1.Clinical analysis of 76 cases of gastric stump cancer after subtotal gastrectomy

Objective To observe the treatment and outcomes of gastric stump cancer(GSC) after subtotal gastrectomy,and to analyse the possible related factors for the development of GSC. Methods The clinical data of 693 patients with subtotal gastrectomy were retrospectively analysed.The development,treatment and outcomes of GSC were observed,and the possible factors associated with the development of GSC were analysed. Results Seventy-six cases of GSC emerged in the 693 patients with subtotal gastrectomy.Fifty-one of the 76 cases were perfomed radical operations(n=36) or palliative resection(n=15).No death was found during the perioperative period,and the median survival time was 32.7 and 14.2 months,respectively.Those 25 without surgical management died 3 to 11 months after the development of GSC.Among the 76 cases with GSC,stomach was found as the original site of ulcer in 59(22.3%,59/259),and duodenal bulb in the other 17(3.9%,17/434)(P

Objective The paper gives an account of the composition, functions and major techniques used in designing the total management system of radiation and management. Materials mid Methods With the use of the Windows as the development platform, VFOXPR06. 0 was employed to design the application software for tha management of radiation and intervention record data and intervention materials. Results The software can be used for registration, the disposition of diagnostic reports, the retrieval of patient information, the browsing of DICOM format images, statistical analysis and the management of intervention materials. Conclusion Easy to operate and fast in data handling, the system is of great practical value and is worthy of being widely spread.
3.Topical application of nerve growth factor in early fracture healing after peripheral nerve injury

BACKGROUND:Fracture healing mechanism is complex and affected by many factors, and delayed fracture healing or nonunion often occurs. How to promote fracture healing has become a serious problem.
OBJECTIVE:To observe the effect of local application of nerve growth factor on early fracture healing after peripheral nerve injury.
METHODS:Thirty-six healthy male Wistar rats were selected to establish tibial fracture models, which were randomly divided into four groups, with 18 limbs in each group. Group A: tibial fracture+normal saline injectionvia bilateral gastrocnemius muscles; group B: tibial fracture+nerve injury+normal saline injection; group C: tibial fracture+local injection of nerve growth factor; group D: tibial fracture+nerve injury+local injection of nerve growth factor. Calus metrology results were compared among different groups.
RESULTS AND CONCLUSION:The calus volume was the most in the group B at 4 weeks of intervention, but there were no different among the other three groups (P > 0.05). At 2 weeks of intervention, the bone resorption area was significantly larger in the group B than the group D (P < 0.05), and the osteoclast index was significantly higher in the group A than the group C (P < 0.05); while at 4 weeks of intervention, the mineralized bone trabecular width was significantly lower in the group A than the group C (P < 0.05) as wel as lower in the group C than the group D (P < 0.05). These findings indicate that after peripheral nerve injury, local application of nerve growth factor can enhance the osteogenic ability, effectively inhibit osteoclast activity, and promote the early healing of fracture.
4.Comparability of sensory retinal thickness measured by different OCTs to histological studies in rabbit eyes

Objective To study correlation of the retinal nerve epithelium layer thickness measured with different optical coher-ence tomography (OCT) in vivo with histological measurement. Design Experimental study. Participants 15 rabbit eyes. Methods The retina measurement position of 15 rabbit eyes were marked by laser, and then were scanned by OSE-1800 OCT and Stratus OCT. Reti-nal nerve epithelium layer thickness was measured in retinal histological shdes of rabbit eyes. The results measured with three methods were compared and linear regression analyses were done with SPSS11.5 software. Results The average retinal nerve epithelium layer thickness measured with OSE-1800 OCT, Stratus OCT and histological method were 119.5±7.4, 118.0±5.6, and 116.3±8.8μm respec-tively(P=0.292). Retinal nerve epithelium layer thickness measured with both OCT instruments had the best correlation (r=0.914, P= 0.000), and the thickness measured with Stratus OCT and histological method had the better correlation (r=0.872, P=0.001), and the thickness measured with OSE-1800 OCT and histological method had the significant correlation (r=0.833, P=0.002). Conclusions The retinal nerve epithelium layer thickness measured with different OCTs in vivo correlate well with histomorphometry, and the measure-ment of both OCT instruments are accurate. 5.Expression of MTSS1 in cholangiocarcinoma tissue and its relationship with the prognosis of patients

Objective To investigate the expression of MTSS1 in cholangiocarcinoma tissue and its relationship with the prognosis of patients.Methods The specimens of bile duct of 49 patients with hilar cholangiocarcinoma who received surgical excision at the Eastern Hepatobiliary Surgery Hospital from January 2003 to December 2005 were collected.Tissue microarrays of the 49 samples of hilar cholangiocarcinomas and the 10samples of adjancent normal bile duct epithelial tissue were constructed.The expression of MTSS1 was detected by the immunohistochemical staining.The pcDNA3.1-MTSS1 was transferred into the RBE cells and the abilities of proliferation of REB cells were measured by MTT assay.The patients were followed up via out-patient examination and telephone till May 2012.The measurement data were analyzed using the t test,and the count data were analyzed using the chi-square test,the survival curve was drawn by the Kaplan-Meier method,the survival was analyzed using the Log-rank test,and multivariate analysis was done using the COX regression model.Results The results of immunohistochemical staining showed that the expression rate of MTSS1 was 10/10 in the adjacent normal epithelial tissue of bile duct,while 59.2% (29/49) in the cholangiocarcinoma tissues.The proliferative rate of cholangiocarcinoma cells transfected with MTSS1 was 1.55 ±0.05,which was significantly lower than 2.32 ±0.08 of cholangiocarcinoma cells without transfection of MTSS1 (t =4.454,P < 0.05).Gender,age,TNM stage,T stage,differentiation,neural invasion and diameter of tumor did not influence the expression of MTSS1 (x2=0.211,3.471,0.507,0.507,0.368,0.882,0.660,P < 0.05),while lymph node metastasis influenced the expression of MTSS1 (x2=10.436,P < 0.05).All the patients were followed up for 1-59 months,and the median time for follow-up was 16 months.The median tumor-free survival time was 17.9 months in patients with positive expression of MTSSI,and 11.3 months of patients with negative expression of MTSS1,with no significant difference (Log-rank value =3.707,P > 0.05).The median survival time was 34.9 months in patients with positive expression of MTSS1,which was significantly longer than 18.7 months of patients with negative expression of MTSS1 (Log-rank value =5.671,P <0.05).Multivariate analysis showed that MTSS1 was not the independent risk factor influencing the prognosis of patients (x2 =0.406,P > 0.05).Conclusions The expression of MTSS1 is decreased in cholangiocarcinoma tissue,which negatively correlates with lymph node metastasis.MTSS1 could be used as a biomarker in predicting the prognosis of patients with cholangiocarcinoma.

OBJECTIVETo study the molecular mechanism of extracts from Euodia rutaecarpa on hepatotoxicity in mice.
METHODTotally 30 KM mice were divided into 3 groups and orally administrated extracts from E. rutaecarpa for consecutively 15 days. The expressions of Erkl/2, CDK8, CK1e, Stat3 and Src were detected by Western blotting method.
RESULTThe extracts from E. rutaecarpa could up-regulated Erkl/2, CDK8 and CK1e expressions (P <0.01) and down-regulate Stat3 and Src (P <0.01).
CONCLUSIONThe molecular mechanism of E. rutaecarpa on hepatotoxicity may be correlated with Erkl/2, CDK8, CKle, Stat3 and Src signal molecules.

7.Comparison between vertebra fracture fixation plus calcium sulfate cement vertebroplasty and simple fracture fixation for treatment of thoracolumbar burst fractures

Objective To compare the clinical outcome between vertebra fracture fixation plus injectable calcium sulfate vertebroplasty and simple vertebra fracture fixation in the treatment of thoracolumbar burst fracture.Methods A total of 61 patients with thoracolumbar burst fracture treated from January 2005 to December 2008 were involved and divided into two groups,ie,Group A ( treated with three-level fixation at fractured vertebra and injectable calcium sulfate vertebroplasty) and Group B ( treated with three-level fixation at fractured vertebra alone).Group A had 22 males and 10 females,at mean age of 36.8 years (21-65 years).The mean follow-up period was 15.6 months (13-27 months).Group B had 18 males and 11 females,at mean age of 38.3 years (19-70 years).The mean follow-up period was 14.7 months (12-28 months).The ratio of anterior vertebral height,Cobb angle,VAS score were compared between the two groups.Results There were no statistical differences in the aspects of age,sex,fracture segments and preoperative neurological status distribution in the two groups( P >0.05 ).All patients with partial neurologic deficits initially improved for 1-2 grade at the final follow-up.Blood loss and operation time in Group A were less than that in Group B (P <0.05 ).The ratio of anterior vertebral height and the Cobb angle showed no statistical significance (P > 0.05 ),but the ratio of anterior vertebral height and the Cobb angle in Group A was less than those in Group B at the latest follow-up (P <0.05 ).The VAS score showed no statistical significance between the two groups at the latest followup (P > 0.05 ).There was one patient with screw breakage in Group B,while there was no implant failure in Group A.Conclusion The vertebra fracture fixation plus calcium sulfate cement vertebroplasty is a safe and effective method for the treatment of thoracolumbar burst fracture as it can restore the vertebral mechanical strength,achieve and maintain kyphosis correction,decrease the instrument failure rate and loss of vertebral height.
8.Expression of spliced XBP-1 s and ADRP in kidney of diabetic rats

Purpose To investigate the expression of XBP-1s and ADRP in kidney of diabetic rats. Methods Diabetic rat models were successfully established by intraperitoneal injection of STZ. After two months rats were sacrificed and XBP-1s and ADRP were de-tected by immunohistochemistry and Western blot. Results XBP-1s and ADRP were located in renal tubular cells and increased by a-bout 2. 017 times and 1. 544 times in comparison with normal control rats (P<0. 05). Moreover, it was shown that high expression of XBP-1s was commonly accompanied with increased ADRP by Pearson correlation analysis and the correlation coefficient was 0. 723 (P<0. 05). Conclusion The increased XBP-1s may cause the up-regulation of ADRP in the kidney of diabetic rats.
9.Comparison of therapeutic effects of different surgery methods on early hypertensive intracerebral hemorrhage in basal ganglia region in elderly patients and their prognostic factors analysis

Objective To explore the therapeutic effects of different surgery methods on early hypertensive intracerebral hemorrhage(HICH)in basal ganglia region in elderly patients and on prognostic factors analysis.Methods 89 elderly patients with early HICH were randomly divided into four groups according to surgery methods and whether their ICP was monitored.Group A(n=21)was given minimally invasive hematoma drainage,group B(n=23)was given small bone window for removal of hematoma,group C(n=21)and group D(n=24)was given ICP monitoring and corresponding management of ICP on the basis of group A and group B,respectively.The changes of intracranial pressure before and after operation,prognosis and post-operative complications were compared.Results The intracranial pressure was significantly decreased at 3rd day,7th day after operation in group C and D as compared with those in group A and B at the same time points(F=11.76,P<0.05),and the score of GCS was also higher in group C and D at 7th day after operation than in group A,B at the same time points(F=4.72,P<0.05).At 14th and 28th day after operation,the score of GCS was higher in group C than in group A and B(F=19.24,P<0.05),and higher in group C than in group D at 28th day after operation(F=22.26,P<0.05).The dosage of mannitol was significantly lower in group C and group D than in group A and group B(F=18.87,P<0.05).The incidence rate of post-operative complications was 14.3% in group C vs.28.6% in group A(P<0.05)and 20.8% in group D vs.47.8% in group B(χ2=7.04,P<0.05).The proportion of a good recovery and a light disability was significantly higher in group C and D(76.2% and 75.0%)than in group A and B(42.9% and 39.1%)respectively(χ2=14.99,all P<0.05).Conclusions Minimally invasive hematoma drainage shows the advantages of small trauma and a few complications for the treatment of elderly patients with early HICH,and its combination with ICP can early change intracranial pressure and further improves the prognosis.
10.Submental ultrasonongraphy in assessing oropharyngeal swallowing disorders in children with cerebral palsy

Objective To explore the application of submental ultrasonongraphy (SUS) in the assessment of oropharyngeal swallowing disorders in children with cerebral palsy.Methods Seventeen children with cerebral palsy and oropharyngeal swallowing difficulties (7 on nasal feeding,10 on oral feeding) constituted the treatment group while 20 normal counterparts formed the control group.SUS was applied to measure any changes in the thickness of the tongue muscle and the range of hyoid bone displacement when they swallowed 5 ml of water.The results were compared with those assessed using the functional oral intake scale to decide the best cut-off point for detecting tube-feeding-dependent dysphagia.The intraclass correlation coefficient (ICC) of the 20 children in the control group was calculated to evaluate the intra-rater and inter-rater reliability of SUS.Results The average tongue muscle thickness change and hyoid bone displacement amplitude of the children on nasal feeding were significantly smaller than those of the children without nasal feeding and the normal children.The best cut-off point for the tongue muscle thickness change data was 1.0 cm,and that of the hyoid bone displacement amplitude was 1.5 cm.All of the ICCs were above 0.4,indicating good intra-rater and inter-rater reliability for the SUS examination.Conclusion Submental ultrasonongraphy can help assess the swallowing function of children with oropharyngeal swallowing disorders.
