1.THRAPEUTIC ERCP FOR CHOLANGIO-PANCREATIC DISEASES: REPORT OF 32 CASES

Objective:In order to obtain more effective endoscopic treatment,we analysed 32 cases data of therapeutic ERCP.Methods:Of which 23 were males,9 females with a median age of 56 years.Therapeutic effort was executed in all cases including biliary drainage (n=16),endoscopic sphincterotomy (EST) (n=15,10 of them received extraction)and extract ascaris(n=1).The discussion focus on the treatment of biliary disease.Results:All common bile-duct stone were cleared up.Satisfactory treatment was achieved in almost all with no severe side effect.Conclusions:Therapeutic ERCP is an effective method for cholangiopancreatic diseases.It is of considerable value and should be promoted in clinical application.
2.Determination of serum visfatin and retinol binding protein 4 ( RBP4 ) in patients with idiopathic sudden sensorineural hearing loss and its clinical significance

ObjectiveTo study the role and clinical significance of serum visfatin and retinol binding protein 4 (RBP4) in idiopathic sudden sensorineural hearing loss by measuring the change of their levels in the patients with idiopathic sudden sensorineural hearing loss.MethodsThe levels of visfatin and RBP4 were determined by ELISA method in the 102 idiopathic sudden sensorineural hearing loss patients who were observed at two different time points ( before and after treatment),and thirty-five patients with other neurologic diseases (20 with sciatica,16 with trigeminal neuralgia) and thirty healthy people were used as control.ResultsThe levels of visfatin and RBP4 in the serum of patients with idiopathic sudden sensorineural hearing loss after treatment [Visfatin (24.26 ± 2.17 ) μg/L; RBP4 (46.65± 5.26 ) mg/L]were markedly higher than the group with other neurologic diseases [Visfatin ( 20.67 ± 2.14 ) μ g/L; RBP4(34.37 ±5.73)mg/L] and the healthy control group[Visfatin(17.61 ±2.45) μg/L; RBP4 (24.82 ±5.24)mg/L] ( t =10.38,10.41,12.16,15.06,P <0.01),and it was significantly less than that before treatment [Visfatin(32.24 ± 2.37) μ /L; RBP4 ( 57.43 ± 6.19 ) mg/L] ( t =17.25,15.12,P < 0.01 ).The levels visfatin and RBP4 in serum of severe group with idiopathic sudden sensorineural hearing loss [Visfatin ( 36.52 ± 2.46 ) μg/L; RBP4 (67.17 ± 5.92 ) mg/L] were markedly higher than those in the moderate group[Visfatin(28.92 ±2.26)μg/L; RBP4 (55.34±5.95)mg/L]( t =11.21,11.17,P <0.01).The levels visfatin and RBP4 in serum of moderate group were markedly higher than those in the mild group [Visfatin ( 25.31 ± 2.32 ) μg/L; RBP4 ( 47.48 ± 5.82 ) mg/L],all these differences were statistically significant( t =10.43,10.49,P <0.01 ).There was a positive correlation between visfatin and RBP4 in patients with idiopathic sudden sensorineural hearing loss ( r =0.68,P < 0.01 ).ConclusionsThe levels of serum visfatin and RBP4 have instructive significance in idiopathic sudden sensorineural hearing loss treatingand prognosis estimating
3.Subcutaneous embedment of tissue-engineered urethra in nude mice

BACKGROUND:Under the pathological conditions of urethral defect,stricture and diverticulum induce by trauma,congenital malformation,tumor and operation,autogenous tissue are used for defect repair.OBJECTIVE:To verify the feasibility of urethra constructed by tissue engineering.DESIGN,TIME AND SETTING:An observational experiment was performed at Linbaixin Medical Research Center,Sun Yat-sen University,from January 2005 to January 2006.MATERIALS:Ten New Zealand rabbits weighing 3.0-3.5 kg were used to prepare acellular urethras.Twenty BALB/cA-nude mice weighing 18-20 g,4-6 weeks old,were cultured with extracellular matrix materials in vivo.METHODS:The whole urethra harvested from New Zealand rabbit prepared the extracellular matrix materials by acellular treatment.Half a glans harvested from adult New Zealand rabbit was used for smooth muscle cells proliferation by tissue explant.The corpus spongiosum smooth muscle cells were implanted in acellular matrix materials.Then,they were cultured by subcutaneous embedding in nude mice.There were two groups in the experiment:experimental and control.The complexs of corpus spongiosum smooth muscle cells implanted in acellular matrix materials were embedded in the experimental group,while the simple acellular matrix materials were embedded in the control group.MAIN OUTCOME MEASURES:After 1,2,4,6,8 weeks of embedment,the growth of the complexs were examined by gross,histologic and electron microscopic observation.The expression of smooth muscle cells were identified by immunohistochemistry method.RESULTS:①Under the gross observation,incisions on the back of the nude mice healed well,the diets and activities of the nude mice were normal.In the experimental group,there were thin tissue membranes on the surface of the acellular matrix materials at 1-2 weeks after the embedment.The thickness of tissue membrane increased gradually and there were small blood vessels grown at 4 weeks.The new cells were instead of acellular matrix materials at 6-8 weeks.②Under HE straining,the acellular matrix materials were absorbed.Various cells,most of them were smooth muscle cells,would be instead of the acellular matrix materials.And unequal blood vessels grown in matrix materials,most of them had the trend of blood sinus formation.These results suggested that the complexs formed the structure similar to normal urethra in the nude mice.③Under the electron microscopic observation,there were blood vessels grown in the matrix materials from 4 weeks after embedment.More smooth muscle cells,endothelial cells,and little fibroblast cells,macrophages were observed,as well as some apoptotic smooth muscle cells.④After immunohistochemical staining of ?-smooth-muscle actin antibody,the growth of smooth muscle cells could be observed.CONCLUSION:The corpus cavernosum tissue similar to normal urethra can be constructed by using tissue-engineering technique.
4.Appraisal of left ventricular function in type 2 diabetes mellitus patients with echocardiography

Objective To study the changes in echocardiography in normotensive patients with type2 diabetes mellitus (T2DM) in order to establish early diagnosis and give preventive measures for diabetic cardiomyopathy (DCM). Methods Microvasculopathy was diagnosed in T2DM patients when microalbuminuria exceeded 15ml/min. Fifty-four T2DM patients were divided into two groups: group A (patients without microvasculopathy, n=32 ), group B (patients with microvasculopathy, n=22). All of T2DM were submitted to echocardiography and the results were compared with healthy people of matched sex and age (group C, n=20). Results There were no differences in systolic function parameters among the groups. Left ventricular mass index (LVMI) was higher in two groups of diabetics compared with group C. Diastolic dysfunction of various degrees was found in the two groups of diabetics, regardless of the presence of microvasculopathy, with a manifestation of a decrease of blood flow velocity in early diastolic phase and increase in late diastolic phase (P
5.EFFECT OF COLD-PRESSED CAMELINA SATIVA OIL ON HYPERLIPEMIA OF RATS INDUCED BY HIGH FAT DIET

Objective To investigate the effect of cold pressed Camelina Sativa oil (CPCO) on growth performance and lipid metabolism of hyperlipemia rats. Method The rats were divided into control group (Cont),high fat diet group (HFD),fish oil group (HFD+FO),low dose CPCO group (HFD+LCPCO) and high dose CPCO group (HFD+HCPCO). The latter 4 groups were fed high fat feed. Fo and CPCO were given by gavage. Total cholesterol (TC),triglyceride (TG),high density lipoprotein cholesterol (HDL-C) and HDL-C subtype levels in serum were determined at the end of 6w. The distribution of HDL-C particles was studied by agarose gel electrophoresis. Results HFD resulted typical hyperlipemia. Body weight,TC and TG level were significantly decreased,and HDL-C particles tended to be miniaturized and the HDL3 level increased by feeding Camelina Sativa oil as compared with HFD group. TC content in HFD+FO and HFD+HCPCO group was decreased 46.06% and 30.60% respectively. TG content in these two groups was decreased 25.81% and 22.72% respectively as compared with HFD group. Conclusion Chinese Camelina Sativa oil which contained high level of ?-linolenic acid(ALA)could prevent and cure hyperlipidemia by regulating blood lipid metabolism and protecting hepatic function.
6.The role of endothelial cells on inflammatory cytokine release in septic shock

Objective To study the role of endothelial cells on the inflammatory cytokine release in septic shock through the septic shock serum stimulating human primary endothelial cells (HPAEC) and peripheral blood mononuclear cells(PBMC).Methods PBMC isolated from healthy people by density gradient centrifugation.HPAEC cell surface markers CD144 and von Willebrand factor(vWF) molecule expression by RT-PCR and Western blot.Serum levels of IL-6,TNF-α,MCP-1 from septic shock patients and healthy human detected by ELISA.HPAEC and PBMC were stimulated with the isolated serums and LPS,respectively.ELISA was used to detect the supernatant IL-6,TNF-α,MCP-1 levels.HPAEC membrane molecules ICAM-1 expression was detected by flow cytometry with serum shock and LPS stimulation.Supernatant levels of IL-6,TNF-α,MCP-1 of HPAEC with S1P1 receptor agonist CYM-5442 pretreatment was detected by ELISA after shock serum stimulation.Results Endothelial cell markers CD144 and vWF molecules could be detected in the HPAEC.Levels of inflammatory cytokines IL-6,TNF-α,MCP-1 in patients with septic shock serum were significantly higher than healthy people (P<0.01).PBMC and HPAEC with LPS or shock serum treatment respectively,compared with normal group,levels of inflammatory cytokines in the culture supernatant were significantly higher(P<0.01).For PBMC,the level of inflammatory cytokines between shock group and LPS group were not significantly different (P>0.05).But for HPAEC,levels of inflammatory cytokines in the supernatant of the shock group compared to the LPS group was significantly higher (P<0.01).Similarly,when two cells after LPS stimulation,IL-6,TNF-α levels of HPAEC's supernatant were significantly lower than PBMC' s (P<0.01),MCP-1 levels was no difference (P> 0.05).But when the stimulation of shock serum,HPAEC of IL-6,TNF-α levels and PBMC no significant difference (P >0.05).MCP-1 was significantly increased (P<0.01).Shock patients serum stimulation S1P1 receptorspecific agonist CYM-5442 pretreatment of HPAEC with pretreatment of S1P1 receptor specific agonist CYM-5442,the culture supernatant of inflammatory cytokines IL-6,TNF-α,MCP-1 levels were significantly lower (P<0.01).Conclusion Endothelial cells may play a central role on the release of inflammatory cytokine during septic shock.
7.Expression of Neuritin and its clinical significance in gastric cancer

Objective To research the expression of Neuritin in gastric cancer .and the relationship between the expression of Neuritin with the occurrence and development of gastric cancer .Methods Collected 58 surgical specimens of gastric cancer from 2010 to 2013 in the first affiliated hospital of Shihezi University ,immunohistochemistry was used to examine the expression of Neu-ritin in gastric cancer and normal tissues near the cancer of the stomach .Results Immunohistochemical staining showed that Neu-ritin was moderately or highly expressed in 96 .55% (56/58) of gastric cancer ,and Neuritin was moderately or highly expressed in 94 .83% (55/58) of normal tissues near the cancer .There was no statistically significantly difference between two groups (P>0 .05);Neuritin was highly expressed in 82 .76% (48/58) of gastric cancer ,and Neuritin was highly expressed in 15 .52% (9/58) of normal tissues near the cancer ,differences between the two groups had statistically significantly (P<0 .05) .Expression of Neuritin was not correlated with gender ,age ,TNM stage ,infiltration depth ,lymph node metastasis ,differentiated degree ,the pathologic type or distant metastasis(P> 0 .05) .Conclusion The Neuritin expression level between gastric cancer and normal tissues near the cancer exist differences .There is overexpression of neuritin in gastric cancer .
8.Resonance Characteristics of Pre-and Post -UPPP Operation in Patients with OSAHS

Objective To study the influence of UPPP on resonance characteristics of four resonance cavity in patients with OSAHS .Methods Using multi-channel voice analysis system to test the cavity resonance character-istics and energy distribution of 36 OSAHS patients (including pre - and post -operative) and 36 normal males when they speak /a:/.The frequency spectrum was 4 000Hz(FR1 ,FR2 ,FR3 ,FR4 ) .Results FR2 energy values of the head cavity and mouth cavity of OSAHS patients heighten significantly after UPPP (P<0 .05);FR1 energy val-ues of head ,mouth and thoracic cavity were higher than that of in normal males'after UPPP(P<0 .05) .FR2 ,FR3 , FR4 energy values of head cavity ,FR2 ,FR3 energy values of mouth cavity ,FR2 energy values of thoracic cavity were lower than those of in normal males'after UPPP(P<0 .05) .Conclusion UPPP surgery removes the obstruction of the upper airway of OSAHS patients and changes the morphology of the pharynx ,thas changes the resonance charac-teristics of the head cavity and oral .
9.Establishment of pharmacodynamic model of sevoflurane in pediatric patients: bispectral index

Objective To establish the pharmacodynamic model of sevoflurane with bispectral index (BIS) as the effective index in pediatric patients.Methods Thirteen ASA physical status Ⅰ or Ⅱ pediatric patients,aged 4-9 yr,weighing 12-39 kg,undergoing non-cardiac surgery,were selected in the study.The pediatric patients sequentially inhaled 1%,5 % and 1% sevoflurane via a face mask and each concentration was inhaled for 15 min.BIS value,HR,BP and SpO2 were automatically recorded every 10 s.Based on nonlinear mixed effect modeling,the population pharmacodynamic model of sevoflurane was established using NONMEM software.The effect of age on the pharmacodynamic parameters was evaluated using a stepwise forward addition then backward elimination modeling approach.The standard for model improvement was defined as a decrease in the value of the objective function by more than 3.84.Results Twelve pediatric patients,aged 4.0-8.5 yr,weighing 12.8-38.0 kg,with body height of 92-135 cm,were enrolled in this study and the data which were enrolled comprised 2964 effective concentration-time-BIS points.The model was not improved significantly with any covariates (age,body height,and body weight) introduced (P > 0.05).The estimated parameters of the final pharmacodynamic model of sevoflurane were as follows:ke0 =O.516/min ; EC50 (BIS50) =2.11% ; γ =2.46 ; E0 =74.6 ; EMAx =11.2.Conclusion The pharmacodynamic model of sevoflurane is successfully established with BIS as the effective index in pediatric patients,and the analysis for each parameter of the model indicates that the sensitivity to sevoflurane is lower,but the blood-brain equilibration time of the drug is shorter and the onset and recovery are faster in children than in adults.
10.Computational chemistry in structure-based drug design.

Today, the understanding of the sequence and structure of biologically relevant targets is growing rapidly and researchers from many disciplines, physics and computational science in particular, are making significant contributions to modern biology and drug discovery. However, it remains challenging to rationally design small molecular ligands with desired biological characteristics based on the structural information of the drug targets, which demands more accurate calculation of ligand binding free-energy. With the rapid advances in computer power and extensive efforts in algorithm development, physics-based computational chemistry approaches have played more important roles in structure-based drug design. Here we reviewed the newly developed computational chemistry methods in structure-based drug design as well as the elegant applications, including binding-site druggability assessment, large scale virtual screening of chemical database, and lead compound optimization. Importantly, here we address the current bottlenecks and propose practical solutions.
