1.Current survey of readiness for hospital discharge of patients who had undergone spinal operation

Objective To investigate the readiness for hospital discharge of patients who had undergone spinal operation at West China Hospital, Sichuan University, to guide the medical care and health education. Methods A total of 164 patients after spinal operation who have readying to discharge were selected and investigated by General Information Questionnaire, Activity of Daily Life Scale and Readiness for Hospital Discharge Scale. Apply the Excel for data inputting, SPSS19.0 for statistical analysis and choose rate, x±s and so on for statistical description according to the types of data. Results Of all the 164 patients, 47 cases (28.7%) weren′t well prepared, 117 cases (71.3%) were well prepared;the score of
Readiness for Hospital Discharge Scale was (142.38±34.23 points and the average score of all the articles was (7.12±1.72) points. Conclusions The readiness for hospital discharge score of patients who had undergone a spinal operation at West China Hospital, Sichuan University is low and their readiness for hospital discharge is inadequate. Medical staff can improve the patients′readiness for hospital discharge by strengthening health guidance in order to promote the recovery of patients.
5.Detection of sICAM-1,sVCAM-1,TNF-?,TNFR-Ⅰ and TNFR-Ⅱ in vasculitis

Objective To study the relationship between sICAM 1,sVCAM 1,TNF ?,TNFR Ⅰ,TNFR Ⅱ and pathogenesis of vasculitis.Method The serum levels of TNF ?,TNFR Ⅰ,TNFR Ⅱ,sICAM 1 and sVCAM 1 were detected by ELISA and their levels between patients with vasculitis and controls were compared.Results Compared with the controls,the serum level of TNF ? was lower,the levels of TNFR Ⅰ,sICAM 1 and sVCAM 1 were higher in the patients with vasculitis,and there was no difference of TNFR Ⅱ between the two groups.Conclusion The low level of TNF ? suggests that this factor may not play a role in these patients,or its role was counteracted by its receptors—sTNFR,whose level was higher in patients with vasculitis.sICAM 1 and sVCAM 1 may play a significant role in the pathogenesis of vasculitis.
6.Balloon dilation of ureter through ureteroscopy for ureter stenosis

ObjectiveTo assess the effect of balloon o f ureter through ureteroscopy for management of ureter stenosis.Met hods42 cases ureter stenosis underwent balloon dilation of ureter,2 2 being male,20 being female.The average age was 46 years.There was 5 ureteropel vic junction stenosis,8 upper ureter stenosis,5 middle part stenosis,24 lower pa rt stenosis.The length of stenosis was from 0.5 cm to 2.0 cm.Result sThe balloon dilating catheter was passed over the ureter stenosis area and was put in proper position in 39 cases.3 cases were failed(7.2%). 37 ca ses successfully underwent balloon dilation of the ureter on one session.All pat ients were followed up for 6 to 24 months(mean 14 months).The degree of hydronep hrosis decreased with no recurrence in 37 cases(88.1%).2 casese failed to gain the effect after the double-J stent was removed(4.7%). There are no serious co mplication found in all patients.ConclusionsBalloon di lation of ureter offers many advantages,such as:good result,repeatable and reduc ing the cost and hospital stay.
7.Regulation effect of insulin on the hypermetabolism of the skeletal muscles of septic patients through ubiquitin-proteasome pathway

Hypermetabolism syndrome,especially in skeletal muscles,is exceedingly common in septic patients.Many studies suggest that the activity of ubiquitin-proteasome pathway is a vital procedure for hypermetabolism of skeletal muscles of septic patients and its activation can be influenced by a variety of factors,such as proinflammatory cytokine and glucocorticoid,etc.Evidences indicate that insulin has played an important role in opsonizing the activation of ubiquitin-proteasome pathway.This paper reviews the regulation effect of insulin to the hypermetabolism of the skeletal muscles of septic patients through ubiquitin-proteasome pathway.
8.Clinical application value of the measurement of thyroid function during early pregnancy

[Objective]To study the clinical application value of the measurement of thyroid function during early pregnancy.[Methods] Six hundred and thirteen women in early pregnancy(observation group)accepted thyroid function lest.Patients with abnormal thyroid function received therapeutic intervention.The results were compared with 586 pregnant women(control group)who didn't get thyroid function screening and treatment.[Results]There were 182 cases(29.69%)out of 613 pregnant women with various thyroid diseases.The incidences of fetal distress,premature rupture of membrane,gestational hypertension and neonatal deformity in observation group were significantly lower than those in control group[3.92%(24/613)vs.7.34%(43/596),5.06%(31/613)vs.12.97%(76/586),2.28%(14/613)vs.5.46%(32/586),1.63%(10/613)vs.5.63%(33/586),P<0.01].[Conclusion] Thyroid function detection and therapeutic intervention during early pregnancy has important clinical value.
9.The significance of lactate dehydrogenase in the diagnosis and treatment of children with refractory Mycoplasma pneumoniae pneumonia

Objective To investigate the significance of lactate dehydrogenase(LDH) in the diagnosis and treatment of children with refractory Mycoplasma pneumoniae pneumonia(RMPP).Methods This was a study of children with Mycoplasma pneumoniae pneumonia(MPP) admitted to hospital from June 2013 to June 2016.All patients were treated with erythromycin.Based on the definition of RMPP,subjects were divided into usual MPP group and RMPP group.The children in RMPP group were treated with erythromycin combined with methylprednisolone.The indicators including LDH,WBC counts,C-reactive protein,erythrocyte sedimentation rate(ESR),alanine aminotransferase(ALT),aspartate amino ransferase(AST) and creatine kinase were analyzed between RMPP group and usual MPP group.The differences of the above indicators between the effective and ineffective group before and after methylprednisolone treatment were also compared.Results In total,253 subjects were enrolled,including 161 in the usual MPP group and 92 in the RMPP group.The average age in RMPP group was older than that in the usual MPP group.The indicators(including LDH,AST,ALT,and ESR) of RMPP group were higher than those in the usual MPP group.Logistic regression showed that LDH and ESR were the significant factors in predicting RMPP.The odds ratio(OR) of LDH was 1.029 with 95%CI 1.020-1.037,and the OR of ESR was 1.063 with 95%CI 1.009-1.120.Receiver operating characteristic curve analysis showed that the area under the curve of LDH(400.50U/L) was the largest(0.959,95%CI 0.936-0.983).The administration of methylprednisolone to patients in the RMPP group resulted in the rapid improvement of symptoms and decrease in serum LDH and ESR levels in effective group(P<0.05).Conclusion Serum LDH may be used as a biomarker to predict RMPP at the early stage,also may be an important marker for the evaluation of therapeutic efficacy in RMPP.
10.Effect of alprostadil injection on serum TNF-α,β2-MG and sIL-2 R in adjunctive treatment of severe hepatitis with liver and kidney syndrome

Objective To investigate the effect of alprostadil injection on serum tumor necrosis factor ( TNF-α) ,β2-microglobulin (β2-MG) and soluble interleukin 2 receptor (sIL-2R) levels in adjunctive treatment of patients of severe hepatitis with liver and kidney syndrome.Methods A total of 60 patients of severe hepatitis with liver and kidney syndrome were collected and randomly divided into experimental group and control group with 30 cases in each group.The two groups were treated by conventional therapy, patients in the control group were treated by continuous renal replacement therapy, patients in the experimental group were treated on the basis of the control group with alprostadil injection in adjuvant therapy for 3 weeks.The serum biochemistry and liver and kidney function and clinical curative effect were detected.Results Compared with the control group, the serum TNF-α, β2-MG and sIL-2R levels of the experimental group were lower (P<0.05); alanine aminotransferase (ALT), aspartate aminotransferase (AST) and total bilirnbin (TBiL) levels were lower (P<0.05), albumin (Alb) level and prothrombin time activity (PTA) were higher (P<0.05), serum creatinine (Scr), urea nitrogen (BUN), CysC and 24h urine protein were lower (P<0.05), 24h urine volume was higher (P<0.05); efficacy in experimental group was better than control group (Z=-2.321,P=0.020).Conclusion Alprostadil injection in adjunctive treatment of severe hepatitis with liver and kidney syndrome patients has significant effect and high safety, could significantly reduce the serum TNF-α, β2-MG and sIL-2R levels.
