2.Study on incidence and risk factors associated with postpartum depression in rural and nursing intervention measures

Objective To investigate the factors affecting rural women suffering from postpartum depression in order to provide scientific basis for rational intervention.Methods 368 rural pierperas whose deliveries were in the First Affiliated Hospital of Shanxi Medical University were surveyed by trained professional nurses with the Edinburgh Postnatal Depression Scale from May 1,2013 to April 30,2014.The data were processed by chi square test and unconditional logistic regression analysis to find the independent risk factors associated with postpartum depression.Results There were 347 valid questionnaires.The incidence of depression after delivery was 25.1% (87/347).Logistic regression analysis showed that the low income family [OR=1.982 (1.238-2.562)],the inconformity of actual and expected baby gender [OR=0.465 (0.183-0.759)] and the mother-in-law as caregiver of pierpera [OR=2.459 (1.950-2.913)] were independent risk factors for postpartum depression of rural women,three of them are significant statistically,P<0.05.Conclusions Understanding the risk factors for postpartum depression and being targeted to guide individually can reduce the incidence of postpartum depression.
3.Advances in Studying the Differentiation of Bone Marrow Mesenchymal Stem Cells into Neurons

Bone marrow mesenchymal stem cells(BMSCs) are a population of multipotent cells. Many researchers have studied whether BMSCs could differentiate into neurons in vivo and ex vivo. Although some studies didn't agree with about some chemical induction medium inducing neurons, the application of cell transplantion and gene therapy of BMSCs has achieved certain progress showing good future for BMSCs in therapy of nervous system diseases.
4.One cases of cervical esophageal fistula.

Twenty days after the operation of anterior cervical decompression fusion with internal fixation, the fistula was found at the lower end of right neck incision with purulent secretion, the intumescent mucosa was founded at the posterior wall of the esophagus, down about 5 cm of the oesophagostomum. The metal plate sample exposed under the intumescent mucosa. The diagnosed was "neck esophageal fistula".

5.The effects of different treatments of tooth extraction wound on alveolar bone healing

Bilateral mandibular first molars of 36 rabbits were extracted.The tooth extraction wounds were treated by:platelet-rich fibrin +acellular dermal matrix(group A),platelet-rich fibrin(group B),acellular dermal matrix (group C) and without treatment (group D,the control) (n =9) respectively.The measurments of alveolar bone height and width showed that there were no significant differences among groups at different times(P > 0.05).Bone histomorphometry showed that at the 2th and 4th week,the best result was found in group A(P<0.01).While,at the 8th week,the result of group A was still better than that of other 3 ones (P < 0.01),but group B and C showed no significant difference(P > 0.05).The combination of PRF and ADM shows the most significant effect.
6.Empirical Study on the Availability of Essential Medicines in Public Health and Medical Institutions of Shang-hai Qingpu District

OBJECTIVE:To investigate the affordability of essential medicines in public health and medical institutions of Shanghai Qingpu district after the implementation of the national essential medicines,and provide the basis for subsequent formula-tion and improvement of relevant policies. METHODS:Based on the WHO/HAI standardized approach,a survey on the affordabili-ty of 30 essential medicines was conducted in public health and medical institutions of Shanghai Qingpu district in aspects of equip-ping staffing,drug availability and the relativity in different levels of public health and institutions for investigation and evaluation. RESULTS:The equipping rate of 30 essential medicines in Shanghai Qingpu District varied from 53.33%-86.67%,while the date of original drugs varied from 16.67%-53.33%,the equipping rate in secondary and tertiary hospitals was high. The availability of 30 essential medicines with Approved Drug Names in secondary and tertiary hospitals (77.78%) was generally higher than that in community health service center(63.00%),while the availability of original drugs was relatively low. The essential medicines with more than 50% relativity in community health service center and tertiary hospitals were only 6 kinds of drugs. CONCLUSIONS:The equipping rate and availability of essential medicines in primary health and medical institutions are lower than that of second-ary and tertiary hospitals in Qingpu district,as well as the relativity. It is suggested to further improve equipping policy for essential medicines from the relativity to improve match rate of production place;while further study and improvement for generic drugs from bioequivalence to improve its bioequivalence with original drugs and ensure clinical efficacy.
7.EFFECTS OF ENDOMORPHINS AND THEIR ANALOGS ON CARDIOVASCULAR SYSTEM

AIM To analyze the effects of endomorphins (EMs) and their analogs ([D-Pro2]EM-1, [D-Ala2]EM-1, [D-Pro2]EM-2 and [D-Ala2]EM-2) on the cardiovascular system of anesthetized rats and to study its mechanism. METHODS Responses to EMs and their analogs were investigated in the systemic vascular bed of rats and the mesenteric microarteria of Bufo gargarizans. Responses to EMs were investigated on the hindquarters of the rat vascular bed under constant-flow conditions and on the isolated rabbit thoracic aorta strips. RESULTS The EMs and their analogs showed dose-related (10-9-10-6 mol*L-1, iv) and naloxone-sensitive (2 mg*kg-1, iv) hypotension in mean arterial pressure of rats with similar duration and vasodilatation in mesenteric microarteria of Bufo gargarizans. The sequence of potencies was EMs > [D-Pro2]EM-2 > [D-Ala2]EM-2 > [D-Ala2]EM-1 > [D-Pro2]EM-1. EMs were shown not to relax the isolated rabbit thoracic aorta strips without endothelium. EMs, however, relaxed them with endothelium significantly. This action was blocked by Nx (10-5 mol*L-1) and L-NNA (10-4 mol*L-1). CONCLUSION The significant hypotensive activity of EMs and their analogs is mainly associated with their vasodilatation, which is related to the release of NO from vascular endothelium, and their potency is not completely related to their affinity for μ-opiate receptor.
8.Influence of recombinant growth hormone on protein metabolism in chronic abdominal infection

Objectives: To study the ameliorated effect of recombinant growth hormone on protein metabolism in chronic abdominal infection. Methods: 20 patients were divided randomly into two groups. Patients in the control group received TPN only for 10 days, while patients in the GH groups received both rGH(13.5u/d) and TPN for 10 days. Sepsis Score grade and detection of serum Growth Hormone (GH), Insulin like growth factor-1 (IGF-1), and Insulin like growth factor binding protein- 3(IGFBP-3) were carried out on day 0 and day 11, Serum albumin, prealbumin, transferring, fibronectin and C-reaction protein were detected on day 0, day 3, day 7 and day 11, Nitrogen balance and 24-hour urine creatinine secretion were assayed daily. Results: GH, IGF-1 and IGFBP-3 in the GH group on day 11 were significantly higher than those in the GH group on day 0 and those in the control group on day 11, Serum albumin, prealbumin, transferring and fibronectin increased, while C-reaction protein decreased when patients were undertaken GH treatment, Nitrogen balance ameliorated and accumulative total nitrogen balance in the GH group was significantly higher than that in the control group, while 24-hour urine creatinine secretion decreased. Conclusions: Recombinant growth hormone can ameliorate protein metabolism in chronic abdominal infection. IGF-1 plays important role in the ameliorated effect of recombinant growth hormone on protein metabolism in chronic abdominal infection.
9.Progress in the studies of endomorphins

Endomorphins(EMs),two tetrapeptides, were discovered in 1997 and believed to be endogenous ligands of μ-opioide receptor(MOR). They mediated various physiological processes by binding to MOR that couple with G-proteins. Based on our works and literatures, we reviewed the distribution in nerve system, receptor binding properties, analgesis, effects on ion channels, conformation and struction-activity relationship of EMs.

BACKGROUND:Previous in vitro studies mainly focused on morphological and nerve marker aspects in the study of bone marrow mesenchymal stem cells (BMSCs)-derived neuron-like cells,but less focused on the olectrophysiological properties of neuron-like cells following differentiation.OBJECTIVE:To study the electrophysiological changes of differentiation from BMSCs into neuron-like cells after induction by brain-derived neurotrophic factor (BDNF)/basic fibroblast growth factor (bFGF)/alltrans-retinolc acid (AT-RA).DESIGN,TIME AND SETTING:The cytological comparative in vitro study was performed at the Department of Neural Cell Laboratory,Tianjin Huanhu Hospital and College of life Science of Nankai University from June 2005 to October 2007.MATERIALS:Totally 3 male Wistar rats (6-week old,weighing about 160 g) were used in this study.METHODS:BMSCs were cultured and purified by their characteristic of plastic adhesion in vitro,and then induced by BDNF,bFGF and AT-RA,and differentiate into neuron-like cells.Whole-cell patch damp technique was used to detect cell membrane current prior to and 3 days following induction.MAIN OUTCOME MEASURES:MSC phenotype was determined by flow cytometry.Cell morphology was observed under the inverted microscope before and after differentiation.Neuron specific enolase expression was assessed by immunocytochemistry.Whole-cell current results were measured.RESULTS:Flow cytometry results demonstrated that CD90 positive rate (99±3)%,CD31 (3.4±0.8)%,and CD34 (0.3±0.1)%.This indicated that these cells were undifferentiated stem cells,with purity of 95%.Undifferentiated MSCs under the optical microscope were mostly fiat cells with processes,similar to fibroblast-like cells.Neuron-like cells appeared 3 days following induction.Immunocytochemistry results showed that MSCs before induction were weakly positive for neuron specific enolase,but strongly positive for neuron specific enolase.At 72 hours,the differentiated rate was (24.01±3.76)%.The peak currents of outward currents in neuron-like cells were significantly higher in the induction group compared with the control group (P < 0.05),but no inward Na~+ current was detected.CONCLUSION:(bFGF & BDNF)/AT-RA could induce the differentiation of MSCs into neuron-like cells.These cells showed the tendency to differentiate into mature neurons,though having no electrophysiological properties of mature neurons.
