibronectin expression were the most abundant at 2weeks and decreased gradually in both groups. Fibronectin and type I collagen expression in ASD group were stronger than control group but no significance. 3. The levels of integrin alpha2beta1 and Integrin alpha5beta1 expression were most abundant at 2 weeks and decreased gradually in both groups. No significant difference was observed in both groups. From this results, anodic oxidized titanium implants were more advantages in early stage of bone formation than control group, but have no significance in tissue responses and late bone formations. It could be stated that although anodic oxidized titanium implant possesses considerable osteoconductive potential but in osteoporotic bone condition dental implant procedure should performed after improving or treating the osteoporotic bone condition.

6.Binding of Lectins to the Zona Pellucida on Sperm-oocytes Interaction in the Pig.

OBJECTIVE: Lectins are cell-agglutinating and sugar specific proteins or glycoproteins of non-immune origin that precipitate glycoconjugates having saccharides of appropriate complementarity. Because of these properties, plant lectins have been used to help characterize the carbohydrate moieties of glycoproteins in the zona pellucida (ZP) of several mammalian species including pigs. Treatment of oocytes with various lectins blocks sperm binding to the ZP in various mammalian species. This study was undertaken to examine the distribution of sugar residues in the ZP of pig oocytes matured in vitro and the ability of spermatozoa to bind to ZP and in vitro penetration in oocytes treated with fluorescein isothiocyanate (FITC)-labelled lectins. MATERIALS AND METHODS: The lectins of Banderiaea simplicifolia (BS-II, bind to beta-D-Nacetylglucosamine), Canavalin ensiformis (Con A, bind to alpha-D-Mannose), Lens culinaris (LCA, bind to alpha-D-Mannose), Ricinus communis (RCA-I, bind to beta-D-Galactose) and Ulex europaeus (UEA-I, bind to alpha-L-Fucose) were examined for spermatozoa penetration, binding capacity to ZP and distribution of lectins. RESULTS: The penetration rates were significantry (p<0.05) higher in control oocytes (63%) than those treated with all lectins, but penetration rates (40~49%) were simililar in group treated with lectins. The incidence of monospermy was similar in oocytes untreated and UEA-I, but it was higher in oocytes treated with BS-II, Con A, RCA-I and LCA. The porcine oocytes cultured for 48 h in TC-199 medium were freed from cumulus cells and treated for 30 min with fluorescein isothiocyanate-labelled lectins. When examined under fluorescein illumination, higher (p<0.001) proportions of oocytes showed fluorescein of zona pellucida after treatment with Con A (93%), LCA (93%) and RCA-I (100%) than BS-II (37%) and UEA-I (50%). All of the oocytes treated with RCA-I exhibited strong fluorescein in the outer region of the zona pellucida while those treated with LCA exhibited strong fluorescein throughout the zona pellucida. BS-II bounded mainly to the outer region and UEA-I bounded mainly to the inner region of the zona pellucida, with either strong or weak fluorescein. At 120 min after insemination in vitro, fewer spermatozoa were bound to the zona pellucida of the oocytes treated with BS-II, Con-A and RCA-I. Of the lectins, Con A most inhibited sperm binding. CONCLUSIONS: These results suggest that beta-D-Galactose residues in the porcine zona pellucida may act as primary sperm receptors and inducers of the sperm acrosome reaction and these sugar residues may be involved in the block to polyspermy.

10.A Clinical Evaluation of Intraocular Foreign Bodies.

Penetrating ocular injuries with retined intraocular foreign bodies(IOFB) may result in significant visual morbidity. Although appropriate use of microsurgical and vitreoretinal surgical techniques can salvage a high percentage of even the most severely traumatized globes with retained IOFB, questions remain about optimal care for these eyes. The authors analysed the results and prognostic factors in 78 cases of penetrating oculr injuries with retained IOFB who were managed at the Pusan National University Hospital from Jan. 1985 to Dec. 1989. The majority of patients were young adult males in their twenties and thirties. The nature of the IOFB was mostly a magnetic property(88.5%), and the causes of injury were hammering in 67.9% of all cases. The majority of the size of IOFB(maximal length) was over 2mm, and most of the IOFB(75.6%) was located at the posterior segment of the eyeball; and the larger the object, the more it was posteriorly located. Success rate of IOFB removal was 91.0%, and 59.0% of all cases were removed through pars plana. Preoperative complications included cataract(48.7%), vitreous hemorrhage(38.5%), and the most common postoperative complication was retinal detachment(10.3%). In our study, better visual result was obtained in cases of better initial visual acuity, smaller size of IOFB, shorter retention time of IOFB in eyeball, and anterior rather than posterior location of the IOFB in the eyeball.
