1.A P300 study of cognition in patients with functional constipation

Objective To investigate the relationship between cognitive function and emotional status in functional constipation patients.Methods Thirty-five functional constipation patients (the functional constipation group) and 24 healthy controls (the HCs) were enrolled in an event-related potential (ERP),behavior and electroeneephalogram study while performing an oddball auditory task.Response time,accuracy,latency and P300 amplitude were compared between the two groups.The Hamilton depression rating scale (HAMD-17),Hamilton anxiety scale (HAMA),symptom check list-90 (SCL-90) and Eysenck personality questionnaire (EPQ) were administered prior to the oddball task.Results The average HAMD-17,HAM A and SCL-90 (the somatization,obsessive-compulsive disorder,depression,anxiety and psychosis dimensions) scores of the functional constipation group differed significantly from those of the HCs.The intro-extraversion and neurosis dimensions of the EPQ were also significantly different.There was no significant difference in response time or latency between the two groups.The ERP results showed that the P300 amplitudes at the F3 and F7 sites were significantly different between the functional constipation group and the HCs.But there was no significant difference in P300 latencies at F3 or F7 between the two groups.Conclusions The findings suggest that patients with functional constipation are more susceptible to depression and anxiety,as well as somatization,obsessive-compulsive tendencies and other neurotic personality characteristics.Patients may employ psychological defense mechanisms to avoid the depression and anxiety.The ERP results imply there may be the implicit cognitive dysfunction with emotion in patients with functional constipation.

In order to meet the requirements of cultivating the practical abilities and creativities of students who receive higher education, we initiated the reformation of education in the microbiology experiment teaching methods, implementing a system for module-based education, carefully monitoring every link in teaching, combining the encouragement and strict requirements together, adopting a proper way of assessment. It is proven that the implementation of the educational reformation mobilizes the interests of students and enhances the comprehensive qualities of students, which accomplishes the purposes of teaching.

OBJECTIVETo explore the clinical value of multi-slice spiral computed tomography portography (MSCTP) in assessing severity of liver cirrhosis and predicting episode risks of hepatic encephalopathy (HE).
METHODSEighty-six patients with liver cirrhosis who were hospitalized in the Department of Gastroenterology at the Affiliated Hospital of Yan'an University were included in the study.All patients underwent 64-slice MSCTP to grade the portal vein anatomy.The West Haven criteria were used for semi-quantitative assessment of each patient's mental state.The Child-Pugh grading system was used to assess the extent of cirrhosis.Comparison of measurement data between multiple groups was made by one-way ANOVA analysis, and comparison of such between two groups was made by the Mann-Whitney U test, Ranked data were compared with the rank-sum test, and count data were compared by the Chi-Square test.Correlation analysis was performed with Spearman's correlation test.
RESULTSComparison of the HE grade III group and the HE grade I group showed significant differences between the two in the diameters of left gastric vein, the splenic vein, the intrahepatic left portal vein and the intrahepatic right portal vein (P less than 0.05).Comparison of the Child-Pugh grade C group and the Child-Pugh grade A group showed significant differences between the two in diameters of the left gastric vein, the splenic vein, the intrahepatic left portal vein and the intrahepatic right portal vein (P less than 0.05).The diameters of the main portal vein were not significantly different between the ChildPugh grades and HE classifications (P more than 0.05).The results of MSCTP did show significant differences between different HE classifications in patients with liver cirrhosis and the rate of formation of portal vein thrombosis and fistulas of the hepatic artery-portal vein (P less than 0.05), .but no significant differences with the esophageal and gastric varices, varicose veins around the esophagus, and periumbilical varicose veins (P more than 0.05).HE classification was significantly correlated with formation of portal vein thrombosis and fistula of the hepatic artery-portal vein (r=0.687, P less than 0.05 and r=0.565, P less than 0.05, respectively).MSCTP grading (grade 1:n=35, grade 2:n=36, grade 3:n=15) was not correlated with the Child-Pugh grade (grade A:n=36, grade B:n=32, grade C:n=18) (Z=-0.135, P more than 0.05).Incidence of HE was significantly different among the different MSCTP grades (grade 1:0%(0), grade 2:33.3% (12/36), grade 3:66.7% (10/15); x2=26.468, P less than 0.05).The MSCTP grade was significantly correlated with the episode risks of HE (r=0.552, P less than 0.05).
CONCLUSIONMSCTP may be valuable for assessing severity of liver cirrhosis and for predicting episode risks of HE; however, future studies with larger sample numbers is required for validation of our findings.

6.Risk factors and nursing interventions of infection in elderly patients with total hip replacement in Operating Room

Objective To explore the risk factors of the infection in elderly patients with total hip replacement in Operating Room and study the effects of the targeted nursing interventions to the infection. Methods A total of 296 patients with total hip replacement in our hospital from January 2001 to December 201 3 were included and divided into infection group and control group. Infection group contained 1 02 patients with surgical incision infection and control group contained 1 94 patients without surgical incision infection.The demographic data,medical history,blood index,Harris function score,operation situation and postoperative drainage time were recorded.Infection diagnosis depended on “Diagnose Standard of Nosocomial Infection”.The risk factors were analyzed by single factor analysis and step-wise Logistic multiple regression analysis.The targeted nursing interventions were proposed.The effects of the targeted nursing interventions were studied in 56 patients from February 201 4 to August 201 5.Results Single factor analysis showed that hemoglobin concentrations, joint operations, operative character, visiting during operations and operative time were significantly different between infection group and control group (P <0.01 ).Logistic multiple regression analysis showed that hemoglobin concentrations,joint operations,operative character,visiting during operation and operative time were relative risk factors of postoperative infection (P <0.01 ).The infection ratio of 56 patients with nursing interventions from February 2014 to August 2015 was 17.8% (1 0 /56),which was significantly decreased comparing with the ratio 34.5% (102 /296)from January 2001 to December 201 3 (P <0.05 ).Conclusions Low hemoglobin concentrations,joint operations, emergency operations,visiting during operation and long operative time are risk factors of postoperative infection in elderly patients with total hip replacement in Operating Room.The targeted nursing interventions can reduce the infection ratio and deserves further clinical promoting.
7.Massive gastrointestinal bleeding from Meckel diverticulum with ectopic pancreatic tissue.

Meckel diverticulum (MD), a congenital gastrointestinal anomaly, is often involved in pediatrics, but less in the adult population. The patient in this report was a 69-year-old female presented with massive gastrointestinal bleeding causing hemorrhagic shock due to MD containing ectopic pancreatic tissue. A review of the literature revealed that gastrointestinal bleeding from MD containing ectopic pancreatic tissue is rare in adults and difficult to be identified preoperation. MD should be considered as one of the differential diagnosis for lower gastrointestinal bleeding, although scarce in adults, especially when the patient has massive painless bleeding.

8.Dual cell seeding to improve cell retention on polytetrafluoroethylene grafts.

OBJECTIVETo improve cell retention on the graft with dual cell seeding technique, adding a layer of smooth muscle cells (SMCs) between the graft and endothelial cells (ECs).
METHODSPolytetrafluoroethylene (PTFE) grafts pretreated with fibronectin were seeded with SMC followed by ECs one day later and exposure to an in vitro flow system. The number of ECs on grafts was counted under microscope.
RESULTSAfter exposure to the flow for 1 hour, 61% of the ECs lost when the ECs were seeded alone, whereas only 27% of the ECs lost when the ECs were seeded on the top of SMCs. Preconditioning of a cell seeded graft in a low rate flow did not improve cell retention when late exposure to a higher rate flow.
CONCLUSIONThis in vitro study indicates that using SMCs as a media layer between ECs and graft surface can improve the retention of seeded ECs on PTFE graft.

9.Osteogenic potential of hBMP-2 gene transduced human adipose-derived stem cells

Objective To investigate the ostengenie potential of adipose-derived stem cells(AD- SCs)when exposed to adenovirns containing hBMP-2 cDNA(Adv-hBMP-2)and offer a choice of cell source for gene therapy and tissue engineering.Methods Human adipose tissues were obtained from patients who received orthopaedic surgery or liposuction.ADSCs were obtained by digesting the adipose tissues.Firstly,flowcytometric analysis was performed for the confirmation of mesenchymal stem cell ori- gin and the surface markers including CD34,CD44,CD68,CD71,CD90,and CD105.The ADSCs were transfected by Adv-hBMP-2 and the effects were tested in vitro,lmmunoprecipitation and Western blotting and ELISA were performed for confirming BMP gone transduction and its stable expression.The transform of ADSCs was assessed by extracellular ALP staining,intracellular ALP spectrophotometry,von Kossa staining and RT-PCR.In the in vivo experiment ADSC-Adv-hBMP-2 cells were injected into the hind limb of nude mice and analyzed radiographically and histologically.Results ADSCs were successfully isolated from human adipose tissues.The isolated ADSCs expressed CD44,CD71,CD90 and CD105 and CD34 and CD68 were absent.The result confirmed the mesenchymal stem cell origin of the cells.West- ern blotting and ELISA confirmed successful and persistent hBMP-2 production by ADSC-Adv-hBMP-2 cells.Extracellular ALP staining,intracellular ALP spectrophotometry,yon Kossa staining and RT-PCR revealed that ADSCs treated with Adv-hBMP-2 had a tendency of transfering into osteoblast.X-ray and H&E sections from hind limb of nude mice injected with ADSC-Adv-hBMP-2 cells confirmed bone forma- tion at 2 weeks.Conclusions Liposuction aspirates contain abundant ADSCs that can be transduced with hBMP-2 gene,and the tranduced ADSCs differentiate into the osteoblast.ADSCs may be an ideal source of mesenchyme-lineage stem cells for gone therapy and tissue engineering.
10.Impacts of electroacupuncture at Lingtai (GV 10) and Shendao (GV 11) on premature heartbeat.

OBJECTIVETo observe the specific clinical efficacy of electroacupuncture at Lingtai (GV 10) and Shendao (GV 11) on premature heartbeat.
METHODSSeventy-two cases of premature heartbeat were randomized into an observation group and a control group. In the observation group, electroacupuncture was applied at Lingtai (GV 10) and Shendao (GV 11). In the control group, electroacupuncture was applied at bilateral Xuanzhong (GB 39). The treatment was given once a day and 10 times made one session in both groups. Respectively, the dynamic electrocardiogram detection was done before treatment and after one session of treatment in each group.
RESULTSThe total effective rate was 47.1% (16/34) in the observation group and 6.5% (2/31) in the control group. The efficacy in the observation group was superior to that in the control group, presenting the statistical significant difference in comparison (P < 0.05).
CONCLUSIONElectroacupuncture at Lingtai (GV 10) and Shendao (GV 11) has specific clinical efficacy on premature heartbeat.
