1.Effect of long-term inhaled corticosteroids on growth indicatorsin children with bronchial asthma

Objective To explore the influence of long-term inhaled corticosteroids on insulin-like growth factor-1(IGF-1),bone mineral density, height and weight in children with bronchial asthma.Methods Bronchial asthma were selected as treatment group from January 2013 to January 2014 in pediatrics department of Nanyang Central Hospital, and 25 healthy children undergoing physical examination at the same time were selected as control group.All children aged from 4 to 5 years old.The treatment group received nebulized budesonide suspension, 250-1 000μg per time, two times per day and 10-15 min per time for a maintenance therapy of 12 months.The control group were without any treatment.The changes of IGF-1 and bone mineral density were compared among pre-treatment, 6 months and 12 months post-treatment in treatment group and the changes of height and weight were compared between two groups.Results There were no significant differences in levels of serum IGF-1 and bone mineral density among pre-treatment, 6 months and 12 months post-treatment in treatment group (F =0.259,F =0.217, all P>0.05).There were no significant differences in height and weight between two groups pre-treatment, 6 months and 12 months post-treatment.Conclusion Long-term inhaled corticosteroids has no significant effect on growth indicators in children aged 4-5 years old with bronchial asthma.
2.Prevalence and risk factors of depression in children with type 1 diabetes

Objective To investigate the prevalence of depression in children with type 1 diabetes (T1DM) and analysis its risk factors. Methods A total of 225 children with T1DM were divided into 2 groups according to BDI score:T1DM+depression group (n=123,BDI>10) and T1DM group (n=102,BDI≤10).Then,the subjects in T1DM+depression group were divided into mild depression subgroup (n=42,102 times/d were higher in T1DM+depression group than in T1DM group(P<0.05 or P<0.01).(2) Age,BMI,FPG,HbA1c,insulin doses and ratio of insulin injection>2 times/d were higher in severe depression subgroup than in mild subgroup and medium subgroup,DM course and ratio of insulin injection≤2 times/d were lower in severe depression subgroup than in mild subgroup (P<0.05 or P<0.01).(3) Multiple stepwise regression analysis showed that age,DM course,FPG,HbA1c,incidence of DKA and hypoglycemia,insulin doses and insulin injection>2 times/d were the influencing factors for BDI levels.Logistic regression analysis showed that girl,long DM course,insulin injection>2 times/d,high HbA1c,high incidence of DKA and hypoglycemia were risk factors for depression in T1DM children (P<0.05). Conclusion The higher occurrence of depression in T1DM children indicates that early screening and intervention is necessary.Children with risk factors should be given more psychological care to prevent the occurrence of depression.
3.Effects of KGF/KGFR on the malignant transformation of hydatidiform mole

Objective To investigate the effects of keratinocyte growth factor(KGF) and keratinocyte growth factor receptor(KGFR) on the malignant transformation of gestational trophoblastic disease(GTD).Methods Immunolocalization of KGF/KGFR was performed on sections prepared with the samples from 26 hydatidiform mole,18 invasive mole and 12 choriocarcinoma.The in situ hybridization was used to detect the mRNA of KGF/KGFR in the tissues of hydatidiform mole and GTD.Analysis was performed according to intensity of staining and number of positive cells.Results It was revealed that specific staining for mRNA and protein of KGF/KGFR existed in hydatidiform mole and gestational trophoblastic tumor(GTT).The mRNA and protein of KGF/KGFR were allocated in cytoplasm of syncytiotrophoblasts and cytotrophoblasts of malignant hydatidiform mole,and the KGF/KGFR protein was also expressed in benign tissue,while the expression of KGFR in malignant hydatidiform mole was significantly higher than that in benign tissue(?2=12.775,P
4.Correlation of height change of intervertebral space of fused segment with axial synptoms after anterior cervical fusion

[Objective] To investigate the height change of intervertebral space after anterior cervical fusion and its relation to neck axial symptoms.[Method]Totally 122 patients receiving anterior cervical fusion were followed up for 2 to 5 years,average 3.9 years.The height change of intervertebral space of the fused segment on X-rays were evaluated before,3 months and 2 years after operation.All patients were classified into 3 groups by means of the postoperative height change of intervertebral space.The axial symptoms after operation were observed and analyzed during follow-up.[Result]The incidence of neck axial symptoms was 54.55%,25.84% and 45.45%,respectively.There were significant differences in statistical analysis in group A group B,group B to group C(x2=7.18,P0.0125).[Conclusion]There is significant relation between height change of intervertebral space and neck axial symptoms after anterior cervical fusion.

8.Clinical study on bacterial translocation in patients after digestive tract reconstruction

Objective:To investigate gut barrier dysfunction and bacterial translocation (BT) in patients who underwent digestive tract reconstruction and to study the relationship between BT and acute systemic inflammatory state (SIRS). Method: Sixty patients who underwent selective digestive tract reconstruction were observed. Blood were collected before surgery and 1, 3, 5 days after surgery to detect plasma diamine oxidase(DAO) and bacterial DNA. PCR analysis was performed with β-Galactosidase gene of Eschenchia coli and 16SrRNA gene as target gene. The SIRS of all the patients were observed for 10 days. Result:All the PCR results before operation were negative, while there was positive in 14 patients after digestive tract reconstruction. There were 23 patients with SIRS after surgery, and 12 patients PCR result were positive among 23 patients with SIRS. 85.7% of the patients(12/14) with positive PCR result had SIRS, while 23.9% patients (11/46) with negative PCR result had SIRS (P<0.01).The positive PCR rate in SIRS was 52.2% (12/23), which was remarkably higher than that without SIRS(5.4%, 2/37, P<0.01).The levels of plasma DAO in patients with positive PCR result was significantly higher than those of the patients with negative PCR result (P<0.01). The levels of plasma DAO in patients with SIRS was significantly higher than those of patients without SIRS (P<0.01). Conclusion:The gut barrier dysfunction was closely related to BT, and BT was closely related to postoperative SIRS. PCR analysis can be used in early diagnosis of BT, the positive PCR result might be a useful early warning sign of postoperative SIRS.
9.Changes in platelet parameters and their influential factors in cirrhotic patients with hepatocellular carcinoma

Objective To analyze the changes in platelet parameters and their influential factors in cirrhotic patients with hepatocellular car-cinoma (HCC).Methods The clinical data of 602 cirrhotic patients with HCC who were admitted to the First Hospital of Jilin University from January 201 1 to December 2012,as well as 200 cirrhotic patients hospitalized during the same period,were collected.Statistical analy-sis was performed using SPSS 19.0.Normally distributed continuous data were expressed as mean ± standard deviation;comparison be-tween two groups was made by t test,and comparison between multiple groups was made by analysis of variance.Non-normally distributed data were expressed as median and interquartile range (P25 -P75 );comparison between groups was made by rank sum test.Results Com-pared with the cirrhotic group,the HCC group had significantly higher platelet count (PLT)and plateletcrit (PCT)(t=5.019,P=0.000;t=5.017,P=0.000)and a significantly lower mean platelet volume (MPV)/PLT (t=5.877,P=0.000);there were no significant differences in MPV and platelet distribution width between the two groups (t=-0.942,P=0.347;t=-1.040,P=0.298).The receiv-er operating characteristic (ROC)analysis showed that the area under the ROC curve was 0.636 for PLT,0.633 for PCT,and 0.639 for MPV/PLT in the diagnosis of HCC in cirrhotic patients.Decreases in PLT and PCT were closely related to hepatitis C virus (HCV)infec-tion.Patients with Child-Pugh class A cirrhosis had significantly higher PLT and PCT than those with Child-Pugh class B and C cirrhosis (P<0.01);patients with a maximum tumor diameter of≥5 cm had significantly higher PLT and PCT than those with maximum tumor di-ameters of2-5 cm and≤2 cm (P<0.01).Patients with Child-Pugh class A cirrhosis had a significantly lower MPV/PLT than those with Child-Pugh class B and C cirrhosis (P<0.01);patients with a maximum tumor diameter of≥5 cm had a significantly lower MPV/PLT than those with maximum tumor diameters of2-5 cm and≤2 cm (P<0.01).Conclusion PLT,PCT,and MPV/PLT can be used in the auxiliary diagnosis of HCC in cirrhotic patients,which are related to HCV,Child-Pugh classification,and tumor size.
