4.NOTES-transvagina endoscopic cholecystectomy:report of 31 cases

Objective:To investigate the feasibility,safety and superiority of transvagina endo- scopic cholecystectomy used in clinical application.Methods:31 patients with gall-bladder diseases were treated in transvagina endoscopic cholecystectomy.Through a 5mm cut below the umbilicus, inserted laparoscopic guide gastroscope and NOTES manipulator enter abdominal cavity through posterior fornix 15mm cut.After the gastroscope can capture stable vision,withdraw the laparoscope and insert normal laparoscopic manipulator.With the gastroscope monitoring,accomplish cholecystectomy using umbilical and pudendum manipulator.Results:All of patients were accepted this new type operation.The operation time was 60~150 minutes,and the average time was 85 minutes.No postoperative draining,bleeding and biliary fistula occurred in those patients. There was no obvious scar in belly,and all of patients felt no pains after operation.The average inpatient time was shorten,the medical cost decreased.Conclusion:Transvagina endoscopic cholecystectomy is feasible and safety.Comparing to traditional laparoscopic operation,it has obviously superiority.Transvagina endoscopic cholecystectomy should be widely applied in good condition hospitals.
5.Determination of serum resistin and high-sensitivity C-reactive protein in patients with idiopathic sudden sensorineural hearing loss and their clinical significance

Objective To study the role of serum resistin and high-sensitivity C-reactive protein (hs-CRP) in idiopathic sudden sensorineural hearing loss. Methods The levels resistin and hs-CRP were determined in the 102 idiopathic sudden sensorineural hearing loss patients who were observed at two differ-ent time points: before and after treatment, and 35 patients with other neurologic diseases and 30 healthy people were used as control. Results The levels resistin and hs-CRP in serum of patients with idiopathic sudden sensorineural hearing loss were markedly higher than those in the controls (P< 0.01). The levels of resistin and ha-CRP in the patients with idiopathic sudden sensorineural hearing loss after treatment were significantly less than those before treatment. There was a correlation between serum resistin and hs-CRP in patients with idiopathic sudden senserineural hearing loss (r=0.65, P<0.01). Conclusion The levels of serum resistin and hs-CRP have instructive significance in idiopathic sudden sensorineural hearing loss treating and prognosis estimating.
6.Study on heparin concentration of sealing fluid for diabetic patients with trocar retain

Objective To compare effect on sealing up tubes by using heparin of 3 different con-centrations and make choice of the suitable concentration.Methods The diabetic patients who needed trocar retain were averagely divided into 3 groups,namely group A,B and C with 10 cases in each group.Heparin concentration was 50 U/ml in group A,75 U/ml in group B,100 U/ml in group C respectively.The recanalization rote was compared between the three groups.Results Statistical significance was observed between group A and group B.The obstruction rate of group A was significantly higher than that of group B(P<0.05).But there was no significant difference in obstruction rate between group B and group C.Conclusion Heparin concentration of group B was suitable for sealing tubes for diabetic patients.
7.Manufacture of double-deck pillow and its application during thyroma operation

Objective To approach the preoperative exercise of body position and the comfortable post operative body position of thyroma patients,to elevate the degree of comfort and satisfaction of patients,reduce the complication and promote the rehabilitation of patients.Methods 60 thyroma patients(all underwent operation and with drainage tube)in general surgery department of our hospital from January 2004 to February 2006 were randomly divided into the experimental group and the control group.Before the operation the double deck pillows were used in the experimental group and the ordinary pillows were used in the control group.The therapeutic result was observed in both groups.Results The degree of comfort,the first-class healing rate of incision and the postoperative complications rate were significantly different between the two groups(P<0.05).Condusiom The application of the double-deck pillow in thyroma patients could augment the degree of comfort of the preoperative exercise and postoperative body position,and could degrade the postoperative complications of thyroma patients and shorten the length of hospital stay.

9.Neuroprotective mechanisms of curcumin in cerebral ischemia

Curcumin,a polyphenol compound,is a bioactive constituent extracted from the roots of the perennial herb Curcuma longa L.It has antioxidant,anti-inflammatory,anti-infective,heart protective and anti-tumor properties.Recent research has shown that curcumin can exert neuroprotective role in cerebral ischemia via the mechanisms of antioxidative stress,antiapoptosis,anti-cerebral edema,and anti-inflammation.This article reviews the neuroprotective mechanisms of curcumin in cerebral ischemia.
10.RADIATION ACCIDENT IN 1963 AND 18 YEARS FOLLOW-UP OF THE 4 SURVIVED CASES

In 1963, we treated 6 cases of acute radiation sickness (200-8,000 rad) resulted from an accidental uneven 60Co ?-ray irradiation; two of them died of intestinal type within two weeks. Four cases survived and one of them recovered from septicemic shock. Loss of hair, systemic infection, high fever and bleeding occurred in 5 patients. The essential therapeutic measures were strict isolation, preventive treatment with antiinfection drugs, fresh blood transfusion and sometimes infusion of blood formed elements. Among the survivals 2 received homologous bone marrow transplantation.The general conditions of 4 patients followed up for a period of 18 years are apparently good with transparent lens, normal thyroid function and immunological reactions, except one who had low serum IgG level. Three of them showed subnormal adrenocortical activity and impairment of sex gland function. A girl patient (400 rad) gave birth to 2 children one of which has retarded intellectual development. Serial EEG changes occurred only in those received high cranial doses. In all the cases persistence of chromosome aberration in peripheral lymphocytes was observed. With high doses (above 10,000 rad) remote regional effects led to amputation of one leg in one patient and pathological fracture of the femur in another.
