1.Application of Propfol Closed-loop Anesthesia System in Pediatric Otorhinolaryngologic Surgery

Objective To evaluate the application of propfol?remifentanil closed?loop and opened?loop anesthesia in children otorhinolaryngologic operation under BIS monitoring. Methods Fourty children underwent elective children tonsillectomy and adenoidectomy were recruited for the study and randomly divided into two groups:closed?loop group(group T)and opened?loop group(group C),with 20 children each. The two groups all received total intravenous induction,then the subjects of closed?loop group were administrated with a method of system automatic regulation of plasma target control and the opened?group cases underwent manual adjustment of plasma target control method according to the BIS value during op?eration. The variables of non?invasive blood pressure(BP),heart rate(HR),pulse oxygen saturation(SpO2),electrocardiogram(ECG),bispec?tral index(BIS),the number of times the manual adjustment of propfol target control applied,and recovery time,were recorded respectively at the time points of before anesthesia(T0),immediately after intubation(T1),5 min after intubation(T2),10 min after(T3),15 min after(T4),30 min after(T5)and the removal of endotracheal intubation(T6). Results The total dosage of propfol in group T was less than the dosage in group C with dosage decrease of 9.83%;there was no significant statistical difference of the total dosage of remifentanil. Conclusion The use of propfol?remi?fentanil closed?loop anesthesia in children otorhinolaryngologic operation is safe,easy,and of strong controllability,which should be widely promoted.
2.Outcome analysis of endemic fluorosis control in Guangxi in 2006

Objective To investigate the distribution of water fluoride and the status of water-improving defluoridation projects,thus to explore the condition of endemic fluorosis in Guangxi Province.Methods According to"The National Technical Scheme for Endemic Disease Contml in 2005",the fuorine content in water Was determined by F-ion selective electrode,children's dental fluorosis was checked by Dean method.and the skeletal fluorosis was checked by the standard of clinical scale of skeletal fluorosis.Results 305 water samples in 61 villages were examined,among which 71 waters were exceeded the standard,accounting for 23.28%(71/305).The projects of defluoriding drinking water were running well except one was discarded.The prevalence of dental fluorosis was 13.55%(356/2627),the prevalence of skeletal fluorosis was 4.02%(65/1615).Conclusions The situation of endemic fluorosis control is not optimistic in Guangxi,which needs fuaher prevention and controls.
3.Correlations between human papillomavirus types and recurrence of perianal and anal condyloma acuminatum among men who have sex with men

ObjectiveTo determine the genotypes of human papillomavirus(HPV) and to estimate their correlations with recurrence of perianal and anal condyloma acuminatum(CA) among men who have sex with men(MSM) in Shanghai.MethodsTissue specimens were obtained from the lesions of perianal or anal CA in 57 MSM.Flow-through hybridization and gene chip were used to determine the genotypes of HPV in these specimens.Follow-up was carried out to observe the recurrence of CA in these patients after CO2 laser treatment.ResultsOf the 57 specimens,56(98.2%) were positive for HPV,and a total of 11 HPV subtypes were identified.The predominant genotypes were HPV 11 (42 cases,75% ) and HPV 6 (30 cases,53.6%),sequentially followed by HPV 16 (8 cases,14.3%),HPV 18 (4 cases,7.1%),HPV 33 (2 cases,3.6%),HPV 59(2 cases,3.6%),HPV cp8304 (2 cases,3.6%),HPV 31 (1 case,1.8%) and HPV 35 (1 case,1.8%).Of the HPV-positive specimens,64.3%(36/56) harbored low risk HPV types(11 and/or 6),and 35.7 % (20/56) harbored high risk HPV types.Among the 20 specimens positive for high risk HPV,17(85%) harbored both low risk- and high risk-HPV genotypes.During the 6-month follow-up after CO2 laser treatment,recurrence was observed in 95% of patients with high risk HPV and 66.7% of patients with low risk HPV (P < 0.05).ConclusionsAmong MSM with perianal and anal CA in Shanghai,the predominant genotypes of HPV are low risk types 11 and 6; high risk HPV infection is usually complicated by low risk HPV infection; the recurrence of CA is higher in patients with high risk HPV than in those with low risk HPV.
4.Quantitative analysis of SOX9 and type Ⅱ collagen mRNA in the three-lineage differentiation of rat mesenchymal stem cells

BACKGROUND:The main component of cartilage, type Ⅱ col agen gene expression in chondrocyte is positively correlated with SOX9 concentration in a dose-dependent manner.
OBJECTIVE:To observe the variation of SOX9 and type Ⅱ col agen mRNA content at different periods in the differentiation process (osteogenic, chondrogenic, adipogenic induction) of mesenchymal stem cel s, and to explore the correlation of SOX9 expression and type Ⅱ col agen.
METHODS:Bone marrow mesenchymal stem cel s were isolated from 4-week-old Kunming mice, and cultured in vitro to passage 3. The cel phenotype was identified with flow cytometry. Cel s were divided into three groups and subjected to three kinds of induction conditions favorable for adipogenic, chondrogenic and osteogenic differentiation, and each group was observed at three time points. In addition, the non-induced cel s were used as a control group. The total RNA of cel s was extracted at 3, 7, 14 days after induction, and SOX9 and type Ⅱ col agen mRNA was quantified with reverse transcription-polymerase chain reaction. The induced cel s were stained by immunofluorescence to observe the differentiation and perform statistical analysis.
RESULTS AND CONCLUSION:Passage 3 bone marrow mesenchymal stem cel s grew wel , and cel phenotype was confirmed as stem cel s by flow cytometry. The staining results showed that, the cel s differentiated into chondrocytes, adipocytes and osteoblasts. The SOX9 mRNA levels in the induced cel s were the highest in chondrogenic differentiation group, then in osteogenic differentiation group, and the lowest in adipogenic differentiation group. Type Ⅱ col agen mRNA levels in the induced cel s were the highest in chondrogenic differentiation group, then in adipogenic differentiation group, and the lowest in osteogenic differentiation group. SOX9 expression in chondrogenic differentiation group increased at 3 and 7 days, and then decreased at 14 days. While type Ⅱ col agen expression increased at 3, 7, 14 days. SOX9 mRNA levels increased as the osteogenic differentiation, while type Ⅱ col agen expression gradual y decreased. There was no significant difference in the SOX9 mRNA expression between adipogenic differentiation group and control group (P>0.05), while type Ⅱ col agen expression was not regularly changed. Experimental findings suggest that, critical effect of SOX9 in chondrogenic differentiation is better than that in osteogenic and adipogenic differentiation. SOX9 is associated with type Ⅱcol agen, which may alter along with the SOX9 in the early chondrogenic differentiation;SOX9 may play a fine-tuning role in the process of chondrogenic and osteogenic differentiation.
5.Depressive symptoms and related factors among HIV-positive men who have sex with men in Shanghai

Objective To examine the prevalence of depressive symptoms and related factors among HIV-positive men who have sex with men (MSM) in Shanghai.Methods All of the HIV-positive MSM were enrolled to participate in a cross-sectional study with questionnaire-based interview during Nov.,2014 to Nov.,2015.Depression was measured with a 12-item version of the Center for Epidemiological Studies Depression Scale (CES-D),where a score of 9 had been recommended as the cutoff score to indicate possible depressive symptoms.Results A total of 505 participants were recruited with a median age of 30.Among them,the majority were aged 21-44 (77.2%),non-local resident (64.6%),unmarried (73.1 %) and university graduate or above (62.1 %).Depressive symptoms were detected from 235 MSM and the prevalence of depression was 46.5 %.Multiple logistic regression analysis showed that MSM who were non-local residents (OR =1.66,95 % CI:1.01-2.49)and had more than one male sex partner in the past year (OR=2.21,95% CI:1.43-3.41) were more likely to have depressive symptoms compared with local residents and had only one male sex partner or less.On the contrary,compared with the education level of junior middle school or below and having no long-term partners,those who were senior high school educated (OR=0.39,95% CI:0.21-0.74) and had homosexual behavior with main long-term partners without condoms (OR =0.38,95 % CI:0.17-0.85) had lower risk to be depressive.Conelusions The prevalence of depressive symptoms among HIV-infected MSM is high.Residence,education level,homosexual behavior with main long-term partner and number of male sex partner are significantly associated with depressive symptoms,suggesting psychological interventions should be further explored and studied.

7.Analysis of 41 cases of neurosyphilis presenting with cerebral infarction

Objective To investigate the clinical characteristics of neurosyphilis presenting clinically with cerebral infarction and study its treatment and prognosis. Methods A retrospective analysis was conducted in 41 patients with neurosyphilis who presented clinically with cerebral infarction. The clinical manifestations, neurological deficit scores, activity of daily life (ADL) scores, mental scale scores, and laboratory and imaging examination results of the patients were analyzed. Results Of the 41 patients, 23 exhibited reduced intelligence, 30 had limb paralysis, 8 had limb numbness, 2 complained of dizziness, and 1 experienced epileptic attacks. Head magnetic resonance imaging (MRI) revealed the presence of brain atrophy and multiple infarction foei without obvious space-occupying effect of cerebral edema in these patients. All the patients showed positive results of serum and cerebrospinal fluid Treponema pallidum hemagglutination assay (TPHA). After treatment with high-dose penicillin, 95.1% of the patients had improvements of the clinical symptoms. Conclusion The diverse clinical manifestations of neurosyphilis often result in misdiagnosis of the disease. Patients with neurosyphilis show multiple cerebral infarction foci and cerebral atrophy without obvious space-occupying effect on MRI. Penicillin can be an effective treatment in such cases, which generally have low rate of negative TPHA within 3 months. Such patients have more favorable clinical outcomes than those with common cerebral infarction, but poor prognosis is indicated in cases with concurrent extensive cerebral infarction.
8.Association on the haplotypes of CYP4F2 gene and myocardial infarction

Objective The aim of this study was to assess the association between human CYP4F2 gene and myocardial infarction (MI),using a haplotype-based case-control study. A separate analysis on gender was also carried out. Methods There were 250 MI patients and 250 control subjects genotyped for 5 SNPs of the human CYP4F2 gene (rs3093105, rs3093135, rs1558139, rs2108622, rs3093200). Data were assessed on 3 separate groups: the total subjects, men and women. Results For men, G allele was significantly higher in the MI patients than in the control subjects and the overall distribution of the haplotypes was significantly different between the MI patients and the control subjects (P=0.002). Also in men, the frequency of T-C-G haplotype was significantly higher for MI patients than for control subjects (P=0.002), and the frequency of T-C-A haplotype was significantly lower for M1 patients than for control subjects (P=0.003). Conclusion Data from the present results indicated that MI was associated with G allele of rs2108622 in men, suggesting that T-C-G haplotype might serve as genetic marker for MI in men.
10.Protective effect of L-carnitine on the testis and epididymis against ornidazole-induced injury in male rats.

OBJECTIVETo investigate the protective effect of L-carnitine on the testis and epididymis against ornidazole (ORN)-induced injury in male rats.
METHODSForty male SD rats weighing 200 -230 g were randomly divided into 5 groups, Group A treated with 0.5% sodium carboxymethyl cellulose, and Groups B, C, D and E with ORN at the daily dose of 400 mg/kg, 800 mg/kg, 400 mg/kg plus LC 100 mg/kg and 800 mg/kg plus LC 100 mg/kg, respectively, all by oral gavage for 20 days continuously. Twenty-four hours after the last administration, all the rats were put to death, their testes and epididymides harvested, weighed and subjected to HE staining. The indexes of the testes and epididymides were obtained and their histopathological changes observed.
RESULTSCompared with Group A, Groups B and C showed significant decreases in the indexes of the testis and epididymis (P < 0.05 and P < 0.01), while Group D exhibited no difference and Group E extremely significant difference (P < 0.01). HE staining revealed that the spermatogenic cells at all levels of testicular seminiferous tubules were neatly arranged in Group B, caduceus in some seminiferous tubules, with decreased number of sperm and sporadic spermatogenic cells in the epididymal duct. Necrotic and caduceus spermatogenic cells were observed in the seminiferous tubules of Group C, with significantly decreased number of sperm and lots of non-sperm cell components in the epididymal duct. No obvious changes were found in the testicular seminiferous tubules, nor evident reduction in the number of sperm in the epididymal duct of Group D. Group E showed decreased number of sperm in the testicular seminiferous tubules, necrotic and caduceus spermatogenic cells, obviously reduced number of sperm and a lot of non-sperm cell components in the epididymal duct.
CONCLUSIONORC can induce histopathological changes in the testis and epididymis of male rats, and L-carnitine plays a role in protecting the testis and epididymis from ORN-induced injury in male rats.
