1.Imaging Features of Systemic NHL Lesions in Parenchymal Brain

Objective To alanyze the imaging features of brain in systemic NHL.Methods CT and MRI of brain in 8 cases of systemicNHL proved pathologically were performed.Imaging findings of brain were analysed and by comparison with that in 73 cases of brainmetastases with marked enhancement after contrast administration.Results 13 lesions in 8 cases were found by CT,the findings on contrast-enhanced CT included:①Ten lesions showed low density with obscure border;②Three nodular lesions showed high density(including 2 homogeneously and 1 ring-like enhanced nodules).All lesions showed neither ventricle enlargement nor occupying effects.Those enhanced showed no edema around the nodules.Of the 4 cases with MRI,low T_1 signal and high T_2 signal with no change after contrast administrationwere seen in 3 cases.One showed ring-like enhancement with no edema around.While 89% of the 73 brain metastatic cases showeddifferent extent of edema.Conclusion The imaging findings in some of the lesions in this study are different from that described before in literatures.In comparison with other metastases,the marked-enhanced nodules with no edema were found to be a specialty of secondary NHL in brain in our cases.
2.Arthrography-assisted reduction in minimally invasive treatment of tibial plateau fractures

Objective:To evaluate arthrography-assisted reduction in minimally invasive treatment of tibial plateau fractures.Methods:From January to May 2019, 9 patients with tibial plateau fracture were treated by arthrography-assisted minimally invasive surgery at Department of Orthopaedics and Traumatology, Hong-Hui Hospital. They were 6 males and 3 females with an average age of 45.5 years (from 27 to 63 years). According to the Schatzker classification, there were 6 cases of type Ⅱ, one of type Ⅲ and 2 cases of type Ⅳ. Arthrography was used to determine the lowest filling point of contrast medium after intra-articular injection and to ensure satisfactory reduction of articular surface after fracture reduction and bone grafting, followed by minimally invasive percutaneous plate implantation. Comparisons were made between preoperation and 6 months postoperation in the range of knee motion and knee scores of The Hospital for Special Surgery (HSS). Knee X-ray films were evaluated by the Rasmussen imaging system at 6 months postoperation. Collapse of articular surface was recorded pre- and post-operation as well as postoperative complications.Results:All the 9 patients were followed up for 6 to 9 months (average, 7 months). All patients achieved bony union within 12 weeks postoperation. The preoperative knee flexion (60.7°±13.1°) and HSS (51.9±5.7) were significantly improved to 122.4°±10.8° and 84.9±5.3 at 6 months postoperation ( P<0.05), but there was no significant difference between preoperation and 6 months postoperation in knee extension (4.4°±2.5° versus 4.6°±2.9°) ( P>0.05). The collapse of articular surface was improved from preoperative 9.5 mm (from 5 to 15 mm) to postoperative 1.3 mm (from 0 to 3 mm). The Rasmussen imaging scores at 6 months postoperation showed 6 excellent and 3 good. No such postoperative complications occurred as incision infection, joint stiffness, loosening or breakage of implants. Conclusion:Arthrography helps minimally invasive treatment of tibial plateau fracture because it can better display fracture collapse during operation and accurately judge indirect reduction of articular surface.
3.Case-control study on the treatment of humerus supracondylar fracture of Gartland III in children by manipulative reduction and Kirschner wire percataneous internal fixation.

OBJECTIVETo study the clinical effect of the Gartland III humerus supracondylar fractures in children by manipulative reduction and Kirschner wire percataneous internal fixation.
METHODSFrom July 2010 and July 2013, 60 patients with Gartland III humerus supracondylar fracture were selected and divided into treatment group and control group. In the treatment group 32 patients were treated with traditional bone setting tetradeca-manipulative reduction and percataneous Kirschner wire internal fixation,included 18 males and 14 females with an average age of (7.8 +/- 2.7) years old ranging from 5 to 11; in the control group 28 patients were treated with open reduction and Kirschner wire internal fixation,included 16 males and 12 females with an average age of (7.2 +/- 3.0) years old ranging from 4 to 12. The motion range of the elbow joint,the time of fracture clinical healing, and the effect after 6 months of Flynm clinical functional assessment standards were observed and compared.
RESULTSThe average fracture healing time of the control group (5.01 +/- 0.43) weeks was longer than that of the treatment group (4.29 +/- 0.29) weeks (t = 7.49, P = 0.00). At 6 months after treatment,the elbow motion range of the treatment group (146.02 +/- 2.28) was more than that of the control group (140.76 +/- 4.42) (t = -5.67, P = 0.00). At 6 months after treatment, according to Flynn evaluation, in the control group,there were 7 cases as excellent, 16 as good, 4 fair, 1 poor; in the treatment group, excellent in 21, good in 9, fair in 2 (U = 3.09, P = 0.002).
CONCLUSIONManipulative reduction and Kirschner wire percataneous internal fixation for treatment of children's Gartland III humerus condyle fractures can shorten fracture clinical healing time and the clinical curative effect is better.

4.Preoperative assessment of gastric artery in patients with gastric cancer by CT angiography on 64-slice spiral CT

Objective To evaluate the efficacy of different three-dimensional CTA methods on 64-slice sprial CT in the preoperative assessment of gastric arteries and their variations.Methods Sixty-six consecutive patients with gastric cancer who underwent 64-slice spiral CT examinations preoperatively were retrospectively studied.To get the STS-MIP images, the thickness of slab was adjusted according to the inner diameter of targeted blood vessels and their cross-layer distribution.After four weeks, the AVVR images of all cases was got by the auto-vessel technique.The demonstration rates and origins of the direct and indirect feeding arteries were analyzed on AVVR and STS-MIP.McNemar tests were used to compare the detection rates of gastric feeding arteries by STS-MIP and AVVR.The relationship between CT value and display rate of vessels was analyzed using independent-samples t test The variations of blood vessels were analyzed.Results The display rate of indirect feeding arteries were all 100% (66/66) by STS-MIP and AVVR.The display rates of left gastric artery (LGA) and right gastroepiploic artery (RGEA) were 98.5% (65/66), 100.0% (66/66) and 97.0% (64/66), 100.0% (66/66) by STS-MIP and AVVR respectively.The display rates of right gastric artery (RGA), left gastroepiploic artery (LGEA), short gastric artery (SGA) and posterior gastric artery (PGA) by AVVR were lower than those of STS-MIP with statistical significances [RGA:68.2%(45/66) vs.98.5% (65/66), P<0.01; LGEA: 53.0% (35/66) vs.97.0% (64/66), P<0.01; SGA: 7.6% (5/66) vs.59.1 %(39/66), P<0.01; PGA: 18.2% (12/66) vs.63.6% (42/66), P<0.01 ].The demonstration rates of LGEA, RGEA and SGA increased accompanied with the increasing of CT value in celiac axis (LGEA: 35 cases displayed with mean CT value of (272±44) HU, 31 cases did not display with mean CT value of (229±42) HU, t=4.043, P<0.01; RGEA: 64 cases displayed with mean CT value of (256±44) HU, 2 cases did not display with mean CT value of (141 ±26)HU, (=3.641, P<0.01; SGA:5 cases displayed with mean CT value of (298 ±39),61 cases did not display with mean CT value of (249±47)HU, t=2.278,P<0.01). 5.n-3 PUFA can inhibit the apoptosis of intestinal epithelial cells of chronic rejection after small intestinal transplantation

Objective: The aim of our work was to investigate the effects of n-3 polyunsaturated fatty acids on apoptosis, granzyme B and perforin expression of intestinal epithelial cells of chronic rejection after small intestinal transplantation. Methods: Small bowel transplantation was performed and rats were divided into three groups: Group 1, Lewis-to-Lewis, group 2, F344-to-Lewis, dietary corn oil, Group 3, F344-to-Lewis, dietary fish oil. All recipients were killed after 16 weeks of posttransplantation. The apoptosis rate of mucosal cells were evaluated by flow cytometry. The expressions of granzyme B and perforin were analyzed by reverse transcriptase RT-PCR. Results: A high apoptotic rate was observed when the allografts demonstrated one or more histological features of chronic rejection. N-3 polyunsaturated fatty acids decreased the rate of the apoptosis and inhibitted the expressions of granzyme B and perforin. Conclusion: N-3 polyunsaturated fatty acids can suppress the chronic rejection in small intestinal transplantation.
6.Interleukin-1α and interleukin-6 expressions in craniopharyngiomas

Objective To investigate the correlations of interleukin-1α (IL-1α) and IL-6expressions in craniopharyngiomas with the pathological subtypes,removal rate and recurrence rate,and explore their clinical significance.Methods Sixty-nine pathologic specimens ofcraniopharyngiomas,collected in our hospital during the microsurgery from January 2003 to October 2009,were chosen in our study; the craniopharyngiomas were classified into two different pathological subtypes:enamel cells and nipple squamous cells.The IL-1α and IL-6 expressions were detected in craniopharyngioma tissues by immunohistochemistry.The correlations of IL-1α and IL-6 expressions in craniopharyngiomas with the pathological subtypes,removal rate and recurrence rate were analyzed statistically.Results The total tumor removal was achieved in 50 cases (72.5%),subtotal removal in 13 cases (18.8%) and partial removal in 6 cases (8.7%).Nine cases (14.1%) were recurrent during the follow-up.IL-6-positive expression was detected in 28 cases,and IL-1α-positive expression in 3 cases; IL-1α and IL-6 expressions in craniopharyngiomas with different pathological subtypes showed no significant difference (P>0.05);craniopharyngiomas with postoperative recurrence or without showed no obvious difference (P>0.05).The difference of IL-1 α expression in patients with total removal,subtotal removal and partial removal showed no statistically significance (P>0.05); while that of IL-6 expression had significant difference (P<0.05).Conclusion IL-6 involves in the inflammatory reaction between craniopharyngioma and its surrounding brain tissues,which may be the important influencing factor of total removal of the craniopharyngioma.
7.Depressive symptoms and related factors among HIV-positive men who have sex with men in Shanghai

Objective To examine the prevalence of depressive symptoms and related factors among HIV-positive men who have sex with men (MSM) in Shanghai.Methods All of the HIV-positive MSM were enrolled to participate in a cross-sectional study with questionnaire-based interview during Nov.,2014 to Nov.,2015.Depression was measured with a 12-item version of the Center for Epidemiological Studies Depression Scale (CES-D),where a score of 9 had been recommended as the cutoff score to indicate possible depressive symptoms.Results A total of 505 participants were recruited with a median age of 30.Among them,the majority were aged 21-44 (77.2%),non-local resident (64.6%),unmarried (73.1 %) and university graduate or above (62.1 %).Depressive symptoms were detected from 235 MSM and the prevalence of depression was 46.5 %.Multiple logistic regression analysis showed that MSM who were non-local residents (OR =1.66,95 % CI:1.01-2.49)and had more than one male sex partner in the past year (OR=2.21,95% CI:1.43-3.41) were more likely to have depressive symptoms compared with local residents and had only one male sex partner or less.On the contrary,compared with the education level of junior middle school or below and having no long-term partners,those who were senior high school educated (OR=0.39,95% CI:0.21-0.74) and had homosexual behavior with main long-term partners without condoms (OR =0.38,95 % CI:0.17-0.85) had lower risk to be depressive.Conelusions The prevalence of depressive symptoms among HIV-infected MSM is high.Residence,education level,homosexual behavior with main long-term partner and number of male sex partner are significantly associated with depressive symptoms,suggesting psychological interventions should be further explored and studied.
8.Prediction of deep venous thrombosis in lower extremities after total knee arthroplasty by preoperative and postoperative platelet changes

Objective To explore the predictive significance of platelet changes on deep venous thrombosis of the lower limbs after total knee replacement,and to guide the early clinical prediction and prevent the occurrence of thrombotic events.Methods A retrospective study was conducted of the 50 patients who had been treated by total knee arthroplasty for degenerative knee osteoarthropathy from January 2016 to June 2018 in the Department of Orthopedics and Traumaology,Xi'an Jiangtong University Medical College Red Cross Hospital.There were 10 males and 40 females,aged from 47 to 80 years (average age 65.1 years).According to the results of B ultrasound examination of the lower extremities,the patients were divided into the non embolic group (24 cases) and the embolic group (26 cases).The platelet counts and hemoglobin levels of the two groups were recorded before and after operation,and their respective differences were calculated.The difference of hemoglobin difference between the two groups was analyzed.At the same time,the difference of platelet count and total knee joint were examined.Whether the deep venous thrombosis of the lower extremity after replacement was statistically significant.If there was no statistical difference in the difference in hemoglobin between the two groups,and the difference in platelet count was statistically significant,the ROC curve was made to determine the optimal screening point.If obeying the normal distribution and the homogeneity of the variance,an independent sample t test was performed on the platelet count difference and the hemoglobin amount difference between the two groups,and the dose data in accordance with the normal distribution was expressed as (Mean ±SD).Results The difference of platelet count was (30.4 ± 14.8) ×109/L in thrombosis group and (53.5 ± 15.2) × 109/L in thrombosis group.The difference of platelet count between the two groups had statistical significance (P < 0.001).The best screening point of platelet count difference was 34.0 × 109/L.The difference of hemoglobin count between thrombosis group and thrombosis group was (20.8 ± 9.3) g/L and (24.0 ± 10.7) g/L,there was no significant difference in hemoglobin between the two groups (P > 0.05).Conclusions Preoperative and postoperative platelet count difference can effectively predict the incidence of lower extremity deep venous thrombosis after total knee arthroplasty.When the platelet count difference before surgery ≥34.0 × 109/L,early prevention of deep venous thrombosis should be avoided.The formation of the occurrence.
9.Study on the thickness-change of different thickness thermoplastic materials after thermoforming and saliva immersion.

OBJECTIVETo survey and compare the thickness-change of different thickness thermoplastic materials under different test condition and make sure the relationship between the thickness-change and the material initial thickness in order to provide a guide in selecting the suitable thickness thermoplastic in practice.
METHODSTo choose Biolon, the thickness include 1.0 mm, 0.75 mm, 0.5 mm. Used Electron Vernier caliper to measure the thickness-change of different thickness thermoplastic materials under different processing mode. The data was analyzed by SPSS 10.0.
RESULTSAfter thermoforming the thickness of thermoplastic became thinner, the thickness of Biolon 0.75 mm decreased by 0.14 mm, Biolon 1.0 mm decreased by 0.22 mm and Biolon 0.5 mm decreased by 0.14 mm. After saliva immersion the thickness became thicker. The thickness of Biolon 0.75 mm increased by 0.02 mm, Biolon 1.0 mm increased by 0.03 mm and Biolon 0.5 mm increased by 0.02 mm.
CONCLUSION1)The influence of different processing mode to the thickness-change had relation to the material initial thickness. 2)The Biolon 0.75 mm had certain superiority in thickness stability compared to the homogeneous brand through the above research.

10.A meta-analysis of neoadjuvant chemotherapy combined with trastuzumab for HER2-positive breast cancers.

OBJECTIVETo evaluate the efficacy and safety of neoadjuvant chemotherapy combined with trastuzumab for HER2 positive breast cancers.
METHODSPubMed online database, ASCO abstract database, SABCS abstract database, ESMO abstract database and CBMdisc database were searched for literatures related to trastuzumab in neoadjuvant chemotherapy for breast cancers. A meta-analysis was performed for retrieved literatures meeting the inclusion criteria.
RESULTSThree clinical trials were included. Meta-analysis showed that compared to chemotherapy only, regimens combined with trastuzumab could significantly improved the pCR rate of HER2-positive breast cancers (RR=1.65, 95% CI 1.28-2.13, P<0.0001) without increasing the frequencies of cardiac toxicity (RR=1.16, 95% CI 0.82-1.64, P=0.41).
CONCLUSIONIn neoadjuvant chemotherapy for HER2-positive breast cancers, chemotherapy combined with trastuzumab is superior to exclusive chemotherapy.
