1.Discussion of non-operation treatment for severe acute pancreatitis

Objective To evaluate the non-operation treatment of the severe acute pancreatitis(SAP).Methods 218 cases of SAP were analyzed retrospectively.Early operation was adopted before 1998(n=96)and early non-operation treatment was applied after 1998(n=122).Results 25 of 96 cases in operation group died (26.1%),while 9 of 122 cases in non-operation group died(7.4%).The complications was higher than in non-operation group(P<0.01).Conclusion Early non-operation treatment is necessary for the early stage of SAP.

3.Study progressing on the effect of bone structural abnormalities on the distal radioulnar joint stability.

The distal radioulnar joint is not only the main load-bearing joint in the wrist, but also the pivot of the rotation of the forearm. It is one of the most important and unique joints in the body. Maintaining the stability of the distal radioulnar joint is very important for our daily life. The tissue to stabilize the distal radioulnar joint includes bone structures and soft tissue structures. Although the contribution of soft tissue structures to its stability is far exceeding that of bone structures, the influence of abnormal bone structure on the distal radioulnar joint cannot be ignored. By reviewing the relevant literatures, this article divides the bone structural abnormalities into congenital and acquired bone structural abnormalities. The effects of congenital and acquiredbone structural abnormalities on the distal radioulnar joint stability are analysized and collated in this article, and its clinical symptoms, clinical grading, clinical treatments are also summerized. The problems of distal radioulnar joint instability in clinical practicing and its future researching directions are briefly described in order to provide some suggestions for future clinical applications.

4.Real-time monitoring of blood transportation temperature conditions based on Internet of Things technology

【Objective】 To monitor the vehicle positioning and temperature of each shipping container during blood transportation by the Internet of Things technology, so as to ensure the blood products for clinical are always within the standard temperature range, and early detect and deal with the temperature runaway during transportation thus realizing the objective and traceable process of blood transportation temperature. 【Methods】 The temperature of each shipping container was collected in real time, and uploaded to the cloud server through the temperature information receiver. The temperature, vehicle positioning and early warning were displayed on the large screen and computer through the cold chain real-time monitoring system platform, and recorded in real time. 【Results】 The comparative validation test of data in 5 months since formal operation showed that the transportation temperature monitoring and recording of the system met the requirements, the handling of abnormal early warning (alarm) was convenient, and the vehicle positioning was accurate. 【Conclusion】 The system achieves real-time monitoring, accuracy and controllability of abnormal disposal, and ensures the blood quality. The blood products with RFID tags can be accurately monitored during transportation.
5.Association between clopidogrel resistance and polymorphism of platelet adenosine diphosphate receptor in patients with coronary atherosclerotic disease.

OBJECTIVETo investigate the relation of clopidogrel resistance to polymorphism of adenosine diphosphate receptor (P2Y12).
METHODSThree hundred and seventy patients with coronary atherosclerotic heart disease, who were admitted in hospital from May 2011 to November 2012 and underwent percutaneous coronary intervention, were enrolled in the study. All patients received antiplatelet therapy (oral aspirin 100 mg per night and clopidogrel 75 mg per day for >7 d). The gene polymorphisms of C34T and G52T P2Y12 receptor were detected by using DNA sequencing technique. The relationship of gene polymorphism with the incidence of clopidogrel resistance and clinical outcomes were analyzed.
RESULTSAmong 370 patients, clopidogrel resistance developed in 100 cases, including 36 males (36%) and 64 females (64%). In the C34T locus, 212 cases were of CC genotype and 158 were of CT+TT genotype; the incidence of clopidogrel resistance in CC genotype was significantly lower than that in CT+TT genotype (P<0.05). In the G52T locus, 218 cases were of GG genotype and 152 cases were of GT+TT genotype; the incidence of clopidogrel resistance in GT+TT genotype were significantly higher than that in GG genotype (P<0.05). After 1-year follow-up, patients with CC genotype had lower incidence of angina recurrence than patients with CT+TT genotype did (13.2% vs 19.6%, Χ2=4.956, P<0.05), and patients with GG genotype had lower incidence of emergency revascularization, angina, and cardiovascular composite endpoint events than patients with GT+TT genotype did (Χ2=4.135,6.823,5.916, Ps<0.05).
CONCLUSIONT-34, -52 mutations on P2Y12 receptor gene may be a risk factor for clopidogrel resistance and adverse cardiovascular events.

8.Effect of 3R Therapy on Quality of Life and Subjective Well-being in Older Patients with Mild Cognitive Impairment in Ningbo Communities

Objective To observe the effect of 3R therapy on quality of life and subjective well-being in older patients with mild cognitive impairment (MCI). Methods 158 patients with MCI were collected from 8 communities in Ningbo city with Diagnostic and Statistical Manual of Mental Disorders (DSM)-IV as the diagnostic criteria, and assessed by Montreal Cognitive Assessment (MoCA). The experimental group (n=79) was intervened by 3R (Reminiscence, Reality Orientation, Remotivation) for 12 weeks, and the control group (n=79) received no intervention. Their quality of life was assessed with WHO Quality of Life-BREF (WHOQOL-BREF) and the subjective well-being was assessed with Philadelphia Geriatric Center Morale Scale (PGC). Results Their quality of life and subjective well-being improved after intervention in the experimental group (P<0.05), and were better in the experimental group than in the control group (P<0.05). Conclusion 3R therapy could effectively improve the quality of life and subjective well-being in patients with MCI.
9.Influence of blood donation before pregnancy on neonatal birth weight

【Objective】 To explore the influence of maternal blood donation before pregnancy on neonatal birth weight. 【Methods】 A total of 6 428 full-term (gestational age ≥37 weeks) singleton pregnant women in Ningbo Medical Center Lihuili Hospital and Ningbo Women and Children′s Hospital from January 2020 to October 2020 were enrolled in this study. The cumulative whole blood donations before pregnancy were obtained through Alipay software. The relevant data of parturients and their fetuses were collected from electronic medical records. 【Results】 The maternal blood donation rate in Zhejiang Province before pregnancy was 14.69%. The average age of women was 29 (27-32), and the median of cumulative blood donation (except 0 mL) before pregnancy was 300 mL. Univariate analysis showed that there was no significant difference in neonatal gender, neonatal birth weight, proportion of low birth weight infants and proportion of macrosomia among non blood donation group, low blood donation group and high blood donation group (P>0.05). After multiple linear regression analysis, it was found that there was no correlation between blood donation before pregnancy and the neonatal birth weight (B=0.123, 95%CI: -1.013-8.461, P>0.05). Multivariate Logistic regression analysis showed that, compared with the non blood donation group, the occurrence of macrosomia was higher in both the low blood donation group and the total blood donation group (OR=1.366, 95%CI: 1.007-1.766, P<0.05; OR=1.369, 95%CI: 1.019-1.851, P<0.05). 【Conclusion】 Maternal blood donation before pregnancy may not be related to neonatal birth weight, but may be related to the probability of macrosomia in their offspring.
10.Design of a closed-loop automatic mechanical ventilation controller

Objective:In the clinical mechanical ventilation practice, the medical workers often set the ventilator parameters only depend on their own experience. In order to change that,we design a Closed-Loop Automatic Mechanical Ventilation Controller.Methods: Design two closed-loop control subsystems to meet the requirements of closed-loop control system. PETCO2 control subsystem used to keep the concentration of PETCO2, and another subsystem design to regulate the respiratory elasticity called PEEP control subsystem. PETCO2 control subsystem can work on its own or work with PEEP control subsystem.Results: Using this closed-loop controller, the partial CO2 pressure and respiratory elasticity of patient was regulated in real time.Conclusion:By using this closed-loop automatic mechanical ventilation controller, the respiratory elasticity was largely reduced with the adequate gas exchange. Therefore, the ventilator using this closed-loop automatic mechanical ventilation controller can improve the ventilation quality and also reduce the risk of lung injury.
