This study was aimed to discuss the anti-inflammatory and immunization properties ofRe-Du-Ning (RDN) injection in the treatment of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with phlegm-heat stagnated in the lung syndrome. A total of 110 in-patients were collected from January 2012 to December 2013. Cases were randomly divided into the treatment group (basic treatment plan + 20 ml RDN injection + 250 mL of 5% GS injection, once a day, intravenous injection) and the control group (basic treatment plan + 20 ml of 0.9% NS injection + 250 mL of 5% GS injection, once a day, intravenous injection), with 55 cases in each group. The treatment course was 14 days. Observations were made on traditional Chinese medicine (TCM) symptom score, clinical effect, blood routine examination, blood gas analysis and T lymphocyte subgroups before and after the treatment in both groups. The results showed that in the aspects of TCM symptom score and clinical effect, the posttreatment TCM symptom score and total integration in the treatment group were obviously improved compared to the control group except for shortness of breath, dry mouth and thirsty, flushing (P < 0.05). The total effective rate was obviously better than that of the control group. In the aspect of anti-inflammation, the total white blood cell (WBC) count and the normal neutrophil percentage of both pretreatment and posttreatment in the treatment group were obviously decreased compared to pretreatment (P < 0.05). However, there were no statistical differences on WBC count and normal neutrophil percentage compared to posttreatment in the control group. In the aspect of blood gas analysis, the posttreatment PaCO2 and PaO2 of the treatment group were obviously better than the control group (P < 0.05). In the aspect of immune regulation, the posttreatment T lymphocyte subgroups CD3+ and CD4+ of the treatment group were higher than the control group. The expression inhibition / cytotoxic lymphocyte (CD8+) was obviously lower than the control group (P < 0.05). The CD4+ / CD8+ was back to the normal reference level. It was concluded that in the treatment of AECOPD with phlegm-heat stagnated in the lung syndrome, on the basis of western medicine symptomatic treatment plan, RDN injection was assisted to clear heat, relieve toxin and remove phlegm. It can obviously improve patient’s clinical symptoms and increase the clinical therapeutic effects. The treatment was especially targeted to infection-induced respiratory failure patients combined low immunity with possible identified therapeutic effects.

Idiopathic pulmonary fibrosis (IPF) is defined as a specific form of chronic and progressive fibrosing inter-stitial pneumonia. Till now, it is lack of effective and safe treatment. In recent years, more and more clinical trials of traditional Chinese medicine (TCM) have been conducted with different evaluation criteria and indexes. The knowl-edge is disunity and unsystematic. From the perspective of combination of disease and TCM syndrome model, this ar-ticle summarized and organized the current data and medical standards, and then come up with the new evaluation system of clinical effect and describe important indexes and methods. This article was aimed to improve the clinical trial design and provide high quality evidences for evidence-based medicine.

Bitter taste is one of the important contents in the theory of"Five tastes"in traditional Chinese medicine(TCM).It has the functions of"releasing, dryness and firmness"and plays an important role in the treatment of respiratory diseases.Bitter taste receptors(T2Rs)is a kind of G-protein-coupled receptors, studies have shown that it can act on human airway smooth muscle, relax smooth muscle and dilate bronchus, and play a vital role in the treatment of bronchial asthma and chronic obstructive pulmonary disease. It can also inhibit the inflammatory reaction and reduce the damage of lung tissue by reducing the release of inflammatory factors and promoting the apoptosis of inflammatory cells. The therapeutic mechanism of bitter Chinese herbs action may depend on the effect of T2Rs,the effects of bitter Chinese herbs such as"lowering qi","releasing heat","eliminating dampness"are quite consistent with the cognitions of western medicine that bitter taste receptors could"relaxe bronchus","reduce airway resistance","inhibits inflammatory factors"and"reduces mucus secretion".Based on the understanding of bitter taste in TCM theory and modern research, this study aims to discuss the effect of bitter Chinese herbs depend on T2Rs as the starting point,so as to provide a new research direction for clinical treatment of pulmonary diseases such as bronchial asthma and chronic obstructive pulmonary disease.

In the past decades, numerous chemical analysis studies have identified numerous compounds in tobacco smoke, and the number is increasing,as of 2013,9582 compounds have been identified in tobacco smoke, including nicotine, tobacco smoke-specific nitrosamines, benzo[alpha]pyrene and other substances,as well as cotinine,4-(methylnitrosamine)-1-(3-pyridyl)-1-butanol and other products that are metabolized in the body. The products of metabolic transformation, such as cotinine and 4-(methylnitrosamine)-1-(3-pyridyl)-1-butanol,can target the heart, lungs and other organs, inducing chronic obstructive pulmonary disease, coronary heart disease, lung cancer, esophageal cancer and other types of cancer.Chinese medicine's understanding of"toxicity"originates from the Nei Jing, which believes that"poisonous evil"can be an external pathogenic factor, but can also cause the five organs to lose harmony and produce phlegm and stagnation and other pathological products that accumulate into internal toxicity. The modern medical understanding of tobacco smoke toxic substances and their metabolites is quite consistent with the understanding of"external toxicity"and"internal toxicity"in Chinese medicine,so the author intends to explore the main components and pathogenic mechanisms of tobacco smoke harmful substances and their metabolites. To clarify the etiological characteristics and pathogenic transformation characteristics of"internal"and"external"tobacco smoke toxicity,so as to explore the modernization of"tobacco smoke toxicity"and the combination of tobacco smoke pathogenesis and TCM theory for TCM.Theoretical foundation is laid.

Type 2 Diabetes Mellitus(T2DM)is a chronic metabolic disease characterized by disorders of insulin and glucose. As a disease closely related to our daily diet, the molecular mechanism of its pathogenesis is still unclear.Sweet taste receptors are a kind of G protein-coupled receptors with 7 times transmembrane.Although the research on the pathogenesis of T2DM has been gradually deepened, there are few studies on the mechanism of sweet taste damage in the state of disease.Therefore,in-depth study of the pathological mechanism of T2DM sweet taste receptor damage is of great significance to further understand the molecular characteristics of sweet taste receptors and explore new targets of traditional Chinese medicine in the treatment of T2DM.In this paper, the biological characteristics of sweet taste receptors, signal transduction mechanism of sweet taste receptors, sweet taste receptors and T2DM,sweet taste receptors and"spleen is sweet"are reviewed in order to provide the latest opinions for the targeted prevention and treatment of this kind of diseases by traditional Chinese medicine and the elucidation of related pathological mechanism.
